The effect of foot deformities on the interplay of forces within the foot: An analysis of multi-segment foot joint moments in cerebral palsy.

BACKGROUND: Foot deformities are common in cerebral palsy (CP) and are likely caused by a disturbed interplay of forces in the foot. Evaluation of foot joint moments would therefore be highly relevant. However, kinetic foot models have not previously been applied to children with CP. RESEARCH QUESTION: What is the difference in multi-segment foot joint moments between children with CP with foot deformities and children with typically developing (TD) feet? METHODS: Children with spastic paresis were retrospectively included and compared to TD children. All children underwent clinical gait ana…

Surgical Strategies for Cleft Foot Deformity in a Pediatric Population.

BACKGROUND: Ectrodactyly (cleft foot) is a rare congenital deformity characterized by central ray deficiency. Surgical intervention is considered for pain, shoe-wear difficulty, callosities, or cosmetic concerns. This multicenter study evaluates indications, surgical strategies, radiographic correction, and incorporates intra- and inter-rater reliability testing of the Blauth and Abraham classification systems. METHODS: A retrospective multicenter review was conducted of patients with ectrodactyly treated operatively between 2010 and 2022. Demographic, clinical, and radiographic data were col

What Are the Updates on Epidemiology of Progressive Collapsing Foot Deformity?

Progressive collapsing foot deformity is one of the most controversial topics in foot and ankle surgery. Much research has been done regarding anatomy, biomechanics, and etiology behind this complex deformity and there is interest in studying metabolic or genetic conditions that could influence the development of this multifactorial disorder. Relevant anatomy includes osseous and soft tissue structures. Several risk factors like obesity, genetics, and flat foot during childhood have been proposed in literature. It occurs 3 times more often in women, the peak incidence happening at age 55, and

Mechanism of orthotic therapy for the painful cavus foot deformity.

BACKGROUND: People who have extremely high arched feet or pes cavus often suffer from substantial foot pain. Custom-made foot orthoses (CFO) have been shown to be an effective treatment option, but their specificity is unclear. It is generally thought that one of the primary functions of CFO is redistributing abnormal plantar pressures. This study sought to identify variables associated with pain relief after CFO intervention. METHODS: Plantar pressure data from a randomized controlled trial of 154 participants with painful pes cavus were retrospectively re-analyzed at baseline and three mont

Correlation of radiographic and pedobarograph measurements in planovalgus foot deformity.

Planovalgus foot deformity is common in children with cerebral palsy. Several pathologies contribute to the deformity. It begins with the lateral displacement of the navicular and the talar head becomes uncovered and prominent in the medial side of the midfoot. The purpose of this study was to assess the correlation between the radiographic and the pedobarographic measurements and the ability to predict foot pressure components using radiographic measurement. The patient sample included 43 patients with cerebral palsy who were ambulatory and had planovalgus foot deformity (76 feet). Medial mid

Ilizarov treatment for equinoplanovalgus foot deformity caused by melorheostosis.

Melorheostosis often is disabling because of progressive contracture of the joint and soft tissue involved. Operative intervention often is hazardous and patients also have recurrences. A 14-year, 2-month-old boy, who presented with a recurrent equinoplanovalgus deformity of the right foot caused by melorheostosis, was treated successfully using the Ilizarov technique, including distraction osteotomy in the calcaneus. The patient had a painless, plantigrade, and functional foot at age 18 years.

Review of the effect of early amniocentesis on foot deformity in the neonate.

A recent publication by the Canadian Early and Mid-Trimester Amniocentesis Trial Group reported an increase in orthopaedic foot deformities in infants whose mothers underwent an amniocentesis from 11 to 12 + 6 gestational weeks versus a group sampled between 15 to 16 + 6 gestational weeks. Because the sampling of the former group was at the time of maximum foot growth and maximum acquisition of amniotic fluid, the authors hypothesize that the foot deformities are secondary to decreased fetal movement during a key phase in foot and ankle development.

Evaluation of the deformity in club foot by somatosensory evoked potentials.

Somatosensory evoked potentials (SSEPs) measure the conduction pathways from the periphery to the brain and can demonstrate the site of neurological impairment in a variety of locomotor conditions. SSEPs were studied in 44 children (64 feet) with surgically corrected club feet. Four children had unreproducible responses, 18 showed abnormal recordings and 22 showed normal responses. In a further 31 feet (21 children) subjected to motor electrophysiological tests, 16 (52%) were abnormal. Overall, 44 of 95 feet (46%) showed abnormal SSEPs or motor electrophysiological tests. Neurological abnormal

Subtalar joint arthrodesis for postpoliomyelitis valgus foot deformity.

The purpose of this study was to evaluate the results of full versus partial subperiosteal fibular bone graft for subtalar joint arthrodesis in patients with pes planovalgus foot deformity associated with residual polio myelitis. A prospective study was carried out on 16 patients with postpoliomyelitis valgus foot deformity secondary to invertor muscle paralysis. All patients were treated by peroneal tendon transfer to the medial metatarsals and subtalar extra-articular arthrodesis. In 12 patients, subperiosteal partial fibular graft was used, while four patients received a full fibular graft.

[Foot deformity as a late consequence of tibial shaft fracture].

Clinical appearance of foot deformity after tibial shaft fracture resulting from the contracture of the deep posterior compartment muscles of the leg is presented in this paper. The contracture may be caused by mechanical damage to the muscles or vessels or by deep posterior compartment syndrome. Cavus deformity, shortening of the foot length, claw big toe and hammer lesser toes are frequently unnoticed at the follow-up examination after surgical treatment of the tibial fracture. These findings are based on analysis of 71 patients treated surgically for leg shafts fracture with AO method.

Hypertension after operative correction of club-foot deformity.

Severe hypertension occurred as a postoperative complication after correction of a club-foot deformity in four children (seven feet) who were between the ages of two and three years and had no history of hypertension. The hypertension subsided slowly after administration of antihypertensive medications or more rapidly after release of the correction that had been obtained operatively.

Correction of combined tibial torsion and valgus deformity of the foot.

Patients with spina bifida cystica commonly have significant disability from a combination of valgus deformity of the ankle and subtalar joints with lateral tibial torsion and plano-abduction deformity of the foot. These deformities can be corrected by a single procedure which combines a supramalleolar tibial osteotomy with a lateral inlay triple fusion. This procedure was carried out on 20 feet in 15 patients and the results were reviewed after an average of three years (range 18 months to 7 years). In 75 per cent of feet the combination of deformities was fully corrected, ulcers and callosit

Assessment of foot deformities in individuals with cerebral palsy using weight-bearing CT.

OBJECTIVE: The aims of this study were to visualize and quantify relative bone positions in the feet of individuals with cerebral palsy (CP) with a foot deformity and compare bone positions with those of typically developed (TD) controls. MATERIALS AND METHODS: Weight-bearing CT images of 14 individuals with CP scheduled for tendon transfer and/or bony surgery and of 20 TD controls were acquired on a Planmed Verity WBCT scanner. Centroids of the navicular and calcaneus with respect to the talus were used to quantify foot deformities. All taluses were aligned and the size and dimensions of the…

Addressing the Data Acquisition Paradigm in the Early Detection of Pediatric Foot Deformities.

The analysis of plantar pressure through podometry has allowed analyzing and detecting different types of disorders and treatments in child patients. Early detection of an inadequate distribution of the patient's weight can prevent serious injuries to the knees and lower spine. In this paper, an embedded system capable of detecting the presence of normal, flat, or arched footprints using resistive pressure sensors was proposed. For this purpose, both hardware- and software-related criteria were studied for an improved data acquisition through signal coupling and filtering processes. Subsequent

Weightbearing Computed Tomography for Assessment of Foot and Ankle Deformities: The Iowa Experience.

BACKGROUND: Weightbearing computed tomography (WBCT) is a reliable and precise modality for the measurement and analysis of bone position in the foot and ankle, as well as associated deformities. WBCT to assess three dimensional relationships among bones allowed the development of new measurements, as the Foot and Ankle Offset (FAO), which has high inter and intra-rater reliability. This study reports the University of Iowa's experience utilizing WBCT for the care of foot and ankle patients by describing its utility across different orthopedic diseases in improving diagnostic assessment, aidin

Distraction osteogenesis for complex foot deformities: Gigli saw midfoot osteotomy with external fixation.

Open midfoot wedge osteotomy correction can cause neurovascular compromise, requires extensive exposure, sacrifices normal joints, and shortens the foot. We used a minimally invasive technique to treat complex foot deformities by combining percutaneous Gigli saw midfoot osteotomy, circular external fixation, and acute, gradual, or gradual with acute manipulation correction. The medical records of 23 patients (26 feet) with complex foot deformities (congenital, 18 feet; neuromuscular, 4 feet; post-traumatic, 3 feet; malunion, 1 foot) who had undergone treatment within an 18-year period (1990 th…

Ilizarov external fixation in the correction of severe pediatric foot and ankle deformities.

Most of the evidence to date on the Ilizarov method in the management of complex foot and ankle deformities in children is based on expert opinion and retrospective case series. Often the technique is used as a salvage option where conventional techniques are inappropriate or have failed. The decision to use the Ilizarov external fixator to an alternative technique depends on several issues: complexity of the pathology, patient compliance, surgeon skills, and the capacity of the institution to manage patients with multidisciplinary requirements. Nevertheless, the Ilizarov method has proved to
