Onychomycosis: Diagnosis and management.

Onychomycosis is a common nail ailment associated with significant physical and psychological morbidity. Increased prevalence in the recent years is attributed to enhanced longevity, comorbid conditions such as diabetes, avid sports participation, and emergence of HIV. Dermatophytes are the most commonly implicated etiologic agents, particularly Trichophyton rubrum and Trichophyton mentagrophytes var. interdigitale, followed by Candida species and non dermatophytic molds (NDMs). Several clinical variants have been recognized. Candida onychomycosis affects fingernails more often and is accompan

Therapies for onychomycosis: a review.

Onychomycosis, a chronic fungal infection of the nail, can be treated using various modalities. Surgical, chemical, topical, and oral methods are reviewed in this article, with an emphasis on the three systemic treatments approved by the US Food and Drug Administration: terbinafine, itraconazole, and griseofulvin.

Hand-foot syndrome in a patient with multiple fire ant stings.

The case is reported of an elderly patient with known previous exposure to fire ant stings, and who presented with hand-foot syndrome (HFS) in the setting of multiple fire ant stings to the lower extremities. Both hands and both feet were red, swollen, and mildly tender. Treatment was with fluocinonide cream, and all symptoms resolved as the classic fire ant skin lesions regressed. HFS was initially reported in association with acute crisis in sickle cell anemia and thalassemia and more recently as a common toxicity of chemotherapy administration. This is the first report of its occurrence in

[Why should we treat onychomycosis with patience and perseverance? How can we overcome therapeutic failure?].

Antifungal treatments for onychomycoses may be long and expensive without, however, affording the guarantee of success. A review of the literature reveals that the current topical treatments and the oral intake of fluconazole are of long duration. They show moderate clinical efficacy and/or a pharmaco-economic profile less favorable than those of intermittent itraconazole therapy and terbinafine continuous therapy. Between the latter two options, itraconazole has some advantages because of its broad spectrum of activity against diverse types of fungi responsible for onychomycoses. The fungicid

The frequency of Candida parapsilosis in onychomycosis. An epidemiological survey in Israel.

Candida albicans is regarded as the major pathogen in yeast-induced onychomycosis. Based on our impression of an increasing prevalence of Candida parapsilosis in this disease, we examined the data of two mycology laboratories in the same geographic location, from 1994 to 1996 in one (centre A) and for 1995 (6 months) in the other (centre B). A total of 954 and 230 toenails and 621 and 190 fingernails, respectively, underwent KOH microscopy and culture studies in each centre. Positive findings were noted in 45 and 65% of the toenails and 44 and 72% of the fingernails, respectively. In the toena

Bullous pemphigoid in infancy: case report and literature review.

We report a 2-month-old girl who developed bullous pemphigoid on her hands and feet shortly after receiving her routine immunizations. Infantile bullous pemphigoid has a clinical presentation distinct from bullous pemphigoid seen in older children and should be included in the differential diagnosis of blisters involving the hands and feet. Our patient responded well to topical corticosteroid therapy.

[Chronic osteitis and arthritis of palmoplantar pustulosis. A familial form of B-27 negative spondylarthropathy].

A 35-year old woman presented with pustulosis palmaris et plantaris evolving in acute episodes parallel to those of a rheumatic disease consisting of bilateral sacro-iliac arthritis, manubriosternal fusion and sterno-clavicular arthritis. Her brother had spondylitis compatible with the diagnosis of pustular osteo-arthritis though without pustulosis. He presented with bilateral sacro-iliac arthritis, manubriosternal fusion, sterno-clavicular arthritis and an ossified fragment detached from the antero-inferior angle of C6. The two patients had the same HLA phenotype: A1, A9, B5, B41, CW4, DR2, D

[Reflections on the treatment of acropathia ulcerans et mutilans; interest of a removable appliance (author's transl)].

The authors report their experience in the treatment of acquired ulcerating and mutilating trophic lesions of the lower limbs. Ligature of the dorsalis pedis artery and of its collaterals did not give good results, according to the pathogenic concept of the authors, in which lesions of the nervous system predominate over vascular alterations. More important lesions (such as osteo-arthritis of the first and fifth metatarsals) require immobilization of the axis of the wounded toe, control of any superinfection and use of a plastic removable limb appliance.
