Intracranial meningioma at foramen magnum presenting as bow hunter's syndrome via V4 segment compression: illustrative case.

BACKGROUND: Bow hunter's syndrome (BHS) occurs when physiological head rotation or extension leads to dynamic vertebral artery (VA) occlusio … ral position. MRI demonstrated a homogeneously enhancing intradural extramedullary mass at the left foramen magnum displacing the VA and compressing the cervicomedullary junction. The patient underwent midline suboccipital craniotomy, C1 laminectomy, and partial occipital condylectomy for microsurgical resection. Gross-total removal was achieved, decompressing t …

Changes in venous drainage after posterior cranial vault distraction and foramen magnum decompression in syndromic craniosynostosis.

OBJECTIVE: The authors' objective was to measure the effect of posterior cranial vault distraction (PCVD) plus foramen magnum decompression (FMD) on dural sinus volume and venous flow in patients with syndromic craniosynostosis. METHODS: The volumes of the sagittal, straight, transverse, and sigmoid sinuses of 5 consecutive patients with syndromic craniosynostosis who underwent PCVD+FMD were calculated in cubic centimeters with T2-weighted volumetric MRI sequences before surgery, immediately after surgery, and after the end of the distraction process. Tridimensional reconstructions of phase-c…

Key Aspects in Foramen Magnum Meningiomas: From Old Neuroanatomical Conceptions to Current Far Lateral Neurosurgical Intervention.

Foramen magnum meningiomas represent a challenge for neurosurgeons. These tumors require careful surgical manipulation as they are often located in proximity to critical neurovascular structures and the cranial nerves. The far lateral approach is considered the safest neurosurgical approach for excising foramen magnum lesions. It facilitates the access to the anterior foramen magnum and reduces the retraction of vital structures. We describe key historical, epidemiological, genetic, epigenetic, clinical, and neurosurgical aspects of foramen magnum meningiomas. We emphasize the far lateral appr…

Extensive spinal cord oedema and early syrinx formation due to tension pseudomeningocele after foramen magnum decompression.

Pseudomeningoceles can cause syringomyelia. However, this is the first case report to show an unusually acute complication of a post-operative pseudomeningocele: spinal cord oedema and early syrinx, treated favourably with an early surgical intervention.

Long-lasting improvement of arterial hypertension after surgical treatment of a foramen magnum meningioma: case report.

BACKGROUND: Neurogenic arterial hypertension has been proposed to be caused by neurovascular compression in many cases. However, there is little reference to tumors causing hypertension by local compression of the vagal nerve or the ventrolateral medulla oblongata. The following case illustrates the effects of surgery for a meningioma of the foramen magnum on arterial hypertension. CASE DESCRIPTION: A 54-year-old woman suffered from arterial hypertension for at least 7 months, for which she required a combined medical treatment regime. She suffered for 6 months from dizziness and tinnitus, mo…

Cerebral oedema following EVD insertion for delayed hydrocephalus after foramen magnum decompression in Chiari I malformation.

CASE REPORT: A case of Chiari I malformation is reported, in which foramen magnum decompression was followed by delayed onset of acute hydrocephalus after a week. Initial treatment of the hydrocephalus was further complicated by severe cerebral oedema. DISCUSSION: A short survey of the literature suggests that both complications are extremely uncommon. The possible mechanism of development of cerebral oedema through stretch of the cerebral vasculature before ventricular decompression is discussed. This report further adds to the list of complications following surgery for Chiari malformations…

Microsurgical resection of foramen magnum meningioma: multi-institutional retrospective case series and proposed surgical risk scoring system.

PURPOSE: Foramen magnum meningiomas (FMMs) are a major surgical challenge, due to relevant surgical morbidity and mortality. The paper aims to review the clinical (symptomatic improvement, complication rate, length of hospital stay) and radiological outcome (completeness of resection) of microsurgical resection of FMMs, and to identify predictors of complications. METHODS: A multi-institutional retrospective review of prospectively maintained database of FMMs included 51 patients (74.5% females) with a median tumor volume of 8.18 cm3 (range, 1.77-57.9 cm3) and median follow-up of 36 months (r…

Gamma Knife radiosurgery associated worsening of superficial siderosis due to a foramen magnum tumor - A case report.

Superficial siderosis (SS) of the brain results from a chronic iron toxicity due to repeated microscopic leakage of blood products into the subarachnoid space. We report on Gamma Knife Radiosurgery (GKRS) associated worsening of superficial siderosis in a patient with skull-base tumor. A 73 year-old male patient presented with clumsiness and gait ataxia and was diagnosed with foramen magnum meningioma. He was also noted to have superficial siderosis involving the mainly the infratentorial compartment. After a thorough evaluation of craniospinal axis, no other cause of bleeding was identified. …

Midline Suboccipital Subtonsillar Approach with C1 Laminectomy for Resection of Foramen Magnum Meningioma: 2-Dimensional Operative Video.

We present a case of a medium-sized foramen magnum meningioma that was resected through a midline suboccipital subtonsillar approach with C1 laminectomy in prone (Concorde) position. The patient is a 77-year-old woman with a 6-month history of intermittent vertigo, moderate gait instability, and slight decline of memory. On magnetic resonance imaging (MRI) an extra-axial intradural lesion was discovered that originated from the right epicondylar region just inferior to the jugular tubercle and occupied the anterolateral aspect of the foramen magnum. There was moderate transposition and compres…

Foramen magnum decompression in infants and children with achondroplasia: clinical outcomes from a 15-patient surgical series.

BACKGROUND: Achondroplasia (ACH) is the most common skeletal dysplasia characterized by disproportionate short stature due to impaired endochondral ossification. One of the most critical and potentially fatal complications of ACH is foramen magnum and upper cervical canal stenosis. Compression at the cervicomedullary junction may lead to myelopathy, hypotonia, developmental delay, and central sleep apnea. Early detection and timely surgical intervention are essential to prevent permanent neurological injury. METHODS: This retrospective study evaluated 15 pediatric patients with ACH (9 girls, …

A comparative analysis of neuroendoscopic foramen magnum decompression versus traditional open surgery for Chiari Malformation Type I.

PURPOSE: Chiari Malformation Type I (CM1) is characterized by the downward displacement of the cerebellar tonsils below the foramen magnum. The standard surgical treatment for CM1 is foramen magnum decompression and atlas laminectomy (FMD-AL). However, there is a growing interest in exploring minimally invasive techniques, such as neuroendoscopically assisted FMD-AL, to optimize surgical outcomes. The aim is to present the results of the less invasive neuroendoscopic-assisted system application as an alternative to decompression surgery in patients with CM-1 with/without syringomyelia. PATIEN…

How I do it: Surgical interruption of high-flow dural arteriovenous fistulas at the foramen magnum region.

BACKGROUND: Dural arteriovenous fistulas (dAVFs) in the foramen magnum region (FMR) are rare entity of dAVFs. There is no established treatment for FMR-dAVFs owing to their rarity and anatomical complexity. Herein, we report cases of high-flow dAVFs located at the posteromedial part of the FMR that were successfully treated by surgical interruption. METHODS: We demonstrated the surgical procedures for the interruption of high-flow FMR-dAVF with representative images and videos. In both cases, endovascular transarterial embolization was performed prior to surgical interruption. CONCLUSION: As…

Morphology of the Occipital Bones and Foramen Magnum Resulting From Premature Minor Suture Fusion in Crouzon Syndrome.

To identify skull-base growth patterns in Crouzon syndrome, we hypothesized premature minor suture fusion restricts occipital bone development, secondarily limiting foramen magnum expansion. Skull-base suture closure degree and cephalometric measurements were retrospectively studied using preoperative computed tomography (CT) scans and multiple linear regression analysis. Evaluation of multi-institutional CT images and 3D reconstructions from Wake Forest's Craniofacial Imaging Database (WFCID). Sixty preoperative patients with Crouzon syndrome under 12 years-old were selected from WFCID. The c…

Foramen Magnum Dural Arteriovenous Fistula Presenting With Thoracic Myelopathy: Technical Case Report With 2-Dimensional Operative Video.

BACKGROUND AND IMPORTANCE: Dural arteriovenous fistulas (dAVFs) are vascular abnormalities of the central nervous system that can cause a wide array of neurological dysfunction depending on their location, flow, and propensity to rupture. Symptomatic dAVFs at the cranio-cervical junction usually result in hemorrhage or cervical myelopathy. Distantly located dAVFs of the foramen magnum are a rare cause of thoracic intrinsic myelopathy. CLINICAL PRESENTATION: An 83-yr-old man presented with progressive lower extremity weakness, numbness, and difficulty walking along with episodes of bowel incon…

Far Lateral Transcondylar Transtubercular Approach for Microsurgical Resection of Foramen Magnum Meningioma: Operative Video and Technical Nuances.

Surgical access to the ventral foramen magnum remains a technical challenge. With large lesions in this region compressing the brainstem and distorting the regional neurovascular relationships, formulating a surgical plan and its appropriate execution have crucial importance in achieving favorable outcomes. While the endoscopic endonasal approaches have gained increasing attention to access the clivus and the ventral brainstem, foramen magnum meningiomas are still preferred to be removed via an approach that obviates a trajectory through the nasopharyngeal mucosa. Therefore, the far lateral ap…

Iatrogenic Acute Ischemic Necrosis Due to Emergent Bleeding Control in Ventral Foramen Magnum Meningioma with Spinal Instability.

Meningiomas are the most common benign intracranial tumors. They often require surgical resection and postoperative radiation/chemotherapy b … ypes are also hallmarks of a higher-grade disease. We describe a case of a man with a large ventral foramen magnum meningioma who underwent temporary intraoperative occlusion of the vertebral artery, leading to ischemic tumor necrosis with abundant neutrophil invasion when the tumor was eventually examined histologically.
