[Role of preoperative visualization in the choice of surgery for primary hyperparathyroidism].

BACKGROUND: Precise localization of abnormal parathyroid glands is important for a successful surgery for primary hyperparathyroidism (PHPT). While a large number of patients can be successfully treated with the focused parathyroidectomy, there is a considerable rate of the persistent PHPT mostly because of undetected multiglandular disease (MGD). AIM: The aim of the study was to evaluate the meaning of preoperative visualization data for planning the surgery for patients with PHPT. MATERIALS AND METHODS: The study was conducted at SPBU Hospital in 2017-2018. 810 patients who underwent a pri

Light-sheet Fluorescence Microscopy to Capture 4-Dimensional Images of the Effects of Modulating Shear Stress on the Developing Zebrafish Heart.

The hemodynamic forces experienced by the heart influence cardiac development, especially trabeculation, which forms a network of branching outgrowths from the myocardium. Genetic program defects in the Notch signaling cascade are involved in ventricular defects such as Left Ventricular Non-Compaction Cardiomyopathy or Hypoplastic Left Heart Syndrome. Using this protocol, it can be determined that shear stress driven trabeculation and Notch signaling are related to one another. Using Light-sheet Fluorescence Microscopy, visualization of the developing zebrafish heart was possible. In this manu

TIPS bilateral noise reduction in 4D CT perfusion scans produces high-quality cerebral blood flow maps.

Cerebral computed tomography perfusion (CTP) scans are acquired to detect areas of abnormal perfusion in patients with cerebrovascular diseases. These 4D CTP scans consist of multiple sequential 3D CT scans over time. Therefore, to reduce radiation exposure to the patient, the amount of x-ray radiation that can be used per sequential scan is limited, which results in a high level of noise. To detect areas of abnormal perfusion, perfusion parameters are derived from the CTP data, such as the cerebral blood flow (CBF). Algorithms to determine perfusion parameters, especially singular value decom

Kinematic tracking of the small bones of the wrist in sequential 3DCT and dynamic 4DCT volume images using open-source Hierarchical 3D Registration, a module within SlicerAutoscoperM.

Quantitative kinematic tracking of in vivo skeletal structures efficiently and with high temporal resolution is important to a wide range of questions in biomechanical research. As such, software tools are needed for efficient, semi-automated registration of bones imaged with either 3DCT or 4DCT. This study presents Hierarchical 3D Registration (3DH), an open-source approach to skeletal tracking of three-dimensional image volume sets. The 3DH approach and software are presented using sequential 3DCT datasets of participants performing various thumb tasks and dynamic 4DCT datasets of simulated

Motion compensated cone-beam CT reconstruction using ana priorimotion model from CT simulation: a pilot study.

Objective. To combat the motion artifacts present in traditional 4D-CBCT reconstruction, an iterative technique known as the motion-compensated simultaneous algebraic reconstruction technique (MC-SART) was previously developed. MC-SART employs a 4D-CBCT reconstruction to obtain an initial model, which suffers from a lack of sufficient projections in each bin. The purpose of this study is to demonstrate the feasibility of introducing a motion model acquired during CT simulation to MC-SART, coined model-based CBCT (MB-CBCT).Approach. For each of 5 patients, we acquired 5DCTs during simulation an

Detailed dosimetric evaluation of inter-fraction and respiratory motion in lung stereotactic body radiation therapy based on daily 4D cone beam CT images.

Objective. Periodic respiratory motion and inter-fraction variations are sources of geometric uncertainty in stereotactic body radiation therapy (SBRT) of pulmonary lesions. This study extensively evaluates and validates the separate and combined dosimetric effect of both factors using 4D-CT and daily 4D-cone beam CT (CBCT) dose accumulation scenarios.Approach. A first cohort of twenty early stage or metastatic disease lung cancer patients were retrospectively selected to evaluate each scenario. The planned-dose (3DRef) was optimized on a 3D mid-position CT. To estimate the dosimetric impact o

Integrated 18 F-FCH PET/4D-CT in a Case of Supernumerary Parathyroid Glandular Hyperplasia With Incidentally Detected Thymic NET.

Less than 10% of primary hyperparathyroidism (PHPT) patients have supernumerary parathyroid glands. In a 52-year-old man with multiple endocrine neoplasia type 1 (MEN 1) syndrome with PHPT, integrated 18 F-Fluorocholine PET/4D-CT (FCH PET/4D-CT) was done for localization of parathyroid adenoma. FCH PET/4D-CT revealed 5 culprit lesions (1 ectopic) along with FCH avid nonenhancing anterior mediastinal mass, possibly NET or thymoma. FNAC of the mass raised a suspicion of NET, which was corroborated by subsequent 68 Ga-DOTANOC PET/CT. Parathyroidectomy for all 5 glands and thymic mass excision was

Real-time lung extraction from synthesized x-rays improves pulmonary image-guided radiotherapy.

Objective.Lung tumors can be obscured in x-rays, preventing accurate and robust localization. To improve lung conspicuity for image-guided procedures, we isolate the lungs in the anterior-posterior (AP) x-rays using a lung extraction network (LeX-net) that virtually removes overlapping thoracic structures, including ribs, diaphragm, liver, heart, and trachea.Approach.73 965 thoracic 3DCTs and 106 thoracic 4DCTs were included. The 3D lung volume was extracted using an open-source lung volume segmentation model. AP digitally reconstructed radiographs (DRRs) of the full anatomy CT and extracted l

Combining Serial and Parallel Functionality in Functional Lung Avoidance Radiation Therapy.

PURPOSE: Functional lung avoidance (FLA) radiation therapy (RT) aims to minimize post-RT pulmonary toxicity by preferentially avoiding dose to high-functioning lung (HFL) regions. A common limitation is that FLA approaches do not consider the conducting architecture for gas exchange. We previously proposed the functionally weighted airway sparing (FWAS) method to spare airways connected to HFL regions, showing that it is possible to substantially reduce risk of radiation-induced airway injury. Here, we compare the performance of FLA and FWAS and propose a novel method combining both approaches

ADAM phantom to test 4D medical imaging and dose delivery devices.

Radiotherapy treatment of moving lesions is a challenging task in which different strategies can be used. The clinical implementation of the variety of complex technical solutions pursued to treat cancer, while sparing healthy tissues, requires accurate quality assurance tests and adequate phantoms. The aim of this work is to report on the anthropomorphic dynamic breathing model (ADAM) and on its applications to test image quality and dose delivery for four-dimensional (4D) techniques used to compensate for breathing-induced tumor motion. ADAM reproduces a male torso containing realistic ribs

Prognostic factors associated with the accuracy of deformable image registration in lung cancer patients treated with stereotactic body radiotherapy.

We evaluated the accuracy of an in-house program in lung stereotactic body radiation therapy (SBRT) cancer patients, and explored the prognostic factors associated with the accuracy of deformable image registrations (DIRs). The accuracy of the 3 programs which implement the free-form deformation and the B-spline algorithm was compared regarding the structures on 4-dimensional computed tomography (4DCT) image datasets between the peak-inhale and peak-exhale phases. The dice similarity coefficient (DSC) and normalized DSC (NDSC) were measured for the gross tumor volumes from 19 lung SBRT patient

Comparative evaluation of a surface-based respiratory monitoring system against a pressure sensor for 4DCT image reconstruction in phantoms.

Four-dimensional computed tomography (4DCT), which relies on breathing-induced motion, requires realistic surrogate information of breathing variations to reconstruct the tumor trajectory and motion variability of normal tissues accurately. Therefore, the SimRT surface-guided respiratory monitoring system has been installed on a Siemens CT scanner. This work evaluated the temporal and spatial accuracy of SimRT versus our commonly used pressure sensor, AZ-733 V. A dynamic thorax phantom was used to reproduce regular and irregular breathing patterns acquired by SimRT and Anzai. Various parameter…

Mild hypercalcemia: an indication to select 4-dimensional computed tomography scan for preoperative localization of parathyroid adenomas.

BACKGROUND: Technetium sestamibi scanning is the most accepted method of imaging used for preoperative localization of parathyroid adenomas. Four-dimensional computed tomography (4D-CT) scanning is a relatively new localization technique that has not been as rigorously evaluated. METHODS: One hundred thirty-five consecutive patients who underwent preoperative sestamibi scanning, 4D-CT scanning, and parathyroidectomy for primary hyperparathyroidism were evaluated. Patient characteristics, parathyroid gland weights, and the probability of having positive preoperative localization were examined.…

Peripheral Vascular Anomalies - Essentials in Periinterventional Imaging.

BACKGROUND: Peripheral vascular anomalies represent a rare disease with an underlying congenital mesenchymal and angiogenetic disorder. Vascular anomalies are subdivided into vascular tumors and vascular malformations. Both entities include characteristic features and flow dynamics. Symptoms can occur in infancy and adulthood. Vascular anomalies may be accompanied by characteristic clinical findings which facilitate disease classification. The role of periinterventional imaging is to confirm the clinically suspected diagnosis, taking into account the extent and location of the vascular anomaly

Effectiveness of 4-dimensional maximum intensity projection (4D-MIP) for respiratory motion management with uncertain interobserver delineation.

This study was conducted to evaluate the use of 4-dimensional (4D) maximum intensity projection (4D-MIP) to compensate for the disadvantages of average intensity projection (AIP), which is used to determine the internal target volume (ITV) in lung tumors. A respiratory motion phantom with a simulated tumor was imaged using 4D computed tomography (4D-CT). AIP and 4D-MIP were generated based on 10 phases of 4D-CT, followed by contouring of the ITVAIP and ITVMIP; these were compared with the ITV contoured in 10 phases of 4D-CT (ITV10). Additionally, the profile curves of the ITVAIP and ITVMIP wer

Fiducial marker position affects target volume in stereotactic lung irradiation.

PURPOSE: Real-time tracking systems of moving respiratory targets such as CyberKnife, Radixact, or Vero4DRT are an advanced robotic radiotherapy device used to deliver stereotactic body radiotherapy (SBRT). The internal target volume (ITV) of lung tumors is assessed through a fiducial marker fusion using four-dimensional computed tomography (CT). It is important to minimize the ITV to protect normal lung tissue from exposure to radiation and the associated side effects post SBRT. However, the ITV may alter if there is a change in the position of the fiducial marker with respect to the tumor. T…

Comprehensive 4D robustness evaluation for pencil beam scanned proton plans.

Due to anticipated clinical benefits, moving targets are potential future indications for pencil beam scanned proton therapy (PBS-PT). However, currently they are not widely treated at PBS-PT facilities due to dosimetric uncertainties caused by motion. We developed a method, the 4D robustness evaluation method (4DREM), to realistically and efficiently assess all possible events impacting PBS-PT treatments in the thorax. Using the 4DREM in large cohorts of lung and oesophageal cancer patients, it will become possible to illustrate, in clinical practice, how to trigger robustness settings for pl

Combined evaluation of blood flow and tissue perfusion in diabetic feet by intra-arterial dynamic 4DCT imaging.

Critical limb ischemia is associated with high mortality and major amputations. Intra-arterial digital subtraction angiography (IADSA) has been the reference standard but has some shortcomings including the two-dimensional projection and the lack of tissue perfusion information. The aim of this exploratory study is to examine four-dimensional computed tomography (4DCT) angiography and perfusion imaging using low-volume intra-arterial contrast injections for an improved anatomic and hemodynamic assessment in patients with foot ulcers. Three patients underwent a low-volume (2 mL) intra-arterial …
