Asymmetric fracture dislocation of the hips: case report.

Bilateral asymmetric hip fracture dislocation is an extremely rare entity. The injury is caused by a high velocity mechanism. We reported a case of bilateral hip fracture dislocation and its management. A 30-year-old man with no previous medical or surgical history was involved in a road accident involving a high-speed collision between two trucks. X-rays of the pelvis revealed asymmetrical bilateral fracture-luxation of the hips. The reduction of the hips was done under general anesthesia. Asymmetrical bilateral traumatic dislocation fracture of the hip is a rare serious injury. Reduction mus…

[Bilateral Dorsal Fracture-Dislocation of the Proximal Humerus].

Fractures of the proximal humerus constitute approximately 5% of all fractures. Shoulder joint injuries without any external mechanical impact during seizures with the occurrence of spasms occur only sporadically. The occurrence rate is reported in approximately 0.4% of patients. Very rarely they occur in the form of epileptic seizure-induced dorsal fracturedislocation impacting both sides. The case report describes a case of a 48-year-old woman with no treatment for epileptic seizures in her medical history. During the first seizure she sustained a bilateral dorsal fracture-dislocation caused

[Trans-scapho-perilunar fracture-dislocation: a case report].

Trans-scaphoid perilunate fractures-dislocations are rare injuries caused by high-energy trauma of the wrist. Diagnosis is based on medical history, physical examination, and tools such as radiographs, computed tomography scan, and magnetic resonance imaging. Early treatment consists of closed reduction and casting to stabilize the limb. Definitive treatment is surgical and includes bone and soft tissue repair. A case of trans-scaphoid perilunate fracture-dislocation is presented, along with diagnosis, management and outcome.

Dislocation fracture of the femoral head in adult.

The smaller the head fragment, the more likely it is located at the bottom and in front of the femoral head, which then determines the most … sthetisation. Avascular necrosis remains a possible complication (8.2%). Sciatic nerve palsy (4% of fracture dislocations) is more common for dislocations associated with posterior wall fractures.

Recognition, Treatment, and Outcome of Calcaneal Fracture-Dislocation.

BACKGROUND: Calcaneal fracture-dislocations are rare but potentially disabling injuries that are regularly overlooked at first presentation. To date, only about 50 cases have been reported in the literature. METHODS: Over a period of 8 years, 10 patients (average age 61.7 years) with acute fracture-dislocations of the calcaneus were treated at 2 level 1 trauma centers. The calcaneocuboid joint was involved in 9 patients. There was a concomitant fracture of the lateral talar process and of the tip of the distal fibula in 7 patients each. Open reduction and internal fixation was performed in 9

Functional Outcomes After Fracture-Dislocation of the Ankle.

Background. Fracture dislocation of the ankle represents a substantial injury to the bony and soft tissue structures of the ankle. There has been only limited reporting of functional outcome of ankle fracture-dislocations. This study aimed to compare functional outcome after open reduction internal fixation in ankle fractures with and without dislocation. Methods. A retrospective chart review of surgically treated ankle fractures over a 3- year period was performed. Demographic data, type of fracture, operative time and complications were recorded. Of 118 patients eligible for analysis, 33 (28…

Distal Radius Radiographic Indices and Perilunate Fracture Dislocation.

BACKGROUND: Distal radius radiographic indices may play a role as risk factors in pathogenesis of Kienbock's disease, scaphoid fracture and nonunion. Perilunate fracture dislocations are devastating wrist injuries, and their relationship and distal radius indices have not been addressed in the literature. OBJECTIVES: The aim of this study was to evaluate the possible role of distal radius radiographic indices including radial height, radial inclination, ulnar variance and volar tilt as risk factors in the perilunate fracture dislocation injury of the wrist. PATIENTS AND METHODS: We studied d…

A Galeazzi-variant type fracture-dislocation in adults.

OBJECTIVE: Fracture of either radius or ulna with a dislocation either at the proximal or distal radioulnar joint (DRUJ) is not a common injury and is inherently unstable. Here we report a case series, with both-bone forearm fractures associated with dislocation of DRUJ, as a Galeazzi-variant type fracture-dislocation, and try to analyze this injury pattern. METHODS: The study was based on 6 patients having Galeazzi-variant type fracture-dislocation of different age (20 to 45 years). All fractures were closed type. Two fractures involved the same level and three fractures were at different le

Treatment of complete fracture-dislocation of thoracolumbar spine.

STUDY DESIGN: A retrospective study. OBJECTIVE: We present our surgical experience with 11 patients who had complete fracture-dislocation of thoracolumbar spine. SUMMARY OF BACKGROUND DATA: Complete fracture-dislocation of thoracolumbar spine is one of the most severe spine traumas and usually results in neurological dysfunction with spine deformity. Treatment of such cases is rarely described in the literature. METHODS: We treated 11 patients with complete fracture-dislocation of the thoracolumbar spine surgically using a posterior approach. Reduction and stabilization was achieved by mean

Triple jeopardy: transscaphoid, transcapitate, transtriquetral, perilunate fracture dislocation.

Carpal injuries are frequently underdiagnosed and underreported injuries of the hand. Scapholunate perilunate dissociation is the most common perilunate instability pattern seen in clinical practice. Transscaphoid, transtriquetral, transcapitate dislocation with a volar intercalated segment instability pattern is a very rare pattern of carpal injury. We describe a case with this unique pattern of injury, explaining its mechanism and treatment. Good outcome can be achieved in these injuries following open reduction and internal fixation with ligamentous repair.

Paediatric biepicondylar elbow fracture dislocation - a case report.

Paediatric elbow biepicondylar fracture dislocations are very rare injuries and have been only published in two independent case reviews. We report a case of 13 years old boy, who sustained this unusual injury after a fall on outstretched hand resulting in an unstable elbow fracture dislocation. Closed reduction was performed followed by delayed ORIF (Open Reduction and Internal Fixation) with K wires. Final follow-up at 14 weeks revealed a stable elbow and satisfactory function with full supination-pronation, range of motion from 0°-120° of flexion and normal muscle strength. This type of inj…

Open medial dislocation of the ankle without fracture.

A 20-year-old man sustained an open medial dislocation of the ankle without an associated fracture after a low-energy inversion injury. Prompt debridement and reduction with primary wound closure of the skin were performed without suture of the capsule. Immobilisation in a non-weight-bearing cast for 30 days followed by ankle bracing for two weeks and subsequent physiotherapy, produced full functional recovery by three months. At follow-up at one year there was a full range of pain-free movement, although the radiographs and MR scan showed early post-traumatic degenerative change at the medial

Periprosthetic patellar fracture after an open knee dislocation.

We present a patient who sustained an open knee dislocation with a periprosthetic fracture of the patella and who was treated satisfactorily with tension band wiring and a conservative rehabilitation program. In addition to describing this unique combination of injuries, we review the literature on the treatment of periprosthetic patellar fractures. Although internal fixation of these fractures historically has provided unpredictable results, we think that it is warranted if the patella has adequate bone stock.

Open Galeazzi fracture with concomitant radial head dislocation.

Galeazzi fractures are uncommon, but a combination of open Galeazzi fracture and radial head dislocation has not been described. This article reports on a variant of an open Galeazzi fracture in which concomitant radial head dislocation and coronoid process fracture occurred. Thorough understanding of the mechanisms and influences of pronation injuries of the forearm is essential in order to properly treat this fracture-dislocation. Our evaluation of the individual elements of this composite injury has allowed us to propose a mechanism of injury.

Multidirectional radiocarpal dislocation without fracture: a case report.

The first reported case of posttraumatic multidirectional instability of the radiocarpal joint is described with a review of the relevant literature. A 31-year-old man sustained a palmar radiocarpal dislocation that also dislocated dorsally with gentle provocation and showed ulnar translation and a dorsal intercalated segmental instability pattern after closed reduction. Treatment consisted of open reduction and internal fixation with 11 weeks of immobilization. Follow-up at 32 months showed excellent clinical and radiographic results. Multidirectional instabilities of the radiocarpal joint ca

Facet fracture-dislocation injuries of the cervical spine.

Sixty-eight patients with acute traumatic cervical facet fracture-dislocation injuries are presented. These patients represented 6.7% of all cervical spine fractures identified over a 12-year period. Thirty-one patients had unilateral facet injuries and 37 had bilateral facet injuries. Neurological morbidity was 90% and was most severe among bilateral facet injury patients (84% complete injuries). Spinal shock was identified in 13 patients and was a poor prognostic indicator for the subsequent recovery of associated neurological deficits. Closed reduction was attempted in 66 of 68 patients and
