UNLABELLED: Iatrogenic vascular injuries are uncommon during the course of proximal femur surgical procedures. We report the case of an 85-year-old female presenting with an intertrochanteric fracture, treated by anterograde (cephalocondylic) intramedullary nailing (Stryker gamma 3 nail) and complicated by a superficial femoral artery laceration at the level of the distal locking screw. Lower limb traction in adduction and internal rotation on the operating table might put at risk the superficial femoral artery during distal screw drilling and insertion. We therefore recommend returning to the

BACKGROUND: Fractures of the distal femur are an increasingly common injury; the optimal management of these injuries remains controversial. The two interventions used in UK practice are intramedullary fixation, with a locked retrograde nail, and extramedullary fixation, with a fixed angle plate.
OBJECTIVES: This study assessed the feasibility of a definitive trial and included a process evaluation to understand the generalisability and likely success of a future trial.
DESIGN: A multicentre, parallel, two-arm, randomised controlled feasibility trial with an embedded process evaluation. Trea

BACKGROUND: The best treatment for fractures of the distal tibia remains controversial. Most of these fractures require surgical fixation, but the outcomes are unpredictable and complications are common.
OBJECTIVES: To assess disability, quality of life, complications and resource use in patients treated with intramedullary (IM) nail fixation versus locking plate fixation in the 12 months following a fracture of the distal tibia.
DESIGN: This was a multicentre randomised trial.
SETTING: The trial was conducted in 28 UK acute trauma centres from April 2013 to final follow-up in February 2017

We compared four methods of metacarpal shaft fixation: 2.2 mm intramedullary headless compression screw; 3.0 mm intramedullary headless compression screw; intramedullary K-wire fixation; and dorsal plate fixation. Transverse mid-diaphyseal fractures were created in 64 metacarpal sawbones and were assigned into four groups. Peak load to failure and stiffness were measured in cantilever bending and torsion. We found that dorsal plating had the highest peak load to failure. However, initial bending stiffness of the 3.0 mm intramedullary headless compression screw was higher than that of the dorsa

OBJECTIVE: Closed reduction and intramedullary nail (IMN) fixation remains the gold-standard treatment for femoral shaft fractures (FSFs). However, technical challenges persist in achieving and maintaining anatomical alignment during the closed reduction process. This study aimed to evaluate the clinical efficacy of Hexapod external fixator (HEF)-assisted closed reduction and IMN fixation in the management of FSFs.
METHODS: A retrospective cohort study was conducted on 25 patients undergoing closed reduction and IMN fixation with eccentric HEF assistance between June 2021 and January 2023. Op

BACKGROUND/AIM: The number of individuals diagnosed with and requiring medical treatment for osteoporosis continues to increase due to global population aging and the high awareness of osteoporosis. Bone-modifying agents (BMAs) including bisphosphonate and denosumab are widely used for osteoporosis, and atypical femoral fracture (AFF) is also gaining attention as a severe potential side effect of long-term BMA treatment. The definition of AFF excludes periprosthetic femoral fracture; here, we describe two cases of a periprosthetic femoral fracture that resembled AFF.

Implant choice is a matter of concern in athletes and active patients who sustain a Jones fracture because they are prone to failure including non-union, screw failure, and refracture. The aim of this study was to compare the biomechanical behavior of a Jones fracture-specific screw (JFXS) with a cannulated headless compression screw (HCS) in a simulated partial weight-bearing and ultimate load Jones fracture fixation model. Ten matched pairs of human anatomical specimens underwent Jones fracture creation and consecutive intramedullary stabilization with a solid JFXS or a cannulated HCS. The b

INTRODUCTION: Hand fractures are associated with significant morbidity. Current management standards often result in prolonged immobilization, stiffness, and delayed return to functional use. Intramedullary (IM) compression screws offer minimal soft tissue disruption and early postoperative active motion. In this study, we describe our outcomes after intraosseous fracture fixation using IM cannulated headless screws for a multitude of fracture patterns.
METHODS: This study is a retrospective review of patients who underwent IM screw placement for fixation of metacarpal and phalangeal fracture

Background and objectives: Treatment of a displaced or comminuted periprosthetic distal femur fracture is challenging, especially in patients with osteoporosis. In this case report, we shared our successful surgical experience of using a long intramedullary fibula bone graft in a plate fixation surgery for a periprosthetic distal femur fracture in an extremely elderly patient with osteoporosis. Case report: A 95-year-old woman with severe osteoporosis (bone mineral density level: -3.0) presented with right knee pain and deformity after a fall, and a right periprosthetic distal femur fracture w

Although early mobilization is essential for functional recovery after phalangeal fractures, many fixation methods still require prolonged immobilization, increasing the risk of joint stiffness. We herein describe a simple, safe, and low-cost technique that combines intramedullary Kirschner wires (K-wires) with external connection, adapted from the Ulson method, which was originally developed for distal radius fractures. The procedure consists of closed reduction under fluoroscopy followed by the insertion of two 1.0-mm intramedullary wires. The external ends of these wires are bent and connec
