Treatment of knee cartilage defect in 2010.

Treatment of knee cartilage defect, a true challenge, should not only reconstruct hyaline cartilage on a long-term basis, but also be able to prevent osteoarthritis. Osteochondral knee lesions occur in either traumatic lesions or in osteochondritis dissecans (OCD). These lesions can involve all the articular surfaces of the knee in its three compartments. In principle, this review article covers symptomatic ICRS grade C or D lesions, depth III and IV, excluding management of superficial lesions, asymptomatic lesions that are often discovered unexpectedly, and kissing lesions, which arise prior

Cartilage Plating in Flail Chest Fixation.

INTRODUCTION: Operative stabilization of flail chest has been shown to have several benefits over nonoperative management. Often, flail chest injuries will involve the anterior ribs and their associated costal cartilage. In certain cases, operative fixation with open reduction and internal fixation (ORIF) of anterior rib fractures involving the costal cartilage may be warranted. Currently, there is scant literature regarding the surgical approach and clinical outcomes of ORIF involving the costal cartilage. The purpose of this study is to describe the surgical approach and first reported clini

Post-traumatic orbital floor reconstruction with nasoseptal cartilage in children.

OBJECTIVE: Repair of orbital floor fractures may require the placement of a graft or implant. Both autogenous and alloplastic materials have been used for this purpose. This article reports the use of nasal septal cartilage for the repair of orbital floor defect secondary to blunt facial trauma in children. METHODS: Three children with disruption of the orbital floor after facial trauma were included in this prospective review. All children underwent open reduction with rigid fixation of the facial fractures and reconstruction of the orbital floor with nasoseptal cartilage. RESULTS: All of t

Articular reductions - how close is close enough? A narrative review.

Intra-articular fractures are a unique subset of fractures as they involve a varying extent of damage to cartilage. The impact of this articular fracture causes significant microscopic and macroscopic changes, as well as biomechanical irregularities, which can lead to further cartilage damage, and ultimately cascade down the dreaded path to arthritis. It is generally believed that an anatomic reduction of an articular fracture is the necessary goal of treatment for these injuries, however it yet to be delineated how perfect this reduction has to be. A comprehensive literature review was carrie

Large osteochondral fractures of the lateral femoral condyle in the adolescent: outcome of bioabsorbable pin fixation.

BACKGROUND: Large osteochondral fractures of the lateral femoral condyle of the knee in adolescent patients can be diagnostically and therapeutically challenging. Historically, management has involved removal of the fragment, leaving a large area of bone devoid of articular cartilage on the weight-bearing surface of the lateral femoral condyle. This study assessed open reduction and internal fixation of the osteochondral fragments with use of multiple polyglycolic acid rods. METHODS: Eight patients, between twelve and fifteen years old, with a large (>4 cm(2)) osteochondral fracture of the la

Particulated Autograft Cartilage Implantation for the Treatment of Osteochondral Lesions of the Talus: A Novel Technique.

UNLABELLED: Osteochondral lesions of the talus often occur following ankle sprains and fractures. Operative intervention is typically required because of the diminished intrinsic repair capability of talar articular cartilage. Several techniques have evolved that emphasize replacing the defect with cartilage that closely mimics the biological properties of hyaline articular cartilage. The goals of operative treatment are resolution of symptoms, physiologic healing, and restoration of function while eliminating the need for further intervention. This article describes a novel, single-step techn

Histopathologic assessment of healed osteochondral fractures.

PURPOSE: To assess the histopathologic features of healed tissue to define its biological and biomechanical properties after internal fixation of osteochondral fractures. Type of Study: Cohort study. METHODS: The general principle of management of detached acute osteochondral fractures is reattachment of the fragment by internal fixation, but the opinions on the quality and structure of the healed tissue that will be obtained after treatment is controversial. This study included 13 patients with acute osteochondral fractures who were treated arthroscopically and had surgical fixation providing

Incidence of concomitant chondral/osteochondral lesions in acute ankle fractures and their effect on clinical outcome: a systematic review and meta-analysis.

INTRODUCTION: Despite successful osteosynthesis, some patients report residual symptoms after ankle fractures. One of the reasons behind the postoperative complaints might be traumatic concomitant chondral lesions (CL) and/or osteochondral lesions (OCL) within the ankle joint. The study aims to systematically review the incidence of CL and/or OCL in ankle fractures and to assess their effect on the clinical outcome. MATERIALS AND METHODS: This work was conducted according to PRISMA checklists. A systematic literature search was performed using following keywords: "Ankle Fractures" OR "Trimall

Cartilage damage in patients with scapholunate lesions: arthroscopic prevalence, location and associated clinical factors.

INTRODUCTION: Scapholunate ligament (SLL) injuries can lead to instability and osteoarthritis through stages of scapholunate dissociation and scapholunate advanced collapse. Although cartilage damage is traditionally seen as a late finding, recent evidence suggests that it may occur earlier. This study aimed to assess the prevalence and distribution of cartilage damage in patients with isolated SLL lesions confirmed by arthroscopy and to explore associated sociodemographic and clinical factors. METHODS: A retrospective cohort study was conducted in patients with arthroscopically confirmed SLL

An Unexpected Complication after Headless Compression Screw Fixation of an Osteochondral Fracture of Patella.

This study evaluated complications associated with implant depth in headless compression screw treatment of an osteochondral fracture associated with a traumatic patellar dislocation in a 21-year-old woman. Computed tomography and X-rays showed one lateral fracture fragment measuring 25 × 16 mm. Osteosynthesis was performed with two headless compression screws. Five months later, the screws were removed because of patella-femoral implant friction. We recommend that the screw heads be embedded to a depth of at least 3 mm below the cartilage surface. Further clinical studies need to examine the

Optimizing Delivery of Therapeutic Growth Factors for Bone and Cartilage Regeneration.

Bone- and cartilage-related diseases, such as osteoporosis and osteoarthritis, affect millions of people worldwide, impairing their quality of life and increasing mortality. Osteoporosis significantly increases the bone fracture risk of the spine, hip, and wrist. For successful fracture treatment and to facilitate proper healing in the most complicated cases, one of the most promising methods is to deliver a therapeutic protein to accelerate bone regeneration. Similarly, in the setting of osteoarthritis, where degraded cartilage does not regenerate, therapeutic proteins hold great promise to p

Pediatric pelvic fractures.

Pediatric pelvic fractures account for only 1% to 2% of fractures seen by orthopaedic surgeons who treat children. They are typically associated with high-energy trauma, requiring a comprehensive workup for concomitant life-threatening injuries. Anteroposterior radiographs and rapid-sequence computed tomography are the standards of diagnostic testing to identify the fracture and recognize associated injuries. Treatment is individualized based on patient age, fracture classification, stability of the pelvic ring, extent of concomitant injuries, and hemodynamic stability of the patient. Most pel

Acute traumatic injury of the larynx.

Laryngeal trauma is rare but serious and potentially deadly injury. The prompt diagnosis and management of acute laryngeal trauma is necessary because the clinical presentation is variable depending on the location, severity, and mechanism of injury. Two case histories are presented: (1) case history A: a 53-year-old male, after motor vehicle accident, fractured the mid anterior thyroid cartilage and both aspects of the cricoid cartilage; however, this patient was asymptomatic from the above fractures; and (2) case history B: a 41-year-old male who sustained trauma to the chest, neck, and left

[Imaging strategies for knee injuries].

Injuries of the knees are common. The Ottawa knee rule provides decisional support to determine whether radiographs are indicated or not. With the use of ultrasound it is possible to detect defects of the extensor ligaments and the anterior cruciate ligament. Furthermore, it is possible to detect indirect signs of an intra-articular fracture, e.g. lipohemarthrosis. In complex fractures, e.g. tibial plateau fractures, further diagnostic procedures with multislice computed tomography (CT) are needed for accurate classification and preoperative planning. Multislice CT with CT angiography enables

CPR related injuries of the chest wall: direct and indirect fractures.

BACKGROUND: Rib and sternum fractures are common injuries associated with cardiopulmonary resuscitation (CPR). The fracture mechanism is either direct by application of force on sternum and anterior ribs or indirect by bending through compression of the thorax. The aim of this study was to determine morphologies of rib fractures after CPR and to reevaluate prior findings on fracture localisation, type and degree of dislocation. METHODS: The present study was based on all inpatients treated for chest wall fractures after non traumatic cardiac arrest at a Level 1 Trauma Centre from 2010 to 2016

Common elbow fractures in children.

Approximately 75 percent of all fractures sustained by children occur in the upper extremities and frequently occur during a fall onto an outstretched hand. The majority of these injuries involve the wrist and forearm, but the elbow alone accounts for approximately 10 percent of all fractures in children. Elbow fractures in children are challenging because of the abundance of unossified cartilage and the high potential for limb-threatening damage to neurovascular structures. Common types of elbow fractures include supracondylar, lateral condylar, medial epicondylar, radial neck and transphysea

Lesions of cartilage and bone around the knee.

A variety of bone and cartilage abnormalities can be demonstrated on magnetic resonance imaging (MRI), which can play an important role in the diagnosis and management of these abnormalities. Ischemic bone lesions, including medullary bone infarction, primary osteonecrosis, and the osteochondroses, can be precisely evaluated as to their anatomic locations and relationship to overlying cartilage. Osteochondritis dissecans may also be diagnosed and staged. Presence of subchondral fractures and bone contusions, which are frequently associated with ligament and meniscal injuries, can also be diagn
