Gastric outlet obstruction (GOO) is a mechanical obstruction of the distal stomach or proximal duodenum. Surgical gastro-jejunostomy and self-expanding metal duodenal stents were the conventional treatments for GOO. In recent years, a new treatment option emerged using echo-guided endoscopic gastroenterostomy (EUS-GE). It appears to be a safe and effective technique with a clinical success rate of 85-90 % and a side effect rate of less than 18 %. We attempted endoscopic sphincterotomy in 35 patients previously subjected to Billroth II gastrectomy, and succeeded in 23 of them. It is often difficult to pass the instrument along the afferent loop and the sphincterotomy incision is seldom placed in an optimal position. Although we had complications in only 2 patpients, it is likely that spincterotomy carries a higher risk in Billroth II gastrectomy patients. It should therefore be reserved for patients who are at high risk for surgery.

The effect of duodenal acid perfusion on pentagastrin-stimulated gastric acid secretion was studied in Sprague-Dawley rats provided with a chronic gastric fistula, gastroenterostomy, and a 4-cm blind duodenal loop anastomosed to the jejunum. During maximal acid stimulation with intravenous pentagastrin (16 micrograms X kg-1 h-1) the loop was perfused with 0.15 M NaCl or 0.05 M, 0.10 M, or 0.20 M HCl at a rate of 2 ml X h-1.
