Open gastrostomy by mini-laparotomy: a comparative study.

BACKGROUND: Gastrostomy tube feeding is the best option for long lasting nutritional support in patients with dysphagia caused by obstructive tumours of the mouth, pharynx, larynx and ooesophagus or neuromuscular diseases. However, these severely compromised patients often present severe respiratory risks, precluding the use of general anesthesia, sedation or even endoscopy. A simplified open gastrostomy (SOG) under local anesthesia has been in practice in our institution, especially for patients with severe neuromuscular diseases and continuous non-invasive ventilatory support. In this study,…

Gastrostomy complications in infants and children: a comparative study.

PURPOSE: Gastrostomy is a commonly undertaken procedure in children. Methods for gastrostomy insertion have evolved and laparoscopy is commonly used to aid its insertion. The aim of this study is to review the outcome and complications of laparoscopic-assisted gastrostomy insertion. METHODS: A retrospective case note review of children undergoing gastrostomy insertion by a single surgeon from 1999 to 2007. RESULTS: 114 children underwent gastrostomy insertion, 68 males and 46 females, with a median age of 55 months (IQR 28-149 months). The majority were neurologically impaired. 98 underwent …

Attention to small details: big deal for gastrostomies.

Gastrostomy tubes are used in the pediatric population when long-term enteral feeding is needed. A common method of placement is percutaneously with endoscopy (PEG, percutaneous endoscopic gastrostomy). Although PEG placement is a straightforward procedure most of the time, it can be associated with a significant rate of minor complications and a smaller but significantly important rate of major complications. Some of these complications may also occur after any type of gastrostomy. We will present representative case studies outlining major complications and discuss how we may be able to prev…

The role of gastrostomy in the staged operation of esophageal atresia.

INTRODUCTION: The aim of this study is to recommend criteria for selection of patients who benefited from the use of gastrostomy rather than emergency fistula closure during the staged operation of esophageal atresia (EA). MATERIALS AND METHODS: Between August 2004 and July 2006, 75 cases of EA, were consecutively operated. Nineteen out of 75 (25%) underwent routine gastrostomy because they required a type of staged operation: Group I: Five cases with pure atresia had gastrostomy and esophagostomy; Group II: Six with severe pneumonia and congenital heart disease (Waterson class C) had gastros…

Percutaneous endoscopic gastrostomy by head and neck surgeons.

The endoscopic placement of percutaneous gastrostomy tubes has been an accepted technique for several years but has traditionally been performed by gastroenterologists and general surgeons. Fluoroscopically guided tube placement is now performed by radiologists. Head and neck surgeons have been slow to adopt the responsibility for placing percutaneous gastrostomy tubes despite the fact that most are proficient in both rigid and flexible esophagoscopy and trained in the Seldinger technique. We report on 41 percutaneous endoscopic gastrostomies performed in 39 patients by the Head and Neck Servi…

Microbiology of gastrostomy site wound infections in children.

Specimens of pus were obtained from gastrostomy site wound infections in 22 children. Polymicrobial flora was found in 21 of the 22 wounds. Aerobic or facultative bacteria only were isolated in eight (36%) instances and mixed aerobic-anaerobic flora were isolated from the other 14 (64%) wounds. A total of 102 bacterial isolates (57 aerobic and 45 anaerobic) and seven cultures of candida were obtained. The most frequent isolates were Escherichia coli (16 isolates). Peptostreptococcus spp. (14), Enterococcus spp. (14), Bacteroides spp. (12) and Staphylococcus aureus (6). Twenty-eight strains pro…

Laparoscopic gastrostomy and jejunostomy: review of 22 cases.

Laparoscopic gastrostomy (LG) and laparoscopic jejunostomy (LJ) were performed successfully on 22 consecutive patients after development of the procedure on a porcine model. Patients did not undergo percutaneous endoscopic gastrostomies (PEG) due to obstruction from head and neck, esophageal cancer, gastropexy for gastric volvulus, perforated esophagus, failed PEGs, or surgeon's preference. Operative time averaged < 20.5 min. Five patients underwent the procedure under local anesthesia with intravenous sedation and three patients were operated on in a strictly outpatient setting. There was one…

Concomitant placement of percutaneous endoscopic gastrostomy and jejunostomy.

Percutaneous endoscopic gastrostomies have gained wide use for long-term enteral nutrition. However, gastroesophageal reflux and aspiration pneumonia have occurred following this procedure. Initial enthusiasm concerning the ability of intrajejunal feeding to negate the risk of aspiration has been challenged by some reports. In this report, a new method is described for concomitant placement of endoscopic gastrostomy and feeding jejunostomy wherein the tip of the feeding jejunostomy is placed at least 40 cm distal to the pylorus while the gastrostomy tube is used for drainage. Twenty critically…

[Efficacy and safety of percutaneous endoscopic gastrostomy in the elderly].

Percutaneous endoscopic gastrostomy (PEG) was performed in 32 patients with nasogastric feeding (NGF) or total parenteral nutrition (TPN), who were unable to swallow. Our cases of PEG included 10 with dementia, 8 with cerebral infarction, 8 with cerebral bleeding, 3 with gastric cancer, and others. TPN was performed after PEG for a short time. Because of the combination of TPN and PEG, there was no mortality or major complication related to the procedure. Minor complications included subcutaneous abscess and TPN catheter fever. PEG can be safely and rapidly performed. Furthermore, painless lif…

Percutaneous drainage and feeding gastrostomies in 100 patients.

Percutaneous gastrostomy was performed in 100 cancer patients. In 67 patients with bowel obstruction, the procedure was performed for gastric drainage with 24-28-F Malecot catheters inserted in one sitting. The remaining 33 patients had supragastric obstructions or fistulas and required 10-14-F pigtail catheters for feeding purposes. Average postgastrostomy hospitalization was 3.6 days. Drainage gastrostomies were ready for use immediately after the procedure, whereas use of feeding gastrostomies started on average within 2 days of tube insertion. There were no major complications or deaths re…

Acute pancreatitis: a complication of Foley catheter gastrostomy.

Acute pancreatitis can be caused by inadvertent migration into the duodenum of the inflated balloon of a Foley gastrostomy catheter. The clinical picture is that of an acute abdomen. The diagnosis is made with a high index of suspicion and confirmed by marked elevation of serum and urine amylase levels. The treatment consists of repositioning of the gastrostomy tube and supportive therapy. Careful external fixation of the tube to the abdominal wall can prevent this complication.

Experience with a new gastrostomy-jejunal feeding tube.

Many patients in need of long-term feeding by tube enterostomy have neurologic or mechanical deficits that increase their risk for aspiration. To reduce this risk, a combined gastrostomy-jejunal tube has been developed. This tube permits gastric decompression while the formula is being delivered into the jejunum. The use of this tube has been evaluated in 45 patients. There were three (7%) major complications and two (4%) postoperative deaths. The deaths did not appear to be directly related to the insertion of the tube. There was no clinical evidence of aspiration. This tube can be used to de…

[Bacterial colonization of the stomach in newborn infants with gastrostomy].

Quantitative bacterial cultures were carried out in 137 gastric aspirates of 52 neonates with gastrostomies due to intestinal malformations. In 28% there was no growth of any organism, in 72% we found one to four bacterial species. Most often enterobacteriaceae, enterococci, pseudomonas aeruginosa, staphylococcus epidermidis and candida albicans could be cultured. We could not find any influence of systemic antibiotic therapy on bacterial colonization of the stomach. On the other hand nystatin given orally significantly decreased colonization by candida albicans. Different types of formulas or

Percutaneous endoscopic gastrostomy tubes: A premier clinical review.

Adequate nutritional support is a critical component of patient management, particularly for individuals who are unable to sustain oral intake due to neurological, structural, or functional impairments. Percutaneous endoscopic gastrostomy (PEG) has become the preferred modality for long-term enteral access, offering a minimally invasive technique with high success rates and favorable safety outcomes. Common indications include cerebrovascular accidents, neurodegenerative disorders, dementia, cerebral palsy, head and neck malignancies, and severe dysphagia of various etiologies. This review sum…
