Xeroderma pigmentosum: from genetics to hopes and realities of cutaneous gene therapy.

Xeroderma pigmentosum (XP) is a rare genodermatosis transmitted as an autosomal and recessive trait. XP patients are highly photosensitive and prone to develop skin tumours in sun-exposed areas. Biochemical and genetic studies have demonstrated that nucleotide excision repair, the most versatile DNA repair mechanism, is deficient in XP cells, leading to ultraviolet-induced hypermutagenesis and a predisposition of XP patients to cancer. Cloning of XP genes responsible for the disease, together with the poor efficacy of classical pharmacological treatments, have motivated approaches towards cuta

[Progressive familial intrahepatic cholestasis (Byler disease): current genetics and therapy].

Progressive familial intrahepatic cholestasis (PFIC) is a congenital liver disease. First symptoms can frequently be seen shortly after birth. Quality and expectation of life are substantially reduced due to severe pruritus and the complications of progressive liver cirrhosis. PFIC is diagnosed on the basis of characteristic clinical and laboratory parameters and genetic analysis after exclusion of other liver diseases leading to intrahepatic cholestasis. Medical therapy is only effective in a proportion of children with PFIC. Partial biliary diversion (PBD) is nowadays considered the therapy

Genetically engineered pancreatic beta-cell lines for cell therapy of diabetes.

The optimal treatment of insulin-dependent diabetes mellitus (IDDM), which is caused by the autoimmune destruction of pancreatic islet beta cells, would require the regulated delivery of insulin by transplantation of functional beta cells. beta-cell transplantation has so far been restricted by the scarcity of human islet donors. This shortage would be alleviated by the development of differentiated beta-cell lines, which could provide an abundant and well-characterized source of beta cells for transplantation. Using conditional transformation approaches, our laboratory has generated continuou

Evaluation of response to hormonal therapy in prepubertal gilts of different genetic lines.

Research was conducted to determine the effect of genetic line on hormonally induced puberty. Two studies were conducted, the first to evaluate estrus response (n = 120 gilts) and the second to evaluate follicular development (n = 24 gilts). Gilts were allotted to treatments in a 2 x 2 factorial arrangement. Gilts from two genetic lines (Age at Puberty x Yorkshire [APY] and Relaxed Selection x Yorkshire [RSY] received either P.G. 600 [symbol: see text] (P) or no injection (C). The percentage in estrus within 5 d after treatment was greater (P < .05) for P than for C gilts. However, no differen

[Genetic analysis and a new therapy for a hereditary disease: familial amyloid polyneuropathy].

Familial amyloid polyneuropathy (FAP) is a hereditary disorder with autosomal dominant trait and is characterized by the accumulation of transthyretin at the nervous systems. The disorder mostly becomes overt in the fourth decade of life among affected individuals. Treatment of FAP has been directed toward the relief of symptoms and not intended to eliminate the yet unknown cause of this disorder. Recently, Val30 to Met substitution in the transthyretin gene was found among the patients with typical FAP. This enabled us to genotype affected individuals by PCR-RFLP and paved the way to the trea

The fetal frontier: A review of current and emerging fetal therapies for genetic diseases.

Pivotal advancements in diagnostics have greatly improved early detection of disease in the fetus. Rapid diagnosis of prenatal disease, including many aneuploidies and single-gene disorders, is now possible. These capabilities present an opportunity for earlier interventions and involvement of multidisciplinary care for infants discovered to have genetic disorders. While advancements continue to be made surgically addressing anatomic pathologies, more recently, the scope of fetal medicine has expanded to include the treatment of genetic disease. This unique and growing group of conditions with

How genetics changed or will change the therapy in thyroid nodules with indeterminate cytology?

The incidence of thyroid cancer has risen in recent decades, largely due to the widespread use of increasingly sensitive imaging techniques that have enhanced the detection of thyroid nodules. Fine-needle aspiration cytology remains the diagnostic gold standard; however, 15-25% of samples fall into indeterminate Bethesda categories (III and IV), for which the traditional approach often involves diagnostic surgery, despite around 75% of these nodules ultimately prove to be benign. Advances in the genetic characterization of thyroid tumors, together with the development of next generation sequen

Inflammatory Signatures in MDS: The Missing Link Between Genetics, Microenvironment, and Therapy.

Myelodysplastic syndromes are clonal hematopoietic neoplasms in which ineffective hematopoiesis arises within the context of chronic inflammation and immune dysregulation. Growing evidence indicates that aging-associated inflammaging and inflammation-driven remodeling of the bone marrow microenvironment are not secondary phenomena, but active forces that shape clonal selection, lineage commitment, and disease evolution. This narrative review integrates recent insights from translational immunology, stem cell biology, multi-omics analyses, and clinical studies to examine the reciprocal interpla

Gene-based Therapies for Genetic Cardiomyopathies: Molecular Medicine for Heart Disease.

Pathogenic genetic alterations are a well-recognized mechanism in cardiomyopathies. As such, genetic testing has become an integral component of the diagnostic pathway for cardiomyopathy. Subsequent developments in gene-specific therapies have advanced precision medicine by enabling direct targeting of pathogenic genetic variants, demonstrating promise as the ultimate therapy for cardiomyopathy. Gene therapy can be categorized into 3 main approaches: gene replacement, gene silencing, and direct genome editing. Clustered regularly interspaced short palindromic repeats (CRISPR)/CRISPR-associated

Advancing precision tacrolimus therapy: a systems genetics dissection in BXD platform.

BACKGROUND: Tacrolimus is a core immunosuppressant in organ transplantation, but its narrow therapeutic window and significant pharmacokinetic variability hinder precision dosing. Although CYP3A5-guided strategies have established clinical relevance for tacrolimus initial dose adjustment, they do not fully account for the marked interindividual variability in tacrolimus exposure, highlighting the need for complementary models to decode more complex genetic regulation. This study aimed to identify candidate genetic modulators of tacrolimus metabolism and develop an integrated predictive framewo

Chordoma Genetic Aberrations and Targeted Therapies Panorama: A Systematic Literature Review.

Background: Chordomas pose a challenge in treatment due to their local invasiveness, high recurrence, and potential lethality. Despite being slow-growing and rarely metastasizing, these tumors often resist conventional chemotherapies (CTs) and radiotherapies (RTs), making surgical resection a crucial intervention. However, achieving radical resection for chordomas is seldom possible, presenting therapeutic challenges. The accurate diagnosis of these tumors is vital for their distinct prognoses, yet differentiation is hindered by overlapping radiological and histopathological features. Fortunat

"Treatment with curative intent": the emergence of genetic therapies for sickle cell anemia.

The root cause of sickle cell anemia has been known for 7 decades, yet no curative therapies have been available other than allogeneic bone marrow transplantation, for which applicability is limited. Two potentially curative therapies based on gene therapy and gene editing strategies have recently received US Food and Drug Administration approval. This review surveys the nature of these therapies and the opportunities and issues raised by the prospect of definitive genetically based therapies being available in clinical practice.
