Managing a common dermatological problem: incontinence dermatitis.

Incontinence dermatitis is an under-recognized and under-researched problem that mostly affects older people who have continence problems. Nurses who are aware of the risk factors can provide care that reduces the risk of this distressing problem. If incontinence dermatitis occurs, evidence-based care can be used to treat the person and reduce the risks of further complications.

YSQ-GeMS: Development of a Short Form of the Young Schema Questionnaire for Geriatric Mental Health Care Using Item Response Theory.

Schema therapy is effective for older adults with personality disorders, but the Young Schema Questionnaire (YSQ) may be too lengthy for use in geriatric mental health care. This study evaluated the construct validity of the YSQ-L2 in adults aged 35-97 years receiving residential or outpatient geriatric, psychiatric or medical care and developed a shorter form for this population. Data from 214 participants from Dutch nursing homes and outpatient services were analysed using Item Response Theory across 16 YSQ-L2 subscales. Each subscale was reduced to five items based on discrimination, diffic

Recognizing delirium superimposed on dementia: assessing nurses' knowledge using case vignettes.

Delirium is a serious and prevalent problem that occurs in many hospitalized older adults. Delirium superimposed on dementia (DSD) occurs when a delirium occurs concurrently with a pre-existing dementia. DSD is typically underrecognized by medical and nursing staff. The current study measured nursing identification of DSD using standardized case vignettes, and the Mary Starke Harper Aging Knowledge Exam (MSHAKE). Results revealed that the nurses in this study had a high level of general geropsychiatric nursing knowledge as measured by the MSHAKE, yet had difficulty recognizing DSD compared to

The quality of nursing home care in Taiwan.

This study describes the quality of care in nursing homes in Taiwan using demographic data, functional status measures, resident clinical care outcomes, and resident's ratings of satisfaction with nursing home care. Three hundred and eight randomly selected elderly residents in 13 nursing homes were interviewed through a structured interview process and demographic and medical information was collected from their nursing home records. Nursing home residents were younger and less functional than their counterparts in the United States. Stroke and dementia were the most common primary diagnoses

Nurses' competence in recognition and management of delirium in older patients: development and piloting of a self-assessment tool.

BACKGROUND: Delirium is a common condition in elderly inpatients. Health care professionals play a crucial role in recognizing delirium, initiating preventive measures and implementing a multicomponent treatment strategy. Yet, delirium often goes unrecognized in clinical routine. Nurses take an important role in preventing and managing delirium. This study assesses clinical reasoning of nurses using case vignettes to explore their competences in recognizing, preventing and managing delirium. METHODS: The study was conducted as an online survey. The questionnaire was based on five case vignett

Assessment of capacity for medical decision making.

Determination of a patient's capacity for medical decision making can be simple or complex, depending on the issue being considered; likewise, the degree of capacity required to make decisions varies with the complexity of the situation. A capacity examination is a focused and impartial evaluation of mental status and medical, surgical, psychiatric, and psychosocial histories. The examination is composed of several basic steps, beginning with an assessment of the patient's ability to communicate, followed by determination of his or her understanding of the proposed care, followed by the patien

Toward the realization of a better aged society: messages from gerontology and geriatrics.

UNLABELLED: 1. BACKGROUND: Recent medical advancements, and improvements in hygiene and food supply have led to Japan having the longest life expectancy in the world. Over the past 50 years, the percentage of the elderly population has increased fourfold from 5.7% in 1960 to 23.1% in 2010. This change has occurred at the fastest rate in the world. Compared with France, where the percentage of the elderly population has increased just twofold in the past 100 years, Japanese society is aging at an unprecedented rate. In addition, the percentage of the very elderly (aged 75 years and over), comp

Geriatric conditions do not predict stroke or bleeding in long-term care residents with atrial fibrillation.

BACKGROUND: Long-term care (LTC) providers prescribe anticoagulation (AC) less frequently in residents with atrial fibrillation (AF) and geriatric conditions independent of CHA2 DS2 -VASc stroke risk score. Geriatric conditions include recent fall, activities of daily living dependency, mobility impairment, cognitive impairment, low body mass index, and weight loss. Multiple publications have suggested that patients with geriatric conditions are at increased risk for stroke. Understanding better the risk of stroke and bleeding in residents with AF and geriatric conditions would be valuable to

Improving outcomes for older adults in the emergency department: challenges, enablers and evidence-based solutions.

Emergency departments (EDs) are increasingly at the front line of care for older adults, a demographic experiencing rapid global growth. This patient group often presents with complex health conditions, polypharmacy, frailty and cognitive impairment, factors which challenge traditional acute care models. This article explores the clinical, organisational and educational challenges in delivering effective, person-centred emergency care for older adults. It presents evidence-based strategies to improve outcomes, drawing on current frameworks such as geriatric EDs and comprehensive geriatric asse

Geriatric Mental Healthcare Training: A Mini-Fellowship Approach to Interprofessional Assessment and Management of Geriatric Mental Health Issues.

INTRODUCTION: The population in United States aged 65 and older has rapidly grown and is projected to grow faster than any other segment of the population. Despite this demographic shift, the nation's geriatric workforce is shrinking. AIM: The primary goal of the fellowship was to form a learning collaborative that would help trainees in medicine, nursing, social work, pharmacy and occupational/physical therapy understand the roles of each discipline involved in the provision of geriatric mental healthcare and to enhance basic knowledge of common geriatric syndromes. METHODS: Faculty from th

Care Coordination and Optimization in Geriatric Surgery (COGS): A Pilot Program to Improve Surgical Outcomes in Older Adults.

Older adults undergoing surgery are at increased risk for complications, functional decline, and prolonged recovery due to frailty and other age-related vulnerabilities. To address these challenges, we developed the Care Coordination and Optimization in Geriatric Surgery (COGS) program, a pilot initiative integrating comprehensive geriatric assessment and targeted optimization into routine surgical care for Veterans. Using the Risk Analysis Index-Clinical (RAI-C) to identify higher-risk patients, the COGS team-comprising geriatric providers, nursing, and surgical staff-provides individualized

A novel multidimensional geriatric screening tool in the ED: evaluation of feasibility and clinical relevance.

PURPOSES: Geriatric problems frequently go undetected in older patients in emergency departments (EDs), thus increasing their risk of adverse outcomes. We evaluated a novel emergency geriatric screening (EGS) tool designed to detect geriatric problems. BASIC PROCEDURES: The EGS tool consisted of short validated instruments used to screen 4 domains (cognition, falls, mobility, and activities of daily living). Emergency geriatric screening was introduced for ED patients 75 years or older throughout a 4-month period. We analyzed the prevalence of abnormal EGS and whether EGS increased the number

Outcomes from the work of registered nurses working with older people in UK care homes.

AIMS AND OBJECTIVES: This research sought to illuminate the distinct contributions made by Registered Nurses (RNs) and Care Assistants (CAs) to outcomes for older people in UK (nursing) care homes and to identify the outcomes of their work. This paper reports on aspects relevant to RNs. BACKGROUND: Older people living in long-term residential care settings around the world are among the most vulnerable individuals within their communities and those with the most complex needs. Nursing has historically been fundamental in the delivery of these services but, in some countries, the role of Regis
