Ageing of the eye affects all ocular structures. Presbyopia is an early manifestation of physiological ageing resulting in the impossibility to read without additional spectacle correction. Some systemic disorders may interfere with the physiological ageing of the eye and cause blindness. In the western countries blindness is caused by age-related macular degeneration (26%), glaucoma (20.5%), cataract (11.2%), diabetic retinopathy (8.9%) and ischemic optic neuropathy (4 %).

The subject of tinnitus in the population extremes-children and the elderly-is ignored by the literature, probably because children do not complain of tinnitus spontaneously, whereas it is only one challenge among other major health problems in the elderly. A short review of the literature on this subject is presented. Presbytinnitus, defined as tinnitus that accompanies the progressive hearing loss of presbycusis is classified as: type 1 (normal aging affecting the cochlea), and type II (preexistent sensorineural hearing loss accompanied by multiple systemic complaints, especially of sensory

An acute abdomen requires prompt diagnosis and therapy in all age groups. In the elderly population, rapid recognition is particularly important, because the patient may have delayed seeking medical care and may have uncommon illnesses complicating the ultimate course. The causes of an acute abdomen in this population are similar to those found in younger patients with some differences in frequency. In each case, a sense of urgency to make the proper diagnosis and then institute appropriate therapy is critical.

Urinary incontinence (UI) is now recognized as a prevalent, physically and emotionally disruptive, and costly health problem in the geriatric population. Because incontinence may be a manifestation of a subacute or reversible process within or outside of the lower urinary tract, and because effective treatment is available, it is important for primary care physicians to identify and appropriately assess incontinence in their geriatric patients. The initial evaluation of an incontinent geriatric patients. The initial evaluation of an incontinent geriatric patient includes a targeted history and

The life expectancy of our citizens has increased due to better living conditions and medical care. This poses new challenges to dental surgeons because geriatric dental patients have different needs brought on by ageing. Tooth loss, periodontal disease, alveolar bone resorption, mucosal changes, edentulism are just some of the problems faced by the elderly. Recognition of the dental needs of this special category of the population compels us to bear the responsibility of treating them now and in the future. We require a gerodontology programme that emphasizes prevention, maintenance of the or

Two hundred twenty-two patients admitted to a geriatric psychiatry unit were surveyed for the commission of acts dangerous to others. Eighteen of these patients had used either guns or knives in acts of violence. Organic illness accounted for a minority of such acts, the majority having been perpetrated by patients with functional diagnoses of late paraphrenia, schizophrenia, or mania. The most dangerous patients were those who, in a clear sensorium, experienced paranoid delusions, hallucinations, or both, and believed that they were in danger of being attacked. One hundred twenty-one aggressi

Elderly Americans make up 11 percent of the population but consume 25 percent of all prescription medications. The incidence of adverse drug effect is 1.5 to 3 times higher in older patients and accounts for numerous hospitalizations. The basic pharmacokinetic parameters of absorption, distribution, metabolism, and excretion are all significantly altered in geriatric patients. Certain specific drug side effects, such as orthostatic hypotension and anticholinergic effects, can be particularly hazardous in older patients. This paper describes general therapeutic principles for geriatric patients

Primary hyperparathyroidism occurs in a significant number of patients over 60 years of age; about one-third of the 182 patients we studied were in this age group. Primary hyperparathyroidism has been recognized with greater frequency in recent years, particularly in patients without manifestations of renal lithiasis or osteitis fibrosa cystica and in older patients. Age alone is not critical in the occurrence of serious complications from hypercalcemia, the presence of other life-endangering disease, or the development of major postoperative complications in the management of primary hyperpar

BACKGROUND: The aim of this study was to evaluate ultrasound as a routinely used procedure and extended physical examination in geriatric pa … NCLUSION: The extended screening by ultrasound provided important answers to classical questions in geriatrics (e.g. urinary retention, volume deficiency/pleural effusion) in many cases. The new findings had therapeutic consequences in one fifth of the patients. The HHUSD can be used in screening.

PURPOSE: Worldwide, the incidence of chronic fungal rhinosinusitis (CFRS) has increased. Although ageing leads to weakening of the immune system, which increases susceptibility to CFRS, the CFRS characteristics in geriatric patients are unclear. Therefore, we comparatively analysed the clinical characteristics of CFRS in geriatric and non-geriatric patients.
METHODS: This retrospective analysis compared the demographics, rhinologic symptoms, multiple allergen simultaneous tests, olfactory function tests, paranasal sinus computed tomography findings, and outcomes of 131 patients with CFRS who

The Comprehensive Geriatric Assessment (CGA) and the corresponding geriatric interventions are beneficial for community-dwelling older persons in terms of reduced mortality, disability, institutionalisation and healthcare utilisation. However, the value of CGA in the management of older cancer patients both in terms of clinical outcomes and in cost-effectiveness remains to be fully established, and CGA is still far from being routinely implemented in geriatric oncology. This narrative review aims to analyse the available evidence on the cost-effectiveness of CGA adopted in geriatric oncology,

BACKGROUND: The Age-Friendly Health Systems (AFHS) movement, guided by the 4Ms framework has been widely adopted to improve care for older adults. While specialized inpatient models such as Acute Care for Elders (ACE) units and Geriatrics Consults aim to improve outcomes, gaps remain in understanding how these models align with 4Ms adherence and their relationship to patient outcomes.
METHODS: We conducted a retrospective observational study of 28,833 hospital encounters for patients aged ≥ 65 at an academic health system. Encounters were categorized into four groups: Usual Care, Geriatrics C…

Geriatrics can enable and monitor a holistic care of older people through a comprehensive geriatric assessment in a structured way. Therefore, it must be integrated much more closely with preventive, rehabilitative and acute care units. Geriatrics are not seen in any aspects as a replacement for general practitioners or in-hospital structures but much more as a supplement to them. With its function-oriented concept, geriatrics can best coordinate the demographically necessary triage between prevention, acute treatment, rehabilitation and palliative care, thus avoiding undertreatment and overtr…
