BACKGROUND: Gonadal dysfunction and infertility are potential late effects of cancer therapy. Ifosfamide, an alkylating agent structurally related to cyclophosphamide, is thought to cause gonadal dysfunction, though there is little published evidence.
PROCEDURE: Patients treated on sarcoma protocols containing ifosfamide as the only potential gonadotoxic agent, were evaluated, assessing pubertal development, menstrual history in the females and semen analysis in males. Biochemical evaluation included measurement of gonadotrophins, inhibin B and anti-mullerian hormone (AMH).

An eighteen years old girl came from Ishargang, Mymensingh complaining of short stature, absence of development of breast, lack of menstruation and other secondary sex characters. She was found in infantile appearance with a height of 123 cm, body weight of 28 kg. Three pediatric phenotypic females presented with gonadal dysgenesis. Their gonads were removed laparoscopically. These phenotypically normal females, who do not have any intersex problems or ambiguous genitalia, represent a unique group of patients having a Y chromosome or a fragment of it in their genetic constitutions. Total 206 gonadal teratomas were diagnosed in Christian Medical College and Hospital, Ludhiana from January 1980 to December 1990. 182 of them were in ovary and other 24 in testis. Majority of them i.e. 45.1% of ovarian and 45.8% of testicular lesions were seen in third decade. On gross examination 161 (88.4%) of ovarian tumours were cystic one (0.5%) was completely solid, while 20 (10.9%) showed solid as well as cystic areas. A successful triplet gestation in a 45,X/46,XY woman is presented. A previously hypoplastic uterus was prepared for implantation by exogenous hormone replacement. Conception was achieved through in vitro fertilization of donor oocytes and transfer of four embryos into a hormonally primed endometrium. This case illustrates that some women with 45,X/46,XY karyotype can have a successful triplet pregnancy. Therefore, a conservative approach during gonadectomy in patients with a Y chromosome may be warranted.
