A 64-year-old black woman presented unilateral acute ocular hypertension. Gonioscopy showed a blood clot obstructing Schlemm's canal. Intraocular pressure returned to normal values after resorption of the blood clot. Hemoglobin electrophoresis found a sickle-cell trait, raising the hypothesis that the obstruction of outflow was probably due to the mechanism of sickling.

A 42-year-old woman presents with headache and anisocoria. Gonioscopy suggested narrow angles. Ultrasound biomicroscopy confirmed the diagnosis. BACKGROUND AND OBJECTIVE: To evaluate the ability of high-frequency ultrasound biomicroscopy to diagnose traumatic cyclodialyses not evident on clinical examination.
PATIENTS AND METHODS: Six eyes to six patients with posttraumatic hypotony and/or shallow anterior chamber and suspected cyclodialysis clefts were examined with slit-lamp biomicroscopy, gonioscopy, B-scan ultrasonography, and ultrasound biomicroscopy. Fluorescein angiography of the angle with the Goldmann gonioscopy lens was used to examine eyes of 100 Japanese patients with diabetes mellitus. Ocular tension was 21 mmHg or over in 31 of these 100 patients. An overview about the classification, diagnostic and therapy of congenital glaucomas is given. Tonometry, gonioscopy, corneal diameter measurements, oculometry and the evaluation of the optic disc are the main diagnostic criteria. Goniotomy is the operation of choice. The technique of goniotomy and their modifications leading to cyclodialysis ab interno are described. Own long-term results of goniotomy are finally demonstrated.

A 54-year-old woman who presented with photopsia was found to have elevated intraocular pressure in both eyes and optic disc cupping in the right eye. Angle infiltration was noted on gonioscopy. She was previously been diagnosed with metastatic breast cancer. This case report describes a rare case of glaucoma as a complication of ciliary body and iris metastases secondary to invasive ductal breast cancer.

BACKGROUND: Peripheral primary cyst of the iris is an uncommon benign condition that may masquerade as a tumor of the ciliary body. Careful examination utilizing the slit lamp and gonioscopy is necessary to successfully differentiate the two.
CONCLUSIONS: Given their uncommon occurrence and the importance of ruling out melanoma or other malignancies, this review is a guide in the evaluation and diagnosis of patients with this condition.
