Mortality following blood culture in premature infants: increased with Gram-negative bacteremia and candidemia, but not Gram-positive bacteremia.

OBJECTIVE: To describe survival following nosocomial bloodstream infections and quantify excess mortality associated with positive blood culture. STUDY DESIGN: Multicenter cohort study of premature infants. RESULTS: First blood culture was negative for 4648/5497 (78%) of the neonates--390/4648 (8%) died prior to discharge. Mortality prior to discharge was 19% in the 161 infants with Gram-negative rod (GNR) bacteremia, 8% in the 854 neonates with coagulase negative staphylococcus (CONS), 6% in the 169 infants infected with other Gram-positive bacteria (GP-o), and 26% in the 115 neonates with

Erysipelothrix rhusiopathiae endocarditis: microbiologic, epidemiologic, and clinical features of an occupational disease.

Ninety percent of the 49 reported cases of serious Erysipelothrix rhusiopathiae infection have been episodes of presumed or proved endocarditis. E. rhusiopathiae endocarditis correlates highly with occupation (farming, animal exposure), affects more males than females, exhibits a peculiar aortic valve tropism, displays a characteristic erysipeloid cutaneous lesion (in 40% of cases), and is associated with significant mortality (overall rate, 38%). Comparison with other unusual gram-positive rods causing endocarditis shows that E. rhusiopathiae resembles Listeria monocytogenes and Lactobacillus…

Actinomyces neuii and neonatal sepsis.

Actinomyces neuii belongs to the coryneform bacteria. In the case presented here this gram-positive rod had to be considered the pathogen causing not only the chorioam nionitis but also the neonatal sepsis. Conventional therapeutic regimes are effective due to the high sensitivity of A. neuii to beta-lactam antibiotics.

Meteorological factors and ambient bacterial levels in a subtropical urban environment.

We conducted a study to investigate the characteristics and determinants of ambient bacteria in Taipei, Taiwan from August 2004 to March 2005. We monitored ambient culturable bacteria in Shin-Jhuang City, an urban area in the Taipei metropolitan areas, using duplicate Burkard Portable Air Samplers with R2A agar. The average concentration of total bacteria was 1,986 colony-forming units per cubic meter of air (CFU/m(3)) (median = 780 CFU/m(3)) over the study period, with the highest level in autumn. Most bacterial taxa had similar seasonal variation, with higher concentrations in autumn and win

[Septicemia causative organisms 1983-1985. The results of a multicenter study].

8,999 septicaemia pathogens were isolated from 8,500 patients during a two-year period by eleven institutes in West Germany, two institutes in West Berlin und two institutes in Austria. 54% of the patients were male. Septicaemias occurred very often in neonates, rarely in the second decade of life, and then with rising incidence up to a maximum in the seventh decade. Gram-negative rods were isolated in 49% of the samples, gram-positive cocci in 45%, anaerobic bacteria in 2.5% and blastomycetes in 1.9%. The spectrum of pathogens in children was markedly different from that in adults. Slight dif

Emerging role of quinolones in the prevention of gram-negative bacteremia in neutropenic cancer patients and in the treatment of enteric infections.

The pharmacokinetic and antibacterial characteristics of the fluorinated carboxyquinolones make them attractive candidates for the treatment and prevention of infections arising from the alimentary canal. Norfloxacin and ciprofloxacin have been shown to suppress and eliminate the pool of potentially pathogenic aerobic gram-negative rods colonizing the alimentary canal of neutropenic patients with acute leukemia, thereby reducing infection-related morbidity and mortality due to gram-negative sepsis. Although norfloxacin appears to have limited efficacy in the prevention of gram-positive infecti

Endocarditis of the native aortic valve caused by Lactobacillus jensenii.

Lactobacilli are Gram-positive anaerobic rods or coccobacilli, commonly found as commensals in human mucosa. Rarely, they can cause serious infections such as infective endocarditis (IE), and the most frequently implicated species causing serious infections are L. casei and L. rhamnosus. IE caused by Lactobacillus jensenii is very rare, with only six reported cases so far, to the best of our knowledge. We present a case of native aortic valve endocarditis caused by L. jensenii, complicated by root abscess and complete heart block, and requiring emergent surgical intervention.

Pontomedullary encephalitis and basal meningitis due to Listeria monocytogenes: report of a case.

A case of CNS listeriosis in a 49-year-old man, previously in good health, is described. The illness showed a biphasic clinical pattern with a prodromal phase characterized by headache, fever and leukocytosis and subsequent development of pontomedullary cranial nerve paresis. The infection was rapidly progressive and fatal. Gram-positive rods, identified as Listeria monocytogenes, were detected in CSF samples only at the end stage of illness. Neuropathological abnormalities consisted of rhombencephalitis and basal meningitis. The major difficulties encountered in the early diagnosis of CNS lis…

[Characteristics of war wound infection].

War wounds are the most complex type of non-targeted injuries due to uncontrolled tissue damage of varied and multifold localizations, exposing sterile body areas to contamination with a huge amount of bacteria. Wound contamination is caused by both the host microflora and exogenous agents from the environment (bullets, cloth fragments, dust, dirt, water) due to destruction of the host protective barriers. War wounds are the consequence of destructive effects of various types of projectiles, which result in massive tissue devitalization, hematomas, and compromised circulation with tissue ische

Pulmonary nocardiosis with probable pulmonary aspergillosis in an elderly patient with COPD and T-cell lymphopenia.

Recovery of multiple microorganisms from respiratory specimens complicates pathogen attribution in patients with chronic lung disease. A 71-year-old woman with COPD and bronchiectasis presented with severe pneumonia and respiratory failure. She had T-cell lymphopenia (CD3+, CD4+, and CD8+ counts: 189, 147, and 41 cells/μL) and negative HIV testing. BALF Gram staining showed branching Gram-positive rods suspicious for Nocardia spp., and cultures yielded a Nocardia isolate, Aspergillus fumigatus, and Candida albicans. The isolate was presumptively identified as N. terpenica by MALDI-TOF MS (scor…

The apical border plaque in severe periodontitis. An ultrastructural study.

This study concerns the apical border (AB) plaque in relation to severe forms of periodontitis (SP), including juvenile, post-juvenile, and rapidly progressing periodontitis. Twenty-four (24) teeth from 16 patients with SP were examined by transmission electron microscopy (TEM). The AB was not discrete, with islands of bacteria in the so-called plaque-free zone (PFZ). Coronal to the AB the established plaque consisted of a layer of Gram-positive cocci and ghost cells and a superficial layer mainly of Gram-negative morphotypes, including cocci, rods, filaments, fusiforms, and spirochetes. The Microbiology of Hidradenitis Suppurativa.

Although the clinical presentation of Hidradenitis Suppurativa (HS) is strongly reminiscent of bacterial infection, the role of bacteria remains controversial. Studies have isolated an array of different bacterial specimens as well as biofilm formation in lesional HS skin. Consistent findings of Gram-positive cocci and -rods including Staphylococus aureus, Coagulase-negative staphylococci (CoNS) and Corynebacterium species (spp) in deep tissue samples have been demonstrated in HS. Although efficacy of antibiotics, i.e., rifampicin, clindamycin or tetracycline may support a microbial role in di

Experience with a novel selective medium for isolation of Actinomyces spp. from medical and dental specimens.

A selective medium containing mupirocin and metronidazole was evaluated for the isolation of Actinomyces spp. from clinical material. The study was a one-year prospective comparison of the method with existing methods, which use nonselective media, at a general hospital and a dental hospital. Significantly more Actinomyces spp. were isolated on the selective medium than on nonselective media from both dental specimens and intrauterine contraceptive devices. However, differentiating between Actinomyces spp. and related nonsporulating gram-positive rods remains a slow and sometimes uncertain pro…

Clinical, microbiological and host defense parameters associated with a case of localized prepubertal periodontitis.

A 4-year-old Japanese boy was referred to Osaka University Dental Hospital because of severe mobility and pain of the right lower primary canine. The canine had severe bone loss and a pocket depth exceeding 5-6 mm. The left lower canine showed slight mobility and moderate alveolar bone loss. The other primary teeth showed no pathogenic findings. The subgingival microflora from the right lower canine was dominated by gram-negative rods, especially capnocytophaga and fusobacterium, while actinomyces sp. were the most common gram-positive bacteria. Human Cellulosimicrobium Infections: A Clinically Oriented Review of Diagnostic and Therapeutic Challenges.

Cellulosimicrobium species, formerly classified within the genus Oerskovia, are environmental Gram-positive filamentous bacilli that rarely cause human infection. Their clinical significance may be underestimated because coryneform Gram-positive rods are frequently interpreted as contaminants in routine cultures. Publications on human infections are dominated by case reports and small case series, most often involving bloodstream infection, infective endocarditis, osteoarticular infection, peritoneal dialysis-associated peritonitis, and endophthalmitis. Reported cases commonly occur in patient…
