Liver rupture followed by multiorgan failure is perhaps the most catastrophic complication of the HELLP syndrome (hemolysis, elevated liver function, low platelets). Specific treatment options are currently limited and not routinely clarified. Here, we present a patient successfully managed by an innovative surgical approach consisting of combined total hepatectomy, portacaval shunt, and liver transplantation. A 26-year-old primipara (39th week of gestation) who suffered liver rupture as a complication of HELLP syndrome after delivery underwent a portacaval shunt after total hepatectomy. This

OBJECTIVE: To describe the maternal morbidity and mortality in pregnancies complicated by HELLP Syndrome.
STUDY DESIGN: This is a descriptive and prospective study of women with HELLP Syndrome managed at our center from January 1998 through March 2000. Patients are grouped into 3-class system of Mississippi classification.
RESULTS: 170 cases were analysed, 156 (92%), ocurred ante partum and 14 (8%) postpartum; 15 cases (9%) developed at < 27 weeks, 112 (66%) between 28 to 36 weeks of gestational age and 43 (25%) at term. Maternal morbidity included acute renal failure (13.5%), abruptio place

HELLP syndrome is a special type of severe preeclampsia that constitutes a management dilemma for obstetricians. The high maternal and perinatal morbidity (and rarely maternal mortality) that can result from this entity mandates continuing efforts to find an effective treatment or prevention. The traditional therapy for HELLP syndrome has been meticulous medical care of the mother, vigilant fetal surveillance, and timely delivery. Proposed new therapies to permit prolongation of preterm pregnancy and fetal maturation include vasodilation therapy using fluids and antihypertensives and intensive

HELLP syndrome is associated with a high rate of maternal and perinatal morbidity and mortality, and often leads to immediate fetal extraction. However, this condition may occur very early in pregnancy and conservative approaches have been recently proposed. The limits of this approach are discussed with two cases of conservative management of HELLP syndrome complicated by abruptio placentae.

A 33-year-old multiparous woman in the 34th gestational week presented with severe upper abdominal pain, nausea and vomiting. Clinical examination revealed severe epigastric tenderness. Abdominal ultrasound examination showed geographical areas with increased echogenicity in the right lobe of the liver. Through haematological examination, we found severe thrombocytopenia and fibrinolysis. The diagnosis of HELLP syndrome was suspected and a caesarean section was performed. We suggest that obstetrical patients with upper abdominal pain and abnormal liver sonography should immediately be haematol

Three cases of HELLP syndrome are presented which illustrate its varied clinical presentation, the difficulty in making a diagnosis and the management problems. The cases varied in severity; all showed thrombocytopenia and abnormal liver function tests, but in one case the blood pressure remained within "normal limits" whereas another patient suffered grand mal seizures due to hypertensive encephalopathy.

Preeclampsia is a multisystem disorder of pregnancy whose clinical presentation is related to the importance and the extent of maternal microvasculature damage. 16 patients with preeclampsia, thrombocytopenia (< 100,000/mm3), elevated liver enzymes and hemolysis are described. Weinstein in 1982 assigned the acronym of Hellp syndrome for this clinical presentation of preeclampsia. This syndrome was seldom recognized during the first years of the study but was diagnosed later on, in 1 pregnancy out of 600 parturitions and in 5% of preeclampsia in 1991. Two out of the patients developed eclamptic

During a 5-year period, 13 cases of HELLP syndrome were seen in hypertensive patients in the Obstetrics Department, Santa Maria Hospital, Lisbon. In 7 cases the syndrome occurred antepartum and in 6 during early early puerperium; gestational age and clinical features were very different from case to case. Four cases of eclampsia and 2 cases of acute renal failure occurred in 5 patients; there were 2 intrauterine deaths. Maternal mortality was nil. HELLP syndrome always implicates an important hepatic involvement with deleterious effects on the clinical status of the patients. Since the syndrom

The syndrome of haemolysis, elevated liver enzymes and low platelet count (HELLP-Syndrome) is a severe form of preeclampsia and eclampsia. The clinical course is characterized by right upper quadrant and epigastric pain, hypertension, proteinuria and edema. Maternal and neonatal morbidity are high. The underlying cause for this pregnancy-related syndrome is still unclear. As soon as a reliable diagnosis is established handling of patients suffering from HELLP-Syndrome is ambivalent: Immediate termination of pregnancy, however, poses a problem at early gestational age. Therefore some authors ha

Spontaneous hepatic rupture in pregnancy is a rare, but dangerous complication of acute or progressing pre-eclampsia. Pain in the right upper quadrant is the most important clinical symptom and should prompt immediate laboratory evaluation (transaminase, coagulation profile, platelet count, red and white blood count). If the diagnosis of HELLP syndrome is confirmed, close supervision in an intensive care unit is mandatory. The danger of pre- or postpartal open hepatic rupture must be considered. Pain in the right upper abdomen and signs of abdominal mass bleeding occurring within a HELLP syndr

The HELLP syndrome is a severe and life-threatening complication of preeclampsia. Despite obstetricians are more aware of this syndrome maternal and perinatal mortality could not be reduced during the last years. The diagnostic problems of the HELLP syndrome are early detection of the disease, problems concerning differential diagnosis to other diseases with similar symptoms, and the different definition of the HELLP syndrome itself. For early detection of hemolysis determination of serum haptoglobin should be included in laboratory screening methods. Manifestation of DIG is not a principal si

Pregnancies complicated by HELLP syndrome require a well-formulated management plan. The development of this syndrome after 34 weeks' gestation or with documentation of fetal lung maturity is an indication for delivery. Vaginal delivery can be accomplished in most cases; however, if cesarean section is required, the use of general anesthesia, subfascial drains, and preoperative platelet transfusion for platelet counts less than 50,000/mm3 can reduce the incidence of complications. It is advisable that patients with complications of HELLP syndrome such as pulmonary edema, acute renal failure, l

In the last trimester of an uncomplicated pregnancy a woman developed a spontaneous vitreous haemorrhage. She was later diagnosed as having HELLP syndrome, a recently described disorder comprising haemolytic anaemia, elevated liver enzymes, and a low platelet count in women with severe pre-eclampsia or eclampsia. We believe this rare complication of pregnancy to be the cause of the intraocular bleeding.
