The Functional and Imaging Implications of Left Bundle Branch Pacing in Ischemic Cardiomyopathy.

Heart failure with reduced ejection fraction due to ischemic cardiomyopathy remains a significant clinical challenge. Electrical conduction delays exacerbate symptoms by causing uncoordinated contractions, reducing pumping efficiency, and increasing mortality. Right ventricular pacing further worsens dyssynchrony, while resynchronization therapy improves outcomes but has a high non-responder rate. Given these limitations, bundle branch pacing engages the heart's conduction system, restoring synchronized contraction and enhancing cardiac function. This review examines the impact of left-bundle-

A case of complete heart block in a COVID-19 infected patient.

In the midst of the COVID-19 pandemic, we herein report the case of an elderly female with multiple comorbidities coming with typical symptoms of the viral infection in addition to the unusual presentation of bradycardia due to complete heart block requiring pacemaker placement. This may be a rare complication of the disease but one has to keep a high index of suspicion since this virus has an ability to affect multiple organ systems with many ways yet to be uncovered. <Learning objective: 1. COVID-19 might affect the conduction system of the heart as part of its disease process. 2. Future stu

The cardiac pathology of sudden, unexplained nocturnal death in Southeast Asian refugees.

Sudden death during sleep has occurred among previously healthy Southeast Asian male refugees, but routine autopsies have not determined the cause of death in any of these cases. We report the first systematic attempt to define the cardiac abnormalities associated with this syndrome. Among 18 hearts examined, 14 showed slight to significant cardiomegaly, characteristic of increased cardiac work load. The reasons for the cardiomegaly remain unexplained. Conduction system anomalies were present in all but one heart. These included persistent fetal dispersion of the atrioventricular node and/or b

[Physiological pacing and cardiac resynchronization therapy: state of the art].

Permanent cardiac pacing has progressively evolved toward increasingly physiological strategies aimed at preserving or restoring ventricular electromechanical synchrony. Cardiac resynchronization therapy using biventricular pacing remains the established standard of care for patients with heart failure, reduced left ventricular ejection fraction and electrical dyssynchrony, particularly in the presence of left bundle branch block. However, anatomical constraints, suboptimal clinical and echocardiographic response, and procedural complexity have promoted the development of conduction system pac

Engineering a conduction-consistent cardiac patch with rGO/PLCL electrospun nanofibrous membranes and human iPSC-derived cardiomyocytes.

The healthy human heart has special directional arrangement of cardiomyocytes and a unique electrical conduction system, which is critical for the maintenance of effective contractions. The precise arrangement of cardiomyocytes (CMs) along with conduction consistency between CMs is essential for enhancing the physiological accuracy of in vitro cardiac model systems. Here, we prepared aligned electrospun rGO/PLCL membranes using electrospinning technology to mimic the natural heart structure. The physical, chemical and biocompatible properties of the membranes were rigorously tested. We next as

Acute effects of cocaine on catecholamines and cardiac electrophysiology in the conscious dog.

To evaluate the electrophysiologic and neurohumoral effects of cocaine on the sinus node, atrium, atrioventricular node, and His-Purkinje system, 14 conscious dogs were studied before and after an infusion titrated to maximum tolerance or to increase the systolic blood pressure by at least 15%. Plasma cocaine and catecholamine levels, blood pressure, surface and intracardiac electrograms were recorded. Programmed electrical stimulation was performed from the right atrium. The sinoatrial conduction time, paced PR interval, atrioventricular nodal conduction time, atrioventricular nodal effective

Novel lamin A/C mutations in two families with dilated cardiomyopathy and conduction system disease.

BACKGROUND: The LMNA gene, one of 6 autosomal disease genes implicated in familial dilated cardiomyopathy, encodes lamins A and C, alternatively spliced nuclear envelope proteins. Mutations in lamin A/C cause 4 diseases: Emery-Dreifuss muscular dystrophy, limb girdle muscular dystrophy type 1B, Dunnigan-type familial partial lipodystrophy, and dilated cardiomyopathy. METHODS AND RESULTS: Two 4-generation white families with autosomal dominant familial dilated cardiomyopathy and conduction system disease were found to have novel mutations in the rod segment of lamin A/C. In family A a missense

Masquerading bundle branch block: An electrocardiographic harbinger of advanced conduction system disease.

BACKGROUND: Masquerading bundle branch block (MBBB) is a rare and underrecognized electrocardiographic pattern characterized by features of both right and left bundle branch block on the same tracing. Unlike typical bifascicular block, MBBB is increasingly recognized as a marker of advanced and often bilateral His-Purkinje system disease, with potential progression to high-grade atrioventricular block. CASE PRESENTATION: We report the case of an 84-year-old woman with a history of atrial fibrillation, heart failure with preserved ejection fraction, chronic kidney disease, and aortic stenosis

Permanent Pacemaker Timing Dilemma in Recovered Lymphocytic Myocarditis.

BACKGROUND: The timing of permanent pacemaker implantation in myocarditis-related complete heart block remains challenging because no established criteria exist for reversibility of the conduction system. CASE SUMMARY: A previously healthy 50-year-old Asian woman presented with lymphocytic myocarditis complicated by cardiogenic shock and complete atrioventricular block (CAVB). Despite successful hemodynamic stabilization with Impella CP support and complete recovery from left ventricular dysfunction, CAVB persisted. A permanent pacemaker was implanted on day 28. Afterward, cardiac enzymes and

Synchronous ventricular pacing with direct capture of the atrioventricular conduction system: Functional anatomy, terminology, and challenges.

Right ventricular apical pacing is associated with an increased incidence of heart failure, atrial fibrillation, and overall mortality. As a result, pacing the ventricles in a manner that closely mimics normal AV conduction with an intact His-Purkinje system has been explored. Recently, the sustainable benefits of selective His-bundle stimulation have been demonstrated and proposed as the preferred method of ventricular stimulation for appropriate patients. Ideally, conduction system pacing should be selective without myocardial capture, overcome distal bundle branch block when present, and no

Conduction System Pacing for Cardiac Resynchronisation.

Conduction system pacing (CSP) is a technique of pacing that involves implantation of permanent pacing leads along different sites of the cardiac conduction system and includes His bundle pacing and left bundle branch pacing. There is an emerging role for CSP to achieve cardiac resynchronisation in patients with heart failure with reduced ejection fraction and inter-ventricular dyssynchrony. In this article, the authors review these strategies for resynchronisation and the available data on the use of CSP in overcoming dyssynchrony.

The cardiac conduction system in progressive systemic sclerosis. Clinical and pathologic features of 35 patients.

Progressive systemic sclerosis may be associated with focal myocardial fibrosis. Electrocardiographic abnormalities including conduction block are common in progressive systemic sclerosis but whether they are due to direct destruction of the specialized conduction tissue of the heart is uncertain. The conduction systems of 35 patients with progressive systemic sclerosis were studied. Of these 35 patients, 17 (50 per cent) had myocardial fibrosis of the type seen in progressive systemic sclerosis. In 10 of the 17, it was severe. Sinus node fibrosis was present in 13 patients and was nearly as f

Revolutionizing cardiovascular medicine: targeted therapies for the cardiac conduction system.

Arrhythmogenic cardiovascular disorders are associated with considerable morbidity and mortality. Whether cardiac conduction disease is caused by genetic defects, procedural perturbations, valvular disease, ischemia, aging, or heart failure, new therapies are warranted. In this issue of the JCI, Goodyer et al. used state-of-the-art technologies to image the cardiac conduction system (CCS) in real time and to deliver targeted therapies to the CCS and its subcomponents. These findings advance the ability to image and treat specific lineages within the adult heart with the potential for broader a

Atrioventricular Delay Optimization in His-Optimized Cardiac Resynchronization Therapy: A Case Report.

His bundle pacing (HBP) has recently emerged as a physiologic pacing strategy that preserves ventricular synchrony and may improve outcomes in patients with heart failure and conduction abnormalities. Optimization of the atrioventricular (AV) delay plays a pivotal role in ensuring hemodynamic efficiency in patients with conduction system pacing. We report a 41-year-old male with dilated cardiomyopathy and severe heart failure (NYHA class IV, EF 16%), who was initially implanted with a cardiac resynchronization therapy (CRT) device. Despite multiple attempts to reposition the right and left ven

A minimal electrical model of the human heart.

We develop a minimal whole-heart model that describes cardiac electrical conduction and simulate a basic three-lead electrocardiogram (ECG). We compare our three-lead ECG model with clinical data from a Norwegian athlete database. The results demonstrate a strong correlation with the ECGs recorded for these athletes. We simulate various pathologies of the heart's electrical conduction system, including ventricular tachycardia, atrioventricular nodal reentrant tachycardia, accessory pathways, and ischemia-related arrhythmias, showing that the three-lead ECGs align with the clinical data.
