Fulminant cytomegalovirus myocarditis in an infant with concomitant large atrial and ventricular septal defects: medical intervention strategy for functional cardiac regeneration.

A cytomegalovirus-associated heart failure in a young infant with atrial and ventricular septal defects is reported in this case report. The patient recovered by an anti-congestive and anti-viral therapy with an extra percutaneous transcatheter treatment strategy. In the context of bi-ventricular predominant right heart failure associated with supra-systemic pulmonary hypertension, the already closed arterial duct was re-opened and stented to unload the right ventricle and thereby augment the systemic blood flow. Either the left-to-right shunting atrial septal defect or bi-directional shunting

Cardiac troponin T is necessary for normal development in the embryonic chick heart.

The heart is the first functioning organ to develop during embryogenesis. The formation of the heart is a tightly regulated and complex process, and alterations to its development can result in congenital heart defects. Mutations in sarcomeric proteins, such as alpha myosin heavy chain and cardiac alpha actin, have now been associated with congenital heart defects in humans, often with atrial septal defects. However, cardiac troponin T (cTNT encoded by gene TNNT2) has not. Using gene-specific antisense oligonucleotides, we have investigated the role of cTNT in chick cardiogenesis. TNNT2 is exp

Coexistence of congenital heart anomalies with noncompaction of the ventricular myocardium. Two case reports.

Noncompaction of the ventricular myocardium (NVM) is a rare cardiac anomaly which can lead to the development of cardiomyopathy. NVM can be isolated or associated with congenital heart anomalies such as ventricular septal defect, atrial septal defect, pulmonic stenosis, bicuspid aortic valve, and Ebstein anomaly. NVM usually presents with heart failure symptoms, ventricular tachyarrhythmias, and thromboembolic events. We report two cases of NVM, one associated with a ventricular septal defect and other associated with bicuspid aortic valve.

Beyond atrial septal defect closure, it is time to start seriously considering closing ventricular septal defects with devices.

PURPOSE OF REVIEW: The purpose of this review is to discuss the history and current state of transcutaneous device closure of ventricular septal defects. RECENT FINDINGS: Newer devices and techniques have expanded the role of transcutaneous device closure of ventricular septal defects. Device closure of ventricular septal defects is a mainstay of therapy in many parts of the world. Device closure in smaller infants has relatively higher risks than larger infants and older children. The potential for the development of complete heart block remains a concern with this procedure. SUMMARY: Trans

Echocardiographic assessment of atrioventricular canal defects.

AV canal defects (AVCD) are caused by maldevelopment of the endocardial cushions and typically include a primum atrial septal defect (ASD), an inlet ventricular septal defect (VSD), and a common atrioventricular valve. The variations in deformities provide the basis for the many terms used in the anatomical classifications: partial, transitional, intermediate, and complete common AVCD (balanced or unbalanced). The balanced complete common AVCDs are classified as Rastelli A, B, C depending on the anomaly of the anterior bridging leaflet division and attachments. Unbalanced complete AVCDs occur

Ventricular septal defects in two goats.

Two young goats with loud, holosystolic heart murmurs heard best over the left and right heart base were suspected to have interventricular septal defects. Cardiac catheterization with angiocardiography supported the clinical diagnosis in both goats and the ventricular septal defects were confirmed at necropsy. In one goat a ventricular septal defect was associated with a hypoplastic aorta and in the other, an ectopic ureter opened into the vagina.

Topsy-turvy heart: A first recorded case report with intracardiac anomaly.

Topsy-turvy heart is a rare congenital anomaly; it has a specific characteristic of cardiac malrotation and superior-inferior arrangement of right/left ventricles. A five-day-old patient was diagnosed antenatally with hypoplastic left heart and right hydronephrosis and had been admitted to the hospital with symptoms of respiratory distress. The postnatal imaging studies revealed an abnormal ventricular arrangement with a large aortopulmonary window, ventricular septal defect (VSD), and the upside-down orientation resulting in the posterior-inferior displacement of the common arterial confluenc

Holt-oram syndrome in adult presenting with heart failure: a rare presentation.

Holt-Oram syndrome is a rare inherited disorder involving the hands, arms, and the heart. The defects involve carpal bones of the wrist and the thumb and the associated cardiac anomalies like atrial or ventricular septal defects. Congenital cardiac and upper-limb malformations frequently occur together and are classified as heart-hand syndromes. The most common amongst the heart-hand disorders is the Holt-Oram syndrome, which is characterized by septal defects of the heart and preaxial radial ray abnormalities. Its incidence is one in 100,000 live births. Approximately three out of four patien

Echocardiographic evaluation of left ventricular function, mass and wall stress in children with isolated ventricular septal defect.

M-Mode echocardiography was performed in 22 normal children and 22 children with ventricular septal defects. Left ventricular and left atrial chamber dimensions and wall thicknesses were measured in all patients. Utilizing these data, indices of left ventricular function were derived: shortening fraction, velocity of fiber shortening, peak diastolic fiber lengthening, end-systolic wall stress, radius thickness ratio, and ventricular mass. The results showed that ventricular septal defect was associated with enlarged left ventricular and atrial dimensions and increased shortening fraction, but

Corrected transposition of the great vessels associated with an obscured ventricular septal defect due to an abnormal systemic atrioventricular valve (report of a case and surgical considerations).

A case of corrected transposition associated with atrial septal defect, obscured ventricular septal defect, mild pulmonary valvar stenosis, severe systemic atrioventricular valve regurgitation and complete heart block is described. Closure of an atrial septal defect and a ventricular septal defect was carried out through a left atriotomy. Systemic atrioventricular valve replacement with a 27 mm. Hancock porcine valve was performed through the same approach. A permanent Demand rechargeable pulse generator unit with myocardial unipolar electrode lead was implanted. In the presence of complete he

Narrowing the Survival Gap: Trends in Survival of Individuals with Down Syndrome with and without Congenital Heart Defects Born 1979 to 2018.

OBJECTIVE: To evaluate the hypothesis that childhood survival for individuals with Down syndrome (DS) and congenital heart defects (CHDs) has improved in recent years, approaching the survival of those with DS without CHDs. STUDY DESIGN: Individuals with DS born from 1979 to 2018 were identified through the Metropolitan Atlanta Congenital Defects Program, a population-based birth defects surveillance system administered by the Centers for Disease Control and Prevention. Survival analysis was performed to evaluate predictors of mortality for those with DS. RESULTS: The cohort included 1671 in

Diagnosis and treatment of adults with congenital heart disease.

Approximately 50 million adults worldwide have known congenital heart disease (CHD). Among the most common types of CHD defects in adults are atrial septal defects and ventricular septal defects followed by complex congenital heart lesions such as tetralogy of Fallot. Adults with CHDs are more likely to have hypertension, cerebral vascular disease, diabetes and chronic kidney disease than age-matched controls without CHD. Moreover, by the age of 50, adults with CHD are at a greater than 10% risk of experiencing cardiac dysrhythmias and approximately 4% experience sudden death. Consequently, ad

Recurrent ventricular septal defect following closure CorMatrix: A case report.

Ventricular septal ruptures are an uncommon complication following acute myocardial infarction. Operative repair, utilizing a patch for closure of the defect, is the primary treatment modality to achieve hemodynamic stability. The use of an extracellular matrix derived from small intestinal submucosa as a scaffold for tissue repair is becoming increasingly common. Here, we present the case of a 58-year-old female found to have a ventricular septal rupture and posterior left ventricular aneurysm following late presentation after a myocardial infarction that required operative repair with a CorM
