AUDIENCE: The aim of this simulation is to educate emergency medicine interns, residents and advanced providers on the recognition, diagnosis, and management of heparin-induced thrombocytopenia (HIT). Communication, teamwork, and crisis resource management are incorporated into the case.
INTRODUCTION: Heparin-induced thrombocytopenia is a life-threatening complication of heparin exposure (eg, unfractionated heparin, low molecular weight [LMW] heparin) that occurs in a small percentage of patients exposed.1 It results in a consumptive coagulopathy in which the clotting cascade is inappropriate…

Nattokinase (NK) is a serine protease extracted from a traditional Japanese food called natto. Due to its strong fibrinolytic and thrombolytic activity, NK is regarded as a valuable dietary supplement or nutraceutical for the oral thrombolytic therapy. In addition, NK has been investigated for some other medical applications including treatment of hypertension, Alzheimer's disease, and vitreoretinal disorders. The most widely used clinical anticoagulants are heparin and low molecular weight heparins. The interactions between heparin and proteins modulate diverse patho-physiological processes a…

BACKGROUND: Unfractionated and low-molecular-weight heparin are commonly used in the prevention and therapy of a variety of cardiovascular diseases. Because the major side effects of these drugs are hemorrhagic events, very little attention is paid to another important side effect (ie, heparin-induced thrombocytopenia [HIT]). HIT is an immune-mediated transient prothrombotic state with very severe implications determined by thromboembolic phenomena in both the venous and arterial circulation.
DATA SOURCES: A PubMed search from 1995 to 2008 was performed. Pertinent literature was identified an…

PURPOSE OF REVIEW: Venous thromboembolism represents a potentially threatening complication in surgical and medical patients. Thromboprophylaxis showed a significant reduction of venous thromboembolic events, and low-molecular-weight heparins have been considered the standardized prophylactic regimen for a long time. The purpose of this review is to provide updated evidence on the use of low-molecular-weight heparins for prevention of venous thromboembolism after the publication of the latest American College of Chest Physicians Evidence-Based Clinical Practice Guidelines on antithrombotic and…

BACKGROUND: Maintaining a therapeutic level of anticoagulation with unfractionated heparin remains a major challenge for clinicians because of the wide variability of patient responses, which may be explained by variable binding of heparin to plasma proteins. Direct thrombin inhibitors may offer an advantage in more predictable anticoagulation.
METHODS: Plasma samples from normal volunteers, stable coronary artery disease (CAD) patients, unstable angina patients, and acute myocardial infarction patients were obtained. A fixed concentration of heparin (.13 U/ml) or bivalirudin (1.6 microg/ml) …

Unfractionated heparin, derived from porcine intestine, is the prototype of a rapidly acting anticoagulant. It has been used for over 60 years to arrest or prevent thrombus growth. Low-molecular-weight heparins, available in the last 20 years, are manufactured from unfractionated heparin and have superior dose-response relationships because of fewer nonspecific reactions with plasma proteins and cells. Fondaparinux is a recently approved five-saccharide synthetic molecule that carries the evolution of heparin further. It is a pure Xa inhibitor, with minimal nonspecific interactions. It does no…

Direct and indirect coagulation inhibitors are used to inhibit the activity of the serine proteases of the coagulation system. Indirect inhibitors act via antithrombin and heparin cofactor II. The main representatives are heparins, lowmolecular-weight heparins, fondaparinux, idraparinux and danaparoid. They bind to antithrombin and potentiate the inactivation of factor Xa and other serine proteases. Direct thrombin inhibitors bind reversibly to thrombin without cofactor. Anticoagulants are determined by global and specific anticoagulant methods. New anticoagulants are developed such as oral fa…

Low molecular weight heparins (LMWH) have become the reference drugs for the prevention and treatment of venous thromboembolism. Among the newer antithrombotic agents, the pentasaccharide is the most advanced and promising alternative of low molecular weight heparins. The pentasaccharide is a synthetic and specific inhibitor of activated coagulation factor X (factor Xa). The pentasaccharide is identical to the active sequence of heparin which is present in about a third of commercially available heparin mixtures. The pentasaccharide binds specifically to antithrombin III (ATIII) thus potentiat…

Heparin-induced thrombocytopenia (HIT) is an immune-mediated adverse effect of heparin that is important because of its relatively high frequency and its strong association with paradoxic venous and arterial thrombosis. As confirmatory laboratory assays are not always immediately available, physicians usually must make initial diagnostic and treatment decisions based on the clinical presentation alone. Three characteristic features of HIT can be helpful in distinguishing it from other causes of thrombocytopenia: (1) timing of the onset of thrombocytopenia, namely, a platelet count decrease tha…

Six-hundred twenty-nine patients with rheumatic mitral stenosis in normal sinus rhythm underwent percutaneous transvenous mitral commissurotomy (PTMC) by the standard Inoue balloon technique. All patients underwent transthoracic echocardiography, when necessary, transesophageal echocardiogram was done before PTMC to exclude left atrial clot. In all cases, the PTMC procedure was completed without administration of heparin. There was no incidence of embolism either in the immediate post-procedure period or at a median follow-up of 3 months. There were no femoral artery or venous complications in…

Two case reports on femoral nerve lesion due to hematoma in the iliopsoas muscle or as the case may be in the iliacus muscles on both sides are presented with review of the literature. Synopsis of the reported cases--certainly a minority of anticoagulant therapy complications nevertheless increasingly mentioned in the literature--suggests that there might be a higher risk of bleeding for women within the first week of so-called high-dose heparin therapy. Coagulation studies do not contribute essentially in preventing retroperitoneal haematoma since they are within therapeutic limits in many ca…

Heparin anticoagulation and its neutralization were monitored by three different techniques: a manual protamine titration, an automated activated coagulation time, and an automated protamine titration. All three techniques provided satisfactory information. The decision of which to use must be based on other considerations such as available manpower and cost of equipment. The effect of using the automated protamine titration test on heparin and protamine requirements, and on blood loss measured intraoperatively after bypass and in overnight chest bottle drainage in two groups of comparable pat…

Among 2,656 patients receiving heparin sodium therapy, bleeding was a dose-related phenomenon that occurred most commonly among women, severely ill patients, and patients who received asprin during heparin therapy. Except for dose, factors that predisposed patients to bleeding were more strongly associated with major bleeding than with minor bleeding. Heavy alcohol drinkers were at particularly high risk for major bleeding episodes during heparinization. The seven-day cumulative risk for any bleeding was 9.1%. Risk was greatest on the third day of heparin administration.
