Diagnostic pitfalls: how availability and anchoring biases lead to errors in dermatology.

OBJECTIVES: We will explore the diagnostic similarities of spindle cell neoplasms and the attributed heuristics that lead to misdiagnosis biases. The biases explored will include availability bias and anchoring bias, with a discussion on the events leading to their formation. CASE PRESENTATION: A 58-year-old African American male with a past medical history of well-controlled HIV presented to the dermatology clinic for a two-year history of several persistent skin nodules on his lower legs. One lesion on his left lateral calf, a 1.5 cm dome-shaped nodule with a centralized keratinous plug, wa

What type of relationship is learned during visual statistical learning?

Statistical learning enables observers to extract regularities from their environment, but what statistical regularity is extracted remains debated. While previous research has mainly focused on conditional probability, recent evidence suggests that observers may instead learn the uniqueness of predictive relationships. In two visual statistical learning experiments, we manipulated the strength and uniqueness of associations between two object stimuli. We contrasted the predictions of three metrics of associative strength, which incorporate the strength and uniqueness of the association differ

Ecologically rational choice and the structure of the environment.

In life, risk is reward and vice versa. Unfortunately, the big rewards people desire are relatively unlikely to occur. This relationship between risk and reward or probabilities and payoffs seems obvious to the financial community and to laypeople alike. Yet theories of decision making have largely ignored it. We conducted an ecological analysis of life's gambles, ranging from the domains of roulette and life insurance to scientific publications and artificial insemination. Across all domains, payoffs and probabilities proved intimately tied, with payoff magnitudes signaling their probabilitie

Reduced NEXI protocol for the quantification of human gray matter microstructure on the Connectome 2.0 scanner.

Biophysical diffusion MRI models such as Neurite Exchange Imaging (NEXI) are essential for probing gray matter microstructure, estimating compartment diffusivities (Di , De ), neurite fraction (f), and exchange time (tex ). However, NEXI multi-shell, multi-diffusion-time requirements lead to prohibitively long acquisitions. Leveraging the Connectome 2.0 ultra-high gradient scanner, we developed a time-efficient protocol using an Explainable AI (XAI) framework. Combining XGBoost, SHAP, and Recursive Feature Elimination trained on synthetic signals, XAI identified an optimal 8-feature subset, ha

Are current theories of panic falsifiable?

The authors examine 6 theories of panic attacks as to whether empirical approaches are capable of falsifying them and their heuristic value. The authors conclude that the catastrophic cognitions theory is least falsifiable because of the elusive nature of thoughts but that it has greatly stimulated research and therapy. The vicious circle theory is falsifiable only if the frightening internal sensations are specified. The 3-alarms theory postulates an indeterminate classification of attacks. Hyperventilation theory has been falsified. The suffocation false alarm theory lacks biological paramet

Happiness across the borders-A cross-sectional study among patients with psoriasis and atopic dermatitis in Europe.

BACKGROUND: Dermatological research has traditionally concentrated on evaluating mental comorbidities, neglecting positive concepts like happiness. Initial studies indicate that psoriasis and atopic dermatitis (AD) impair the happiness of those affected. Considering global happiness variations, this study aimed to explore the disease- and country-specific differences in disease-related quality of life and happiness, and potential influential factors on heuristic happiness among psoriasis and AD patients in Europe. METHODS: A cross-sectional multicentre study was conducted in dermatology depar

Does previous chemotherapy-induced nausea and vomiting predict postoperative nausea and vomiting?

BACKGROUND: Postoperative nausea and vomiting (PONV) remains a problem in the postoperative period. Previous PONV in oncology patients has recently been associated with chemotherapy-induced nausea and vomiting (CINV). We assessed if CINV could improve Apfel's heuristic for predicting PONV. METHODS: We conducted a retrospective study of 1500 consecutive patients undergoing intermediate or major cancer surgery between April and July 2011. PONV was assessed in the first postoperative day during post-anaesthesia care. The assigned anaesthetist completed an electronic medical record with all of th

A critique of Venkatagiri's hypothesis (2004) about the cause of stuttering.

This paper critiques a recent proposed causal hypothesis of stuttering. It is argued that the proposal is a classic example of the misuse of the representativeness heuristic in the search for the cause of the disorder.

Cadence (steps/min) and relative intensity in 61 to 85-year-olds: the CADENCE-Adults study.

BACKGROUND: We previously demonstrated that a heuristic (i.e., evidence-based, rounded yet practical) cadence threshold of ≥ 100 steps/min was associated with absolutely-defined moderate intensity physical activity (i.e., ≥ 3 metabolic equivalents [METs]) in older adults 61-85 years of age. Although it was difficult to ascertain achievement of absolutely-defined vigorous (6 METs) intensity, ≥ 130 steps/min was identified as a defensible threshold for this population. However, little evidence exists regarding cadence thresholds and relatively-defined moderate intensity indicators, including ≥ 6

Cognitive bias and severity of harm following surgery: Plan for workflow debiasing strategy.

INTRODUCTION: This study analyzes the relationship between cognitive bias (CB) and harm severity as measured by Clavien-Dindo Scores (CD). METHODS: A prospectively collected series of 655 severity matched general surgical cases with complications were analyzed. Cases were evaluated for CB and assigned harm scores as defined by CD grade. Potentially mitigating "debiasing" strategies were identified for each bias attribution. RESULTS: Among cases with CB, 24% (55/232) were CD(I-II) and 76% (177/232) were CD(III-V). Odds ratio suggests that serious complications occur nearly 60% more frequently

Characteristics of positive feedback provided by UK health service users: content analysis of examples from two databases.

BACKGROUND: Most feedback received by health services is positive. Our systematic scoping review mapped all available empirical evidence for how positive patient feedback creates healthcare change. Most included papers did not provide specific details on positive feedback characteristics. OBJECTIVES: Describe positive feedback characteristics by (1) developing heuristics for identifying positive feedback; (2) sharing annotated feedback examples; (3) describing their positive content. METHODS: 200 items were selected from two contrasting databases: (1) https://careopinion.org.uk/; (2) Nationa

'No Pink Ribbons': How Women's Lived Experiences With Breast Atypia Inform Decisions Involving Risk-Reducing Medications.

PURPOSE: Atypical hyperplasia (AH) is associated with a nearly 4-fold elevation of lifetime risk for breast cancer, and lobular carcinoma in situ (LCIS) is associated with a 7- to 8-fold risk. Women with AH/LCIS make numerous decisions in the course of treatment, including whether to take a risk-reducing medication, an option relatively few women pursue. We explored women's decision-making processes through patient narratives in an effort to inform decision supports for AH/LCIS. METHODS: We conducted in-depth interviews with 20 English-speaking women with AH/LCIS and no subsequent diagnosis o

Testing the validity of fluency as a metacognitive cue for judgments of distraction in serial recall.

When forming prospective judgments of distraction, people do not have direct metacognitive access to the cognitive effects of task-irrelevant sounds that determine auditory distraction. Instead, they rely on a fluency heuristic, using the subjective ease or difficulty with which a sound is processed as a metacognitive cue. Empirically, this conclusion is supported by metacognitive illusions, with judgments of distraction failing to align with objective distraction. Here we address why and when the fluency heuristic leads to incorrect or correct judgments of distraction. Specifically, we test t

How conceptualizing obesity as a disease affects beliefs about weight, and associated weight stigma and clinical decision-making in health care.

OBJECTIVES: This study empirically investigated how conceptualizing obesity as a disease (i.e., pathologizing obesity) affects beliefs about weight, and weight stigma and discrimination among health professionals. DESIGN: An experiment that manipulated the pathologization of obesity was completed by a multi-nation sample of health professionals from Australia, UK, and USA (N = 365). METHODS: Participants were randomly assigned to one of two conditions where they were asked to conceptualize obesity as a disease or not a disease; then presented with a hypothetical medical profile of a patient

Has the time come for a staging model to map the course of eating disorders from high risk to severe enduring illness? An examination of the evidence.

AIM: To examine the evidence to support using a staging heuristic for eating disorders, suggesting that the diagnosis of an eating disorder follows a trajectory across the life course. Specifically, to examine whether high-risk markers and prodromal features presenting in childhood and adolescence can later transition to the full manifestation of the illness in early adulthood, and whether over time, the illness can be described as becoming severe and enduring, often resistant to treatment. METHODS: We conducted a comprehensive literature search on the MEDLINE, PubMed, PsycINFO, EMBASE and Co

Practical approaches in evaluating validation and biases of machine learning applied to mobile health studies.

BACKGROUND: Machine learning (ML) models are evaluated in a test set to estimate model performance after deployment. The design of the test set is therefore of importance because if the data distribution after deployment differs too much, the model performance decreases. At the same time, the data often contains undetected groups. For example, multiple assessments from one user may constitute a group, which is usually the case in mHealth scenarios. METHODS: In this work, we evaluate a model's performance using several cross-validation train-test-split approaches, in some cases deliberately ig

Premature conclusions in the diagnosis of iron-deficiency anemia: cause and effect.

Case records of patients with iron-deficiency anemia were audited using a computer-based algorithm. In 24 of 35 cases, the diagnostic conclusion seemed unjustified and characteristic of premature closure, one of four recurring errors in diagnostic reasoning described previously. Premature closure appeared to originate from subjects at all levels of training, to be easily and unquestioningly accepted by other physicians, and to inappropriately condition diagnostic and therapeutic decisions. Heuristics and biases described by Tversky and Kahneman are considered as contributory factors and patien

Tip-of-the-tongue (TOT) states: retrieval, behavior, and experience.

The tip-of-the-tongue state (TOT) is the feeling that accompanies temporary inaccessibility of an item that a person is trying to retrieve. TOTs have been studied experimentally since the seminal work of Brown and McNeill (1966). TOTs are experiences that accompany some failed or slow retrievals, and they can result in changes in retrieval behavior itself, allowing us to study the interplay among experience, retrieval, and behavior. We often attribute the experience of the TOT to the unretrieved target, but TOTs are based on a variety of cues, heuristics, or sources of evidence, such as partia
