PURPOSE: Macrophages play a central role in the non-specific defense against viruses. We aim to report on the abundance of CD163+ foamy histiocytes in the vitreous humor of a patient with chorioretinitis secondary to West Nile Virus (WNV).

We describe 3 unusual cases of granuloma annulare with multiple macular lesions in a distribution that simulated mycosis fungoides in patients with no associated underlying diseases. Repeated biopsies showed typical well-formed palisading granulomas and no evidence of an atypical lymphocytic infiltrate. Malignant lymphoma with a high content of epithelioid histiocytes is a rare non-Hodgkin lymphoma first described by Lennert and Mestdagh (Lennert's lymphoma) in 1968. Multicentric reticulohistiocytosis (MRH) is a rare multisystem disease characterized by skin nodules and a destructive polyarthritis. We describe cardiac disease in a 42-year-old African American female with a history of MRH for 29 years and dilated cardiomyopathy for 5 years. During a congestive heart failure flare, the patient was slow to respond to standard treatment. A left/right heart catheterization and endomyocardial biopsy were performed, and the patient was found to have evidence of myocardial MRH infiltration.

BACKGROUND: Histiocytic diseases are significantly rarer than diseases derived from lymphocytic, plasmacytic, or myeloid lineages, and thus are encountered infrequently in hematology and oncology clinics. The most common form is Langerhans cell histiocytosis, which in adults has an incidence of 1-2 cases per 1 million; the others are considerably rarer, with their occurrence reported only by the number of described cases rather than through incidence or prevalence. Their rarity leads to delays in establishing an accurate diagnosis.
