Various factors influencing bone turnover and bone loss in rheumatoid arthritis (RA) are illustrated using the example of a postmenopausal woman with a highly active RA. In particular, the relationships between disease activity, vitamin D metabolism, parathyroid hormone (PTH) levels and calcium metabolism are described. We investigated immunologic function in six cases with hypophosphatemic vitamin D-resistant rickets (VDRR) before and after treatment with 1 alpha-hydroxycholecalciferol (1 alpha(OH) vitamin D3). Serum 25-hydroxycholecalciferol (25[OH]D3) levels and other parameters of vitamin D nutriture were examined in 58 subjects aged 70 or more, living in Jerusalem. They were compared with those of 54 young adults living in the same neighbourhood. No evidence was obtained of a lower level of vitamin D nutriture in the elderly compared to younger adults. The effect of hypomagnesemia occurring in phosphate deprivation on elevated levels of plasma 1,25-dihydroxycholecalciferol (1,25(OH)2D3) was evaluated in rats. Phosphate deprivation was associated with an elevated plasma 1,25(OH)2D3, hypophosphatemia, and hypomagnesemia. Serial 25-hydroxy vitamin D (25-OHD) concentrations were measured in long-stay geriatric patients treated with vitamin D. Comparison between a treatment and a control group showed that a daily dose of 500 IU vitamin D produced a significant increase in 25-OHD levels by two months. The supplement had a striking effect when the initial 25-OHD level was low and very little effect when it was high. 25-OHD levels in subjects on 2000 IU vitamin D daily were only marginally higher than those in subjects on 500 IU. OBJECTIVES: The objective of this systematic literature review was to evaluate the effect of obesity and/or body mass index (BMI) on radiographic findings of spondyloarthritis (SpA) for both axial and peripheral inflammation and damage.
METHODS: Medline, Embase and Cochrane databases were screened on February 13, 2017. The titles and the abstracts were independently screened by two investigators. Articles that have evaluated the link between BMI and plain radiography, ultrasound (US) and magnetic resonance imaging (MRI) in SpA were investigated.

An LD50 of 0.2 mg/kg body wt has been determined for 1 alpha-hydroxyvitamin D3 in the rat. In comparison, the LD50 for 1 alpha-hydroxyvitamin D2 is between 3.5 and 6.5 mg/kg. A maintenance dosis of 2 microgram/day of 1 alpha-OH-D3 could keep the level of serum calcium normal in all 5 cases with pseudohypoparathyroidism, in contrast with 4.0 +/- 1.26 microgram/day needed for the 11 cases with hypoparathyroidism (8 idiopathic and 3 postoperative). The conspicuous effect of 1 alpha-OH-D3 on pseudohypoparathyroidism is likely to be attributed to the fact that the unresponsiveness of bone tissue to parathyroid hormone is corrected by the action of active vitamin D.
