DESIGN: Nested prospective cohort study as part of the Australian Snakebite Project.
PATIENTS AND SETTING: Patients receiving snake antivenom in Australian hospitals between 1 January 2002 and 30 November 2007.

Regulation of the immune response requires the cooperation of multiple signals in the activation of effector cells. For example, T cells require signals emanating from both the TCR for antigen (upon recognition of MHC/antigenic peptide) and receptors for costimulatory molecules (e.g., CD80 and CD60) for full activation. Avian schistosomes are the primary causative agent of cercarial dermatitis in humans, but despite its worldwide occurrence, little is known of the immune mechanism of this disease. An association between rapid reversion of supraventricular tachycardia by carotid sinus stimulation and carotid sinus reflex hypersensitivity during sinus rhythm is described in a 55-year-old patient. The findings indicate that a high level of vagal tone facilitates vagal maneuvers in terminating some supraventricular tachyarrhythmias. The briskness of the response during tachycardia may also be a marker for underlying carotid sinus hypersensitivity.

We present a case of significant envenomation from a severed, preserved rattlesnake head in a 22-year-old man. The patient was treated successfully with 15 vials of Crotalidae polyvalent antivenin despite a history of severe immediate hypersensitivity reaction to antivenin. The patient developed a mild case of serum sickness five days after antivenin infusion that was treated successfully with a course of steroids. The patient experienced complete recovery. This case demonstrates the hazard of even preserved snake heads and fangs.

BACKGROUND: Rituximab (RTX) has been associated with immediate hypersensitivity reactions (HSRs) involving multiple phenotypes and endotypes.
OBJECTIVE: To characterize the clinical phenotypes and endotypes of immediate RTX HSRs.
METHODS: A prospective study was conducted as part of a pharmacovigilance program from July 2020 to December 2021. Participants who experienced immediate RTX HSRs were enrolled. Skin testing, basophil activation testing (BAT), IFN-γ ELISpot assays, and IgG anti-drug antibody (ADA) ELISA assays were performed.

BACKGROUND: Non-immediate drug hypersensitivity reactions (DHRs) in children are frequently overdiagnosed, leading to unnecessary drug avoidance. Evidence regarding predictors of more extensive reactions, drug provocation test (DPT) outcomes, and re-exposure behavior and avoidance remains limited.
METHODS: This retrospective cohort study included 161 children referred for suspected non-immediate-type DHRs. Clinical characteristics, reaction features, laboratory findings, and DPT outcomes were collected from medical records. Re-exposure behavior and avoidance were assessed at 6 and 12 months.
