An ovine surgical model for peptide investigations is presented. Techniques for exteriorization of the carotid artery, catheterization of the sagittal sinus, hypophysectomy via a transnasopharyngeal approach, and ventricular and cisternal cannulation in the sheep are employed within the model. Several experimental applications in neurosurgical research are presented.

Diabetes insipidus is a clinical entity that is often seen in neurosurgical patients either during or immediately after transsphenoidal hypophysectomy. Rarely, diabetes insipidus can manifest as a new entity months later in patients who have previously had an intracranial injury or operation.

The level of blood eosinophils was studied in patients with adrenalectomy and/or hypophysectomy and in patients with asthma and eosinophilia. In the latter group was a patient who had asthma associated with hypopituitarism, which is cited in detail. Hepatic histidase activity in adult female rats is twice that in adult male rats. Hypophysectomy and thyroidectomy result in a significant increase in hepatic histidase activities in males, but not in females. This effect on histidase is reversed by the exogenous administration of tri-iodothyronine, but not by ectopic pituitary glands or purified pituitary hormones.

We describe a case of spontaneous muscle necrosis of the anterior tibial compartment occurring in a patient who had a hypophysectomy for a craniopharyngioma five years previously. We know of no other reported cases of spontaneous muscle necrosis in association with diabetes insipidus and feel that there should be increased awareness of the association between the two.

Pituitary thyrotropin excess resulting in hyperthyroidism has been previously reported in only 25 patients, of whom 19 had a pituitary tumour. This report describes a patient in whom a thyrotropin-producing pituitary tumour was associated with triiodothyronine thyrotoxicosis. Hypophysectomy was followed by a prompt fall in serum thyrotropin and a return to a euthyroid state.

A case of hypopituitarism and minimal sellar enlargement was found at hypophysectomy to have a giant cell granuloma of the pituitary. The clinical and histopathological features of this rare entity are reviewed. It is proposed that hypopituitarism which is out of proportion to minimal sellar enlargement may be a suggestive clue to the preoperative diagnosis of giant cell granulous which normally simulates a pituitary tumour.
