Lymphocytic hypophysitis in a 43-year-old woman.

A woman with sarcoidosis and primary hypothyroidism presented with partial hypopituitarism without pituitary gland enlargement. A clinical diagnosis of lymphocytic hypophysitis was established after exclusion of other possibilities, since a definitive diagnosis can only be made after histological studies. This rare form of chronic inflammation and destruction of the anterior pituitary gland is discussed.

Lymphocytic hypophysitis: unusual features of a rare disorder.

OBJECTIVE: Lymphocytic hypophysitis is a rare disorder which usually affects women and is often associated with pregnancy. We reviewed our experience of this disorder in order to see whether these features were universal amongst our patients. DESIGN: A retrospective review of case notes. PATIENTS: Four patients with histologically proven lymphocytic hypophysitis. MEASUREMENTS: Each patient had undergone full radiological and biochemical assessment of anterior and posterior pituitary function. RESULTS: Only one woman presented during pregnancy, one patient was a man with coexistent active a

Lymphocytic hypophysitis, pustulosis palmaris et plantaris and eosinophilia.

We describe here a unique case of lymphocytic hypophysitis accompanied by pustulosis palmaris et plantaris and eosinophilia. The patient also suffered from panhypopituitarism with hyperprolactinemia and pituitary diabetes insipidus caused by lymphocytic hypophysitis. Complications of pustulosis palmaris et plantaris and eosinophilia with lymphocytic hypophysitis have not been reported previously. In the present case, the activities of the three diseases correlated well throughout the patient's course, suggesting that a common mechanism might possibly participate in their pathogenesis.

Autoimmune Hypophysitis: A Cause of Central Diabetes Insipidus.

Diabetes insipidus (DI) is a rare disorder characterized by polyuria and polydipsia due to a deficiency of antidiuretic hormone (ADH) or insensitivity to ADH. This case report presents a rare cause of DI secondary to autoimmune hypophysitis, emphasizing the diagnostic and management challenges associated with the condition. The discussion highlights pathophysiology, clinical presentation, diagnostic approaches, and therapeutic strategies. The importance of considering autoimmune hypophysitis in the differential diagnosis of central DI is underscored, along with long-term implications of the di

Cystic Lymphocytic Hypophysitis Mimicking Pituitary Apoplexy: Daunting Challenge.

BACKGROUND: Inflammatory lesions of the pituitary gland including hypophysitis are poorly understood, and it is difficult to make a correct diagnosis on the basis of only clinical and radiologic findings. CASE DESCRIPTION: An otherwise healthy 68-year-old man presented with sudden onset of headache, visual impairment, and pituitary insufficiency. Magnetic resonance imaging revealed a pituitary mass with cystic component showing changing intensity on follow-up images. Also, changing density of the lesion was demonstrated on computed tomography over a short period of time. These findings sugges

Superior Pituitary Border Analysis in Immunotherapy-Induced Hypophysitis.

Immunotherapy-induced hypophysitis presents with headache, fatigue, and visual disturbances. The diagnosis is supported by imaging findings of pituitary swelling, enhancement, and hypermetabolism and established by low levels of pituitary hormones. A 64-year-old man with metastatic melanoma on nivolumab presented with a severe headache, initially attributed to sinus disease. Contrast CT was interpreted as minor sinus disease and no pituitary abnormality. Hypophysitis was eventually diagnosed and successfully treated based on PET and laboratory findings. Our retrospective analysis demonstrated

IgG4-Related Hypophysitis: Case Report and Literature Review.

IgG4-related hypophysitis is a rare, inflammatory process of the pituitary that mimics more commonly seen pituitary tumors. We report a case of IgG4-related hypophysitis in a 16-year-old female with diabetes insipidus who was found to have IgG4-related hypophysitis based on tissue diagnosis. This entity has not been previously described in a pediatric patient. Recognition of certain inflammatory processes of the pituitary may lead to alternative means of diagnosis and medical management without a biopsy.

Autoimmune polyendocrinopathy and hypophysitis after Puumala hantavirus infection.

UNLABELLED: Puumala hantavirus (PUUV) infection causes nephropathia epidemica (NE), a relatively mild form of haemorrhagic fever with renal syndrome (HFRS). Hypophyseal haemorrhage and hypopituitarism have been described in case reports on patients with acute NE. Chronic hypopituitarism diagnosed months or years after the acute illness has also been reported, without any signs of a haemorrhagic aetiology. The mechanisms leading to the late-onset hormonal defects remain unknown. Here, we present a case of NE-associated autoimmune polyendocrinopathy and hypopituitarism presumably due to autoimmu

[Antipituitary antibodies in patients with suspected autoimmune hypophysitis].

INTRODUCTION: Definitive diagnosis of autoimmune hypophysitis (AH) is histological. However, a presumptive diagnosis can be made through clinical, biochemical and imaging data. OBJECTIVE: The objective of this study was to review the presence of antipituitary antibodies (APA) and antithyroid antibodies (ATA) in patients with suspected AH in order to determinate the utility of APA in the diagnosis of AH. MATERIAL AND METHODS: We studied 36 patients divided into seven groups according to the data suggesting AH (isolated corticotropin deficiency, other idiopathic pituitary deficiencies, idiopat

Hypophysitis caused by Rathke's cleft cyst. Case report.

A 62-year-old woman presented with general malaise persisting for 2 months and narrowing of her visual field. T1-weighted magnetic resonance (MR) imaging showed swelling of the pituitary gland and stalk, and a homogeneous isointense intra- and suprasellar mass enhanced by gadolinium. During outpatient follow up, her condition deteriorated rapidly and she developed diabetes insipidus and panhypopituitarism. T1-weighted MR imaging demonstrated shrinkage of the lesion and disappearance of the central hypointense area indicating the cyst cavity, especially in the pituitary stalk. She underwent sur

[A case of lymphocytic hypophysitis mimicking pituitary macroadenoma].

Lymphocytic hypophysitis is a rare autoimmune disease of the pituitary gland mimicking pituitary macroadenoma on magnetic resonance imaging (MRI). We encountered a 32-year-old female who presented with a typical pituitary macroadenoma causing sudden on-set of visual disturbance during her second pregnancy. She underwent an endonasal-transsphenoidal resection of the mass diagnosed as a non-secreting pituitary macroadenoma after a cesarean operation. Intraoperatively, the lesion was found to be extremely tough and mucinous, and apparently differed from pituitary adenoma. The frozen section revea

Lymphocytic hypophysitis presenting with diabetes insipidus: MR findings.

It has been thought that neurohypophysial involvement manifesting as central diabetes insipidus in lymphocytic hypophysitis is rare. The radiological and clinicopathological features of two cases represent a variant of lymphocytic adenohypophysitis and/or lymphocytic infundibulo-neurohypophysitis are discussed.

Lymphocytic and granulomatous hypophysitis: experience with nine cases.

OBJECTIVE: Lymphocytic hypophysitis and granulomatous hypophysitis are rarely encountered. The aim of this study was to demonstrate their clinical peculiarities among pituitary disorders and to provide an approach for their clinical management. METHODS: In a retrospective study, we reviewed our surgical experience with nine patients harboring hypophysitis. The series included six cases of lymphocytic hypophysitis, two cases of granulomatous hypophysitis, and one case with evidence of coexisting lymphocytic and granulomatous hypophysitis. RESULTS: A striking similarity of clinical signs was f

Pregnancy in lymphocytic hypophysitis: case report and review.

Lymphocytic hypophysitis has reported in 22 cases, all but one in women. More than 80% of the cases in women occurred during pregnancy or in the postpartum period. We describe a girl with partial hypopituitarism secondary to lymphocytic hypophysitis who, five years after the diagnosis, successfully delivered a healthy infant at term. Of the previously reported cases, most patients remain amenorrheic after the diagnosis. From the present case report and review of the literature, it would appear that a diagnosis of lymphocytic hypophysitis does not necessarily preclude the patient's ability to c

Giant-cell granulomatous hypophysitis: a distinct clinicopathological entity.

Idiopathic giant-cell granulomatous hypophysitis is a chronic inflammatory disorder of the pituitary gland. It presents clinically as a sellar mass lesion with pituitary insufficiency and/or hyperprolactinemia, and is radiologically indistinguishable from a pituitary tumor. In most of the previously reported cases the documentation of the disorder has been limited to autopsy tissue. Only a few cases documented by biopsy have been recorded. Four cases are presented here with radiological, endocrinological, and surgical findings. The appropriately documented cases collected from the literature a

Postpartum Hypophysitis: A Case Report and a Literature Review.

Hypophysitis is a rare pituitary gland disease primarily seen in females of reproductive age. Patients can present with various non-specific symptoms, which makes diagnosis challenging. Appropriate endocrine workup supplemented with magnetic resonance imaging (MRI) helps establish a diagnosis. We present a case of a 22-year-old postpartum female who came with nausea, vomiting, and abdominal pain. Global endocrine insufficiency was seen in the laboratory workup, and an MRI confirmed the diagnosis of hypophysitis. She was treated with steroid and thyroid hormone supplementation.

Pregnancy in Autoimmune Hypophysitis: Management of a Rare Condition.

INTRODUCTION: Hypophysitis is an inflammatory disorder of the pituitary gland. It can manifest variously, with endocrinological and neuro-ophthalmologic symptoms and signs, due to the compression of sellar and parasellar structures. CASE REPRESENTATION: Although hypophysitis is rare, this pituitary disease can occur during pregnancy or in the postpartum period. In this report, we describe the case of a woman with partial hypopituitarism secondary to autoimmune hypophysitis who, five years after the diagnosis and the immunosuppressive treatment, had an uneventful pregnancy and successfully del

Case Report: IgG4-Related Disease Presenting With Isolated Hypophysitis.

BACKGROUND/OBJECTIVE: IgG4-related disease (IgG4-RD) is an immune-mediated condition that affects multiple organs, including the pituitary gland. Here we present a patient with isolated pituitary involvement of IgG4-RD mimicking pituitary apoplexy. CASE REPORT: A 49-year-old woman presented to the emergency department with abdominal pain, nausea, vomiting, and weight loss. Her blood pressure was low, and she appeared euvolemic with the rest of physical examination being noncontributory. Her electrolytes showed low serum sodium of 118 mmol/L (normal 135-145).
