Surgical syndromes of the hypothalamus.

The clinical syndromes described with lesions of the hypothalamus are summarized in Table 9.5-9.7. The anterior hypothalamic syndrome consists of insomnia and loss of thirst regulatory mechanisms. In occasional larger lesions which interrupt the output from the supraoptic and paraventricular nuclei, diabetes insipidus has been noticed. In the tuberal region of the hypothalamus the most prominent findings are those that are caused by the disruption of the final common pathway to the pituitary. This results in endocrinopathy, most often the syndrome originally reported by Frohlich, with failure

Treatment of true precocious puberty with a potent luteinizing hormone-releasing factor agonist: effect on growth, sexual maturation, pelvic sonography, and the hypothalamic-pituitary-gonadal axis.

We used the LHRH agonist D-Trp6-Pro6-N-ethylamide LHRH (LHRH-A) to treat 19 children (12 girls and 7 boys) with true precocious puberty. Fourteen patients had idiopathic true precocious puberty, 4 had a hamartoma of the tuber cinereum, and 1 had a hypothalamic astrocytoma. Basal gonadotropin secretion and responses to native LHRH decreased within 1 week of initiation LHRH-A therapy, and sex steroid secretion decreased within 2 weeks to or within the prepubertal range. Ultrasonographic evaluation of the uterus indicated a postmenarchal size and shape in all 11 girls studied before treatment, wh

Hormonal and metabolic abnormalities associated with central nervous system germinoma in children and adolescents and the effect of therapy: report of 10 patients.

We describe the results of clinical and endocrinological investigations performed on 10 children and adolescents (5 males and 5 females) with a primary central nervous system germinoma. Eight of 10 patients were between 10-20 yr of age at the time of initial presentation. Polyuria (7 of 10) and a decrease in or cessation of linear growth (5 of 10) were the most common presenting symptoms, while only 2 of 10 patients complained of visual problems. Two patients presented with the syndrome of polyuria, adipsia, hypernatremia, profound muscle weakness, and hyperlipidemia. Initial physical exam rev

Anaplastic evolution of childhood optico-hypothalamic pilocytic astrocytoma: report of an autopsy case.

A 19-month-old boy was found to have an optico-hypothalamic tumor with obstructive hydrocephalus, and received a ventriculoperitoneal (V-P) shunt followed by irradiation to the hypothalamic area. Although his visual disturbances continued, general conditions improved subsequently and he enjoyed normal life with only a visual disturbance. Four years later, however, he developed vomiting, left hemiparesis and dysuria, and died at the age of 6.5 years. Autopsy revealed a huge tumor occupying the hypothalamus, thalamus, basal ganglia and the right temporal lobe. Histologically, the tumor was compo

Clinical aspects of neuroendocrinology.

Recent studies have identified structures of most of the hypothalamic hormones and clarified their actions. In addition, the importance of a variety of classical and peptide neurotransmitters or neuromodulators in the regulation of pituitary hormone secretion and other hypothalamic functions has been partly elucidated. These advances in the physiological aspects of the hypothalamus have greatly helped our understanding of the pathogenesis of a variety of hypothalamic syndromes.

[Dexamethasone for Perioperative Analgesia].

BACKGROUND: Dexamethasone is a highly potent, long-acting synthetic glucocorticoid that does not exhibit any mineralocorticoid activity. It is commonly used during surgery to prevent postoperative nausea and vomiting (PONV), but also due to its perceived ability to reduce postoperative pain and the associated consumption of opioids. OBJECTIVE: To summarise the fundamentals relating to the substance, the clinically relevant forms of administration, the common dosages, the contraindications, the adverse effects and the guideline recommendations. RESULTS: Parenteral preparations typically conta

Safety of growth hormone (GH) treatment in GH deficient children and adults treated for cancer and non-malignant intracranial tumors-a review of research and clinical practice.

Individuals surviving cancer and brain tumors may experience growth hormone (GH) deficiency as a result of tumor growth, surgical resection and/or radiotherapy involving the hypothalamic-pituitary region. Given the pro-mitogenic and anti-apoptotic properties of GH and insulin-like growth factor-I, the safety of GH replacement in this population has raised hypothetical safety concerns that have been debated for decades. Data from multicenter studies with extended follow-up have generally not found significant associations between GH replacement and cancer recurrence or mortality from cancer amo

[Clinical course of brain tumors in childhood].

Brain tumors are, after leukemias, the most frequent fatal neoplasms of infancy. The clinical features and symptoms are often markedly different from those observed in the adult forms, according to the peculiar anatomy and behaviour of the child. Persistent headache, vomiting, astenia , behavioral alterations may be the precocious findings. Later, some more specific and suggestive signs such as strabismus, dyplopia , fast head size increase, funduscopic alterations, ataxia, paresis and nystagmus may be observed. On their appearance a prompt diagnostic work-up should be performed. The tumors of

Case Report: First description of an intracranial "NET" in ROHHAD syndrome; time to rename to the ROHHAD-NeCT syndrome?

In around 50% of patients with Rapid-onset Obesity Hypothalamic dysfunction, Hypoventilation and Autonomic Dysregulation (ROHHAD) syndrome, a neuro-endocrine tumor (NET) is found. Reported NETS are (ganglio)neuroblastoma or ganglioneuroma located extracranially. Here, we report for the first time a young patient with signs and symptoms fitting the criteria of ROHHAD syndrome in whom an intracranial ganglion cell tumor was found, pathologically resembling the reported extracranial NETs. Our finding leads to the discussion of whether the NET in ROHHAD-NET syndrome should be classified as true NE

Neurofibromatosis type 1: A neuro-psycho-cutaneous syndrome?

Neurofibromatosis type 1 (NF1) is an inherited disorder often associated with optic nerve gliomas, low-grade brain tumors, and readily visible signs. Though these features are frequently emphasized, the psychosocial and emotional morbidities are often overlooked. We present a patient with depressive disorder resulting in suicide in a patient with NF1.

[Metastasis from breast cancer to the pituitary gland causing hyponatriaemia].

Breast cancer is the most common cancer in Danish women. Pituitary metastases are rare events. We report a 75-year-old woman with metastatic breast cancer who presented with pituitary insufficiency nine years after her initial cancer diagnosis. MRI of the pituitary region showed a parasellar mass and thickening of the infundibulum. Her condition progressed within months and deterioration of visual field and acuity developed. A transsphenoidal resection was performed to rescue her vision and pathology confirmed metastasis from a breast invasive ductal adenocarcinoma.

[Disorders of viscero-autonomic and neurohumoral regulation in tumors of sellar-diencephalic localization].

Twenty-nine patients with varying degrees of hypothalamic lesion were studied for disorders of viscero-vegetative and neurohumoral regulation. Three types of impairments were identified: intact under-compensated, reduced, and perverted. The type of disorders was found to correlate with the degree of the impact of the pathologic process on the hypothalamic structures. It is emphasized that preoperative examination of the central hemodynamics and biogenic amines allows the prediction of their impairment in the postoperative period.

Acquired spontaneous periodic hypothermia and pancytopenia secondary to hypothalamic compression from a large skull base meningioma.

A 69-year-old woman with a recurrent skull-base meningioma developed recurrent hypothermia during rehabilitation, initially misattributed to infection. Episodes followed a circadian pattern and were associated with delirium, pancytopenia, and signs of central endocrine dysfunction. MRI showed a large meningioma compressing the hypothalamus with an intact corpus callosum. Infection and adrenal insufficiency were excluded. Hypothermia resolved with external warming, and pancytopenia normalized spontaneously. Subclinical hypothyroidism and central hypogonadism were noted, consistent with hypothal

Characteristics and factors influencing hypothalamic pituitary dysfunction in patients with craniopharyngioma.

BACKGROUND: Craniopharyngioma is a benign tumor originating from the sellar region. Damages in this area caused by the tumor itself, surgery, or radiotherapy may result in severe hypothalamic-pituitary dysfunction (HPD) and eventually lead to a significant impairment in the long-term quality of life of patients. This study aimed to investigate the characteristics of HPD in patients with adamantinomatous craniopharyngioma (ACP) or papillary craniopharyngioma (PCP) and to identify the factors affecting HPD after surgery. METHODS: In this single-center retrospective study, a total of 742 patient

Extended Transsphenoidal Approach for Gross Total Resection of Hypothalamic Tumor.

Tumors of the optic pathway and/or hypothalamus are uncommon, and the majority occur in patients with neurofibromatosis type 1.1,2 These lesions can be managed with a "watch-and-wait" approach; however, when treatment is indicated, the optimal strategy is wholly case dependent. We present a 22-year-old female patient with neurofibromatosis type 1 who had a partially cystic mass within the right mamillary body (Video 1). The mass was present on prior imaging but had increased in size and was newly enhancing with evidence of mass effect on the right optic tract. Given her history and the lesion'

Hypothalamic dysfunction in a patient with primary lymphoma of the central nervous system.

A 60-year-old woman with no previous history of chronic disease or malignancy presented with intense back and left leg pain and sleep disturbances. The patient had been treated unsuccessfully for the past 6 months with analgetics. Magnetic resonance imaging showed a soft tissue tumor in the L5-S1 region that involved the spinal canal, and a pathohistological analysis of the tumor specimen confirmed the presence of non-Hodgkin, diffuse large B cell lymphoma. After the diagnosis was confirmed, malaise, nausea, and vomiting developed. Multislice computed tomography of the endocranium showed focal
