In a 55-year-old woman with intestinal obstruction and intussusception, an excised segment of distal ileum was shown by light and electron microscopy and immunohistologically to contain an inflammatory fibroid polyp. This rare, usually solid lesion of the gastrointestinal canal should be included in differential diagnosis of small-bowel obstruction and intussusception.

A case of replacement of a damaged ureter with ileum, in a previously constructed transverse colon conduits, is reported. This is an attractive surgical alternative that deserves a place as a reconstructive procedure in the management of ureteral complications following urinary diversion.

A 47 year old man had a ten year history of intestinal bleeding due to arteriovenous malformation of the distal ileum. Selective angiography is the most reliable technique for the diagnosis of the lesion and a correct diagnosis was not made until th selective superior mesenteric arteriography was done after repetitive laparotomy and intestinal resection. Transcatheter embolization was transiently successful. This case seemed to belong to type 2 according to Moore's classification and was probably of congenital origin.

The results are described of a retrospective study of 142 patients with atresias of the jejunum, ileum and colon in the Netherlands. It is pointed out that pregnancy had often been complicated by polyhydramnios. This may lead to an early diagnosis in the prenatal period. Whenever a neonate presents with bilious vomiting or distension of the abdomen, closer examination for intestinal atresia should take place. The production of meconium does not rule out the diagnosis of intestinal atresia.

The effect of ethacrynic acid on the motor function of guinea pig ileum was studied in vitro. Ethacrynic acid produced dose-related (5-160 microgram/ml) contractions in this tissue. Morphine, tetrodotoxin and sodium-free medium prevented the contractions while hexamethonium, diphenhydramine, methysergide or indomethacin did not. Atropine in a high concentration (0.1 microgram/ml) only inhibited the contractions. Ethacrynic acid inhibited the contraction of ileum induced by electrical stimulation of intramural nerves. Three cases of premature infants with obstruction of the ileum and proximal colon in the absence of cystic fibrosis are presented. Meconium inspissation complicated low-grade bowel obstruction in 2 infants. In the third, the bowel was anatomically normal, with the tenacious meconium the apparent primary cause of obstruction. Distinguishing features of this unusual type of blockage and other meconium syndromes are also discussed. This condition may simulate necrotizing enterocolitis, requiring surgery for differentiation. One theory of compensatory hypertrophy of the gut suggests that ileal growth after jejunectomy results from exposure of the ileum to villus-enlarging factors emanating from the papilla of Vater. This hypothesis was tested by examination of the ileum after bypass of pancreatobiliary secretions to the colon. Hypertrophy of the ileum occurred after esophagoileostomy and gastroduodenojejunal exclusion, suggesting that adaptive growth of the gut depends on additional factors.
