PURPOSE OF REVIEW: Biliary tract cancer (BTC) is a heterogeneous group of malignancies with limited systemic treatment options and poor prognosis. While immunochemotherapy has become the first-line standard, second-line therapy remains inadequate. Antibody-drug conjugates (ADCs) have emerged as a promising therapeutic modality leveraging antigen-specific targeting to deliver potent cytotoxic payloads. This review provides a comprehensive, multi-target appraisal of the ADC landscape in BTC.
RECENT FINDINGS: Trastuzumab deruxtecan (T-DXd), supported by DESTINY-PanTumor02 (ORR 22%) and HERB (ORR

Acute myeloid leukemia (AML) is a hematologic malignancy with a poor prognosis and high relapse rates, especially in high-risk patients and older adults. Conventional treatment modalities confer limited benefit, specifically in relapsed and refractory cases. Antibody drug conjugates (ADCs) are a rapidly advancing treatment option that provides a novel approach to treating AML. The design and mechanistic aspects of ADCs have also been discussed. ADCs combine cytotoxic chemotherapeutic drugs with the specificity of monoclonal antibodies. This review primarily focuses on the current role of ADCs

Antibody-drug conjugates (ADCs) are transforming targeted cancer therapy, yet conventional IgG-based formats are often limited by stochastic conjugation, yielding heterogeneous ADCs. Dual-variable domain IgG1 (DVD-IgG1) antibodies offer a modular solution by enabling precise, site-specific conjugation through the catalytic lysine and engineered arginine or cysteine residues, affording homogeneous ADCs. This review summarizes recent advances in DVD-IgG1-based ADCs, highlighting site-specific conjugation, compatibility with diverse linker chemistries, and incorporation of both established and ex

Drug-to-antibody ratio (DAR), free-drug-related impurities (FDRI) content, and purity are critical quality attributes of antibody-drug conjugates (ADCs), which substantially impact product safety and efficacy. However, growing efforts in developing ADCs with higher complexity in a faster timeline impose a great challenge to their analytical support. Herein, for the ADC carries an antibody and linker-payload with distinct UV/vis absorption maxima, we propose a high-throughput and multiattribute ADC analysis strategy based on size exclusion chromatography coupled with UV detection (SEC-UV). In recent years, significant progress has been made in the structural optimization and mechanistic research of antibody-drug conjugates (ADC), with multiple clinical trials for prostate cancer underway and demonstrating broad clinical application prospects. Compared with traditional chemotherapy and immunotherapy, ADC combines the targeting specificity of monoclonal antibodies with the potent tumor-killing ability of chemotherapeutic drugs, offering potential therapeutic options for prostate cancer patients. Currently, four prostate-specific membrane antigen-targeted ADC have entered the clini

Classic Hodgkin lymphoma (cHL) is highly curable at all stages. Research efforts over the past few decades have largely focused on interim PET-adapted strategies for therapy de-escalation or intensification. The overarching goals have been to increase cure rates, minimize potential therapy-related effects, and optimize survivorship. Better understanding of the biology of cHL has led to the development and approval of effective novel agents including the antibody-drug conjugate brentuximab vedotin and the checkpoint inhibitor immunotherapies. In this review, we discuss recent trial results and

Relapsed or refractory lymphoma is commonly treated with combination chemoimmunotherapy and cellular immunotherapy. Modest response rates and associated toxicities are obstacles to achieving durable remission using traditional cytotoxic chemotherapy, especially in frail patients with advanced disease. Antibody drug conjugates represent a new class of novel targeted agents with significant improvement in therapeutic efficacy in the treatment of lymphomas. Several of these agents, which offer improved targeting, greater potency, and better therapeutic index over traditional chemotherapy, are cha

Monoclonal antibody-drug conjugates (ADCs) are an expanding therapeutic class of biomolecules for which relatively few analytical and preparative separation options exist. Purification of ADCs with a specific drug antibody ratio is even more challenging. We report the first application of countercurrent separation (CCS) to this problem. An ADC mimic was successfully chromatographed using an aqueous two-phase system (ATPS) consisting of PEG 1000/sodium citrate pH 7.5/water, 17.75/17.75/64.50 (w/w/w). Notably, different partition coefficients (K) in this ATPS for the ADC mimic (0.09 < K < 0.16)

Antibodies and their derivatives (scFv, Fabs, etc.) represent a unique class of biomolecules that combine selectivity with the ability to target drug delivery. Currently, one of the most promising endeavors in this field is the development of molecular diagnostic tools and antibody-based therapeutic agents, including antibody-drug conjugates (ADCs). To meet this challenge, it is imperative to advance methods for modifying antibodies. A particularly promising strategy involves the introduction of carbonyl groups into the antibody that are amenable to further modification by biorthogonal reactio

PURPOSE OF REVIEW: Therapies that target the immune system are increasingly used across oncology, including in hematologic malignancies such as myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML). While allogeneic transplant has been a key therapy in these cancers, new approaches that target the immune system are being explored including immune checkpoint therapies, antibody-drug conjugates, and cellular therapies.
RECENT FINDINGS: This review outlines updates in the preclinical rationale for immune directed therapies in MDS and AML, as well as recent clinical trials exploring th

Pretargeted radioimmunodiagnosis and radioimmunotherapy aim to efficiently combine antitumor antibodies and medicinal radioisotopes for high-contrast imaging and high-therapeutic-index (TI) tumor targeting, respectively. As opposed to conventional radioimmunoconjugates, pretargeted approaches separate the tumor-targeting step from the payload step, thereby amplifying tumor uptake while reducing normal-tissue exposure. Alongside contrast and TI, critical parameters include antibody immunogenicity and specificity, availability of radioisotopes, and ease of use in the clinic. Each of the steps ca

We studied the effect of Methylpredisolone (MP) on ischemic brain edema after temporary occlusion of the middle cerebral artery for 90 min (MCAo90). We verified the presence of ischemic edema by determining the brain water content (BWC) by measuring dry/wet weight and by examining MRI - T2-weighted imaging, T2 relaxation times and apparent diffusion coefficient (ADC). In another group, animals were administered MP intraperitoneally 30 min after MCAo90, followed by 24 h reperfusion (MCAoMP). Edema changes were documented by the same MRI examinations. A statistically significant increase in BWC

Trastuzumab deruxtecan (T-DXd; DS-8201) is an antibody-drug conjugate targeting human epidermal growth factor receptor 2. Interstitial lung disease (ILD)/pneumonitis is an adverse event associated with T-DXd; in most cases, it is low grade (grade ≤ 2) and can be treated effectively but may develop to be fatal in some instances. It is important to increase patient and provider understanding of T-DXd-related ILD/pneumonitis to improve patient outcomes. Drug-related ILD/pneumonitis is a diagnosis of exclusion; other possible causes of lung injury/imaging findings must be ruled out for an accurate

Generating effective RNAi-based therapies with the potential to achieve leukemia remission remains critical unmet need. Despite a growing number of leukemia clinical trials, tissue specific delivery of therapeutic siRNA is a major roadblock in translating its clinical potential. The most recent reports in the antibody-siRNA-conjugates (ARCs) field add new dimensions to leukemic therapy, where a covalently ligated therapeutic antisense-RNA with the potential to repress the oncogenic transcript is selectively delivered into the cancer cells. Despite ARC localization to leukemic cells due to high

Hodgkin lymphoma (HL) is a B cell-derived lymphoid malignancy most often affecting young adults. More than 80% of HL patients achieve long-term remission after appropriate first-line treatment consisting of multiagent chemotherapy and/or radiotherapy (RT). In addition, approximately 50% of patients with disease recurrence remain relapse-free after salvage therapy with high-dose chemotherapy followed by autologous stem cell transplantation (ASCT). However, patients with multiple relapses are mostly in a palliative situation, and novel drugs for this patient group are needed. Furthermore, novel
