Types of hymen in the newborn infant.

The incidence of various types of hymenal shape, its orifice and their interference with spontaneous vaginal discharge were investigated during the routine physical examination of 333 female infants within the first 24 h of life. A smooth hymen with a central orifice was observed in 53.5% of the female neonates, a folded hymen with a central orifice in 27.3%; folded hymen with eccentric orifice in 4.5%; an anterior opening of the hymen in 10.8%; posterior opening in 0.6%; hymenal band in 3%; almost imperforate hymen in 0.3% of the newborns. The presence of anterior opening, posterior opening,

[Transitory diabetes mellitus in the newborn infant].

A female newborn of 1,870 g with low Apgar scores, after septicemia, shock and blood exchange transfusion, developed a transient diabetes mellitus on the seventh day after birth. The disease was kept well under control by insulin, given at first intravenously and later subcutaneously. The amount of insulin required decreased continuously. Even when the blood glucose level was high, there was no coma or ketoacidosis.

Urinary catecholamine levels in the newborn infant.

The urinary concentrations of dopamine, noradrenaline and adrenaline were measured by a radioenzymatic method in 212 full-term and premature newborns. The ranges, means and standard deviations from birth to 4 days + are presented. The excretion of dopamine was ten times that of noradrenaline or adrenaline. The absolute concentrations of each catecholamine were reduced as birth weight decreased. The values were increased in babies with fetal distress. Any changes found in hypoglycaemic or jaundiced infants were attributable to prematurity. Very high levels were found in a few infants given tola

Transient myocardial ischemia of the newborn infant.

Ten full-term newborn infants with transient myocardial ischemia developed radiographic evidence of cardiomegaly and pulmonary venous congestion within the first few hours of life. In five patients, radiographic findings returned to normal during the first week of life. In the remaining five patients, radiographic findings improved within the second week and eventually became normal. Echocardiography done on six patients demonstrated left ventricular dysfunction in all six patients. Those infants with documented perinatal insults tend to have a protracted clinical course with eventual recovery

The retina of the newborn human infant.

We have examined a pair of eyes from a normal, full-term infant who died at 8 days as a result of accidental injury. Eyes were obtained immediately after death, fixed, and sectioned for light microscopy. Results from both eyes were substantially the same. The macular region was still drastically immature at 1 week. Even though a foveal depression existed, all cell layers were still present across it. Furthermore, the inner nuclear layer was divided into two separate layers. The receptor layer was reduced to one or two cells thick; receptors had both inner and outer segments, but they were very

[Polycythemia in the newborn infant (author's transl)].

Respiratory distress syndrome and cyanonis in newborns (first neonatal week) may be caused by polycythemia. During 14 months twentyone newborns with symptoms of cardiorespiratory or neurological distress caused by polycythemia on the neonatological division of our hospital were observed. The values of hemoglobin ranged between 22.7 and 28.6 g% (mean 25.65 g%), the values of hematocrit ranged between 72 and 90% (mean 81%). The central venous hematocrit measured on 14 patients ranged between 65 and 77% (mean 71%). On 20 newborns isovolemic haemodilution by 5% plasma was performed. The volume of

Persistent pulmonary hypertension of the newborn infant.

Persistent pulmonary hypertension of the newborn infant can be difficult to distinguish from other cardiopulmonary causes of cyanosis during the newborn period. Infants with PPHN have cyanosis, tachypnea, acidemia, normal pulmonary parenchymal markings on the chest radiography, and anatomically normal hearts. We have identified and treated 11 infants and have noted several signs and symptoms not previously emphasized. These are cineangiocardiographic evidence of atrioventricular valve insufficiency in association with systolic murmurs and slow ventricular emptying, apnea, hypocalcemia, only a

Spinal cord injury in the newborn infant.

Birth injury to the spinal cord secondary to traction forces during delivery is a common but frequently undiagnosed disorder. The injury usually affects the cervicodorsal junction, with both extradural hematoma and direct cord damage at that level. Clinical findings of a paraplegic infant with abdominal breathing are sometimes obscured by secondary pneumonia and/or hypoxia. Radiologic manifestations include a bell shaped chest indicative of loss of the external muscles of respiration; spinal roentgenograms are usually normal. Myelography in neonatal spinal injury demonstrates a block in the su
