Parents' Views on Family Care at a Neonatal Intensive Care Unit - A Qualitative Study.

PURPOSE: The current study aims to explore parents' views on family care at a neonatal intensive care unit. METHODS: We conducted a qualitative exploratory study with a descriptive approach based on open-ended written narrative responses from the Danish version of EMpowerment of PArents in The Intensive Care-Neonatology questionnaire. The study was conducted at a Danish level IV NICU working with family-centred care and family integrated care. The data was analysed using inductive content analysis by Graneheim and Lundman. The Standards for Reporting Qualitative Research checklist was followe

Rapid Point-of-Care Genotyping to Avoid Aminoglycoside-Induced Ototoxicity in Neonatal Intensive Care.

IMPORTANCE: Aminoglycosides are commonly prescribed antibiotics used for the treatment of neonatal sepsis. The MT-RNR1 m.1555A>G variant predisposes to profound aminoglycoside-induced ototoxicity (AIO). Current genotyping approaches take several days, which is unfeasible in acute settings. OBJECTIVE: To develop a rapid point-of-care test (POCT) for the m.1555A>G variant before implementation of this technology in the acute neonatal setting to guide antibiotic prescribing and avoid AIO. DESIGN, SETTING, AND PARTICIPANTS: This pragmatic prospective implementation trial recruited neonates admit

Implementing Screening for Neonatal Delirium in the Neonatal Intensive Care Unit: A Quality Improvement Initiative.

INTRODUCTION: Delirium is not commonly diagnosed in neonatal intensive care units and can adversely impact patient outcomes in the ICU setting. Recognition of delirium in the NICU is a necessary first step to address the potential impact on neonatal outcomes. METHODS: We conducted a quality improvement initiative implementing screening for neonatal delirium. We aimed to increase screening in NICU patients from 0% to 85% by March 2022. Interdisciplinary meetings were held with key stakeholders to develop a clinical algorithm. We used standardized tools for delirium screening. Our process measu

Empiric antibiotic regimens for neonatal sepsis in Australian and New Zealand neonatal intensive care units.

AIM: Neonatal sepsis remains an important cause of morbidity and mortality, and requires prompt empiric treatment. However, only a minority of babies who receive antibiotics for suspected sepsis have an infection. Antimicrobial exposure in infancy has important short- and long-term consequences. There is no consensus regarding empirical antimicrobial regimens. METHODS: The study included a survey of empiric antimicrobial regimens in all tertiary neonatal intensive care units in Australia and New Zealand in 2013-2014. RESULTS: All 27 units responded. For early-onset sepsis, all units used a c

Disability in the neonatal intensive care unit: are current frameworks applicable?

Decisions for patients in the neonatal intensive care unit (NICU) are made under the auspices of the shared decision-making model, which uses the best interests standard as a guide. Decisions made regarding the withdrawal of life-sustaining measures (WLSM) are also made using the shared decision-making model with attention to either physiological parameters indicative of survival or the potential for disability. The two dominant frameworks for considering disability are the medical and social models. At its core, the medical model views disability as something broken within the person, somethi

Analgosedation in Neonatal Intensive Care: Current Strategies, Challenges, and Future Perspectives.

Pain and stress are frequent and clinically relevant challenges in neonatal intensive care, particularly among preterm and critically ill newborns exposed to repeated invasive procedures, mechanical ventilation, surgery, and advanced life-support interventions. Effective analgosedation is essential to reduce discomfort, attenuate physiological instability, improve tolerance of intensive care procedures, and potentially limit adverse neurodevelopmental consequences. However, neonatal pain and analgosedation management remain complex because of developmental immaturity, pharmacokinetic and pharm

Respiratory Physiotherapy Techniques in Neonatal Intensive Care Units: A Scoping Review.

BACKGROUND AND PURPOSE: Certain physiological characteristics increase the susceptibility of newborns, particularly preterm infants, to pulmonary collapse. In addition, newborns are prone to the accumulation of airway secretions, which, even in small volumes, can compromise respiratory function. Consequently, breathing therapy plays a crucial role in promoting airway clearance and supporting effective ventilation in this population. This scoping review aimed to map the breathing therapy methods used for newborns admitted to intensive care units and also to identify the clinical outcomes assess

End-of-life decision-making in the neonatal intensive care unit.

Most paediatric deaths occur in the neonatal period, many of them in neonatal intensive care units after withdrawal of life support or the decision not to initiate new treatments. In these circumstances, discussions with families and decision-making are fundamental elements of the care and attention given to newborn babies. In this context, bioethical deliberation can help us to identify the values at stake, the different courses of action to be taken, and the means to ensure that family-shared decision-making is appropriate to the patient's situation and in accordance with the family's values

Reducing Intraoperative Hypothermia in Infants from the Neonatal Intensive Care Unit.

UNLABELLED: Infants from the neonatal intensive care unit (NICU) undergoing surgery in the operating room (OR) are at greater risk for hypothermia during surgery than afterward due to environmental heat loss, anesthesia, and inconsistent temperature monitoring. A multidisciplinary team aimed to reduce hypothermia (<36.1 °C) for infants at a level IV NICU at the beginning of the operation (first OR temperature) or at any time during the operation (lowest OR temperature) by 25%. METHODS: The team followed preoperative, intraoperative (first, lowest, and last OR), and postoperative temperatures.
