Autoimmune enteric leiomyositis is an extraordinary rare cause of acquired chronic intestinal pseudo-obstruction in children. We report a 5-year-old girl who developed chronic intestinal pseudo-obstruction 3 years after an autoimmune hepatitis. Mucosal biopsies of the upper gastrointestinal tract and colon showed minimal inflammatory changes. On full-thickness biopsies of the small intestine, a dense lymphocytic infiltrate of the muscularis propria was seen, mainly consisting of cytotoxic T lymphocytes. Intestinal pseudo-obstruction has been associated with several types of underlying neoplasms and has been hypothesized to result from a paraneoplastic process in patients with small cell lung carcinoma and bronchial carcinoid. This article documents the first reported association of gastroparesis and a retroperitoneal leiomyosarcoma. A paraneoplastic process is the proposed basis of this relationship, inasmuch as these syndromes have been noted in other tumors of mesodermal origin. The resolution of intestinal dysmotility after tumor resection, as described here, has only rarely been reported.

Bowel obstruction and nutrient deficiencies are commonly encountered complications seen during follow-up of patients with short bowel syndrome. It is rare to see vitamin B12 deficiency among micronutrient deficits. The onset of B12 deficiency can be insidious, with autonomic features preceding other symptoms. In this study, we report a case in which vitamin B12 deficiency presented with intestinal dysmotility mimicking mechanical bowel obstruction.

Arsenic trioxide (As(2)O(3)) is an effective agent for the treatment of relapsed acute promyelocytic leukemia (APL).
