Ketamine: A Cause of Urinary Tract Dysfunction.

Drug addiction as a result of improper use of prescribed and illicit use has been on the increase globally. The effects of such use have implications in the urologic disease space. To this end, Ketamine has been reported to affect urologic function, causing a number of voiding symptoms. It may also confound the differential diagnosis of urologic diseases, such as interstitial cystitis, among others.

[Ketamine as anesthetic agent in electroconvulsion therapy].

BACKGROUND: Electroconvulsive therapy (ECT) is a well-established, safe and effective treatment for severe psychiatric disorders. Ketamine is known as a core medication in anesthesiology and has recently gained interest in ECT practice as there are three potential advantages: (1) ketamine has no anticonvulsive actions, (2) according to recent studies ketamine could possess a unique intrinsic antidepressive potential and (3) ketamine may exhibit neuroprotective properties, which again might reduce the risk of cognitive side effects associated with ECT. OBJECTIVES: The use of ketamine in psychi…

Mechanism of action of dextromethorphan/quinidine:comparison with ketamine.

Reports of rapid-onset but short-duration antidepressant effects in patients with treatment-resistant mood disorders after intravenous administration of ketamine have prompted efforts to find an agent with ketamine's properties that can be administered orally in repeated doses in order to sustain that action. One candidate for this dextromethorphan, and here the pharmacologic mechanism of action is compared and contrasted with that of ketamine.

Ketamine for palliative sedation in the emergency department.

BACKGROUND: Palliative sedation to treat severely distressing symptoms in those with a poor prognosis is well-accepted. OBJECTIVE: We discuss palliative sedation in the Emergency Department and the use of ketamine. CASE REPORT: We present the case of a patient with angioedema of the tongue and severe respiratory distress. The patient's nursing home was unable to control her symptoms and she was transferred to the Emergency Department. The patient received fentanyl 50 μg i.v. and ketamine 50 mg i.v. every 5 min until adequate palliative sedation was achieved. CONCLUSION: Ketamine can be cons…

[Advances in research of ketamine addiction mechanism].

Ketamine is a phencyclidine derivative acting primarily as a noncompetitive antagonist of N-methyl-D-aspartate (NMDA) excitatory glutamate receptors. As a common intravenous anaesthetic in clinic, it is also increasingly abused because of its hallucination and addiction effects. Based on the pharmacological and toxicologic characteristics of ketamine and the acknowledged addiction mechanism of other abused drugs, this article reviews the possible addiction mechanism of the ketamine in the aspects of its enhanced effects and reward systems, the anatomic structures, the related receptors and the…

In vitro investigation of the antibacterial effect of ketamine.

BACKGROUND: Antibacterial activity of local anesthetics especially lidocaine has been shown previously. In this study, the antibacterial effect of ketamine, a general anesthetic agent was investigated. METHODS: The antibacterial effect of ketamine was studied using six different strains of bacteria (Staphylococcus aureus, Staphylococcus epidermidis, Entecoccus faecalis, Streptococcus pyogenes, Pseudomonas aeruginosa and Escherichia coli) with disc diffusion method. Ciprofloxacine discs (CIP, oxoid) were used as a control to verify the methodology. Minimal inhibition concentration (MIC) and mi…

Effects of ketamine on precipitated opiate withdrawal.

OBJECTIVE: N-methyl-D-aspartate antagonists were shown to be effective in suppressing the symptoms of opiate withdrawal. Intravenous anesthetic, ketamine, is the most potent N-methyl-D-aspartate antagonist available in clinical practice. The present study was designed to evaluate the effects of subanesthetic ketamine infusion, as little human data are available on ketamine in precipitated opiate withdrawal. MATERIALS AND METHODS: A total of 58 opiate-dependent patients were enrolled in a randomized, placebo-controlled, double-blind study. Patients underwent rapid opiate antagonist induction u…

[Treatment of intraoperative status asthmaticus with ketamine].

A case report of an intraoperative asthmatic state that was untreatable by conservative methods is described. These methods include elevation of the patient's chest, ventilation with pure oxygen, and administration of volatile anaesthetics, bronchodilative drugs and corticoids. If the patient does not respond to conventional therapy, the bronchial smooth muscle relaxant effect of ketamine should be used.

[20 years of ketamine--a backward look].

Twenty years ago, ketamine was introduced into clinical use in Germany. Since this time, advantages and disadvantages of "dissociative anesthesia" with ketamine have been reported in many studies. Improvements were seen in stabilization of blood pressure during induction of shock patients, induction of patients with asthma, and preservation of spontaneous breathing as well as protective reflexes. On the other hand, psychomimetic reactions and an increase in cardiac oxygen consumption restricted the clinical use of ketamine. The incidence of these side effects was reduced when ketamine was comb…

Local myotoxicity of ketamine hydrochloride in the marmoset.

An investigation of raised plasma aspartate aminotransferase (AST) in marmosets after intramuscular ketamine injection suggested a local myotoxicity. This was confirmed by a range of histopathological findings from myofibrillar striation loss to necrosis. In addition to the elevations in AST levels, creatine kinase and the lactate dehydrogenase-5 isoenzyme levels were elevated. It was further demonstrated that, although the physical properties of the injectable solution (pH, osmolality) and to a lesser extent the injection procedure itself caused slight changes in plasma enzyme levels, the ket…

Ketamine for anaesthetic induction at Caesarean section.

Fifty healthy mothers, with normal placental function, were anaesthetised with ketamine (2 mg/kg body mass) for Caesarean section. Surgery was conducted with the patient in the lateral tilt position and anaesthesia was maintained with nitrous oxide, oxygen, muscle relaxants and controlled ventilation. Eight of the 50 infants delivered were clinically depressed, judged on the basis of the modified Apgar score at 2 minutes after delivery. The average time to sustained respiration was 58, 1 minutes. Studies of maternal blood gases before induction and at delivery revealed mild respiratory alkalos…

Effects of ketamine on left ventricular performance.

The cariac response to anesthetic doses of ketamine hydrochloride was studied in dogs, initially in the absence of other drugs and subsequently during beta adrenergic block with propranolol and combined beta adrenergic and cholinergic blockade with propranolol and atropine. Ketamine (4 mg/kg) was injected into the left atrium or the jugular vein. Administration of ketamine alone resulted in increases in heart rate (61 beats/min, P smaller than .001), cardiac output and left ventricular systolic pressure, but left ventricular end-diastolic pressure and dP/dt max (maximum rate of change of left …

Molecular Mechanisms and Toxic Effects of Ketamine.

Ketamine is a dissociative anesthetic. It is clinically used as a surgical anesthetic or anesthetic inducer and has a certain degree of mental dependence. Its abuse can lead to nerve damage, adverse emotional reactions and other toxic side effects. The primary mechanism by which ketamine exerts its pharmacological effects is to block N-methyl-D-aspartate receptors (NMDAR). It also functions through pathways such as α-amino-3-hydroxy-5-methyl-4-isox-azolepropionic acid receptors (AMPAR), opioid receptors, γ-aminobutyric acid (GABA) receptors, monoaminergic receptors, cholinergic receptors, hype…
