Symptomatic and complicated adult and adolescent primary obstructive megaureter--indications for surgery: analysis, outcome, and follow-up.

OBJECTIVES: Primary obstructive megaureter is an uncommon presentation in adult patients. Although not reported frequently in published studies, the anomaly exists and warrants aggressive surgical management in contrast to its presentation in children. METHODS: This study was composed of 55 patients (47 with unilateral and 8 with bilateral megaureters) with adult primary obstructive megaureter who were treated from January 1989 to December 2001. Their clinical presentation, renal function, radiologic data, treatment, complications, and follow-up were studied. RESULTS: Of the 55 patients, 36

Renal extravasation of urine due to bladder outflow obstruction.

Spontaneous extravasation of urine from the kidney is uncommon. The majority of cases in adults are associated with ureteral calculi, while in children the condition is associated with urethral valves of ureteropelvic junction obstruction.

Stone Between Crossed Paths: Retrograde Intrarenal Surgery for Isthmic Calculus in the Setting of Renal and Gut Malformations.

Horseshoe kidney is the most frequent type of renal fusion anomaly, and it often has associated changes in the orientation of the kidneys, anomalous vascular supply, and poor urinary drainage, as well as an increased risk of nephrolithiasis. Horseshoe kidney occurs in conjunction with intestinal malrotation very infrequently, and the anatomical and technical difficulties are significant because of the distorted anatomy of the kidney and the abnormal bowel position; therefore, careful preoperative planning and an individualized surgical strategy are needed for the management of renal calculi in

A case of staghorn stones in a kidney with an ileal ureter treated by percutaneous nephrolithotomy.

BACKGROUND: A 59-year-old man was admitted to hospital for investigation of a 1-year history of intermittent hematuria. He had undergone ileal ureteral replacement for left renal stones 36 years earlier. INVESTIGATIONS: Renal ultrasonography, physical examination, abdominal plain radiography, intravenous urography, CT urography, measurement of serum levels of creatinine, urea and electrolytes, renal scintigraphy, urinalysis and urine culture. DIAGNOSIS: Staghorn calculi in the left kidney, with a high-lying anastomosis between the renal pelvis and the proximal ileal segment. MANAGEMENT: The

[Urolithiasis].

Epidemiologic investigation did not show any special environmental combination causing urinary calculi creation. The disease concerns usually people between 25-50 year old. Most of the urinary calculi are compound of calcium oxalate and phosphate. In contaminated urine more often ammonia-magnesium phosphate. General constitutional factors having influence on urinary stones creation are specific gravidity, crystallization inhibiting factors concentration, hypercalcuria, urine acidity, hyperoxaluria and urinary tract infection. Unilateral, single urine stone is usually the effect of the local fa

Complications of Mini-Percutaneous Nephrolithotomy for Staghorn Stones.

Background and objective Staghorn calculi are big, intricate kidney stones that reside in the calyceal and renal pelvis. They present serious surgical difficulties and a high risk of complications. A less invasive option to standard percutaneous nephrolithotomy (PCNL), mini-percutaneous nephrolithotomy (mini-PCNL) uses smaller tract diameters to minimize bleeding, postoperative pain, and hospitalization while preserving similar stone-free rates (SFR). However, there is still little proof of its effectiveness and safety for staghorn stones, especially in local contexts. This study aimed to asse

Extraction of urethral calculi by transabdominal cystoscopy and urethroscopy in nine dogs.

OBJECTIVES: To describe a minimally invasive technique for treating urethral obstructions in male dogs and to review the postoperative results. METHODS: All dogs (n=9) had urethral obstruction due to calculi. Obstructions were verified by radiographic and ultrasonographic examinations. Dogs with impaired kidney function were not included in the study. A 5-mm diameter trocar and cannula were placed in the ventral midline, 2 cm cranial to the umbilicus, allowing placement of a 10-mm diameter cannula under visual guidance, adjacent to the apex of the bladder. The bladder was then partially exter

Minimally Invasive Approaches in Pediatric Urolithiasis. The Experience of Two Italian Centers of Pediatric Surgery.

Background: Over the last 30 years, the incidence of pediatric urolithiasis (PU) has been increasing and the surgical management has evolved toward a minimally invasive approach (MIA). We reported the experience of two Centers of Pediatric Surgery in the management of PU, focusing on MIA as first choice in treatment. Methods: Data were retrospectively analyzed from October 2009 to October 2019 in children with urolithiasis who were admitted to two referral Italian Centers of Pediatric Surgery. Demographic and clinical data of the patients, features of the urolithiasis, type of surgery were con

Renal calculi in dogs and cats: prevalence, mineral type, breed, age, and gender interrelationships (1981-1993).

Three hundred seventeen specimens of urinary calculi of renal origin from 214 female dogs and 103 male dogs, and 71 specimens of urinary calculi of renal origin from 38 female cats and 33 male cats were submitted for mineral analysis between July 1, 1981, and December 31, 1993. Among dogs, 45 breeds were affected with renal calculi. Thirty-three breeds and a crossbred group were represented among females, but 8 breeds and the crossbred group accounted for 81% of the total. Among male dogs, 30 breeds and a crossbred group were represented, but 7 breeds and the crossbred group accounted for 69%

Replacement lipomatosis: CT and MRI findings of a rare renal mass.

Replacement lipomatosis of the kidney (RLK) is an advanced form of renal sinus lipomatosis, in which infection, renal calculi and long-standing hydronephrosis are accompanied by renal parenchymal atrophy. The kidneys are usually poor or non-functioning. We present CT and MRI findings of an unusual focal RLK of a 52-year-old male, who was examined with the suspicion of renal malignancy.

Xanthogranulomatous pyelonephritis with calculus migration into the psoas abscess: an unusual complication.

Xanthogranulomatous pyelonephritis (XGP) is a rare variant of chronic pyelonephritis. It is characterized by progressive parenchymal destruction caused by chronic renal obstruction due to calculus, stricture, or rarely tumor, resulting in kidney function loss. Herein, we describe the case of a 36-year-old female who presented with left loin pain, left lower limb pain, and dysuria. On contrast-enhanced computed tomography (CECT), multiple abscesses and an obstructive staghorn calculus were depicted in the left kidney with the classical appearance of "Bear Paw Sign." An abscess with calculi was

Nephrocalcinosis in a child with autosomal dominant polycystic kidney disease and a prolapsing ectopic ureterocele.

Although autosomal dominant polycystic kidney disease commonly presents in adults, it can occur in children. Usually, renal calcification in patients with autosomal dominant polycystic kidney disease is manifested as calculi or as hemorrhage into a renal cyst. An ectopic ureterocele is a well-known finding in patients with renal duplication. To our knowledge, this is the first case report of a child who had combined findings of autosomal dominant polycystic kidney disease, nephrocalcinosis, and an obstructing ectopic ureterocele.

Anterior transperitoneal approach for stone removal in horseshoe kidneys (its advantage for bilateral stones).

The anterior transperitoneal approach for removal of stone from a horseshoe kidney is described. The valuable advantages inherent in this technique have been shown in 13 patients, 7 with unilateral calculus and 6 with bilateral calculi, and all with horseshoe kidneys. No urinary leak occurred and the post-operative course was uneventful. The operative field is extensive and a good exposure of both sides of the horseshoe kidney can be obtained, enabling bilateral pyelolithotomy or other conservative renal procedures to be performed.

Assessment of the efficacy of reduced-radiation noncontrast computed tomography scan compared with the standard noncontrast computed tomography scan for detecting urolithiasis: A prospective single-center study.

BACKGROUND: Noncontrast computed tomography (CT) scan of the kidneys, ureter and bladder is the standard investigative modality for diagnosing and following up patients with urolithiasis. With each scan, a patient receives radiation of 18-34 mGy. Dose considerations become pertinent because of a 10% lifetime incidence rate and higher than 50% risk of recurrence, necessitating repeated imaging in the lifetime of a stone former. Hence, this study aimed to assess the sensitivity of "reduced-radiation" CT imaging by altering scan settings to lower than the "standard" norms. MATERIALS AND METHODS:

Changing composition of staghorn calculi.

PURPOSE: The 2005 American Urological Association Guideline on the Management of Staghorn Calculi suggests that metabolic stones are uncommon in the composition of staghorn calculi. We determined the incidence and treatment outcomes of metabolic stones in patients with complete staghorn calculi compared to infection stones in those undergoing percutaneous nephrolithotomy. MATERIALS AND METHODS: We retrospectively analyzed all patients who underwent percutaneous nephrolithotomy for complete staghorn calculi between April 2005 and April 2010. Stone analysis was reviewed to classify stone type a
