[Megacalycosis in childhood].

Congenital megacalycosis is a renal dysplasia characterised by hypoplasia of the medullary pyramids and associated non-obstructive dilatation of the calyces. Calyceal dilatation promotes via disturbed urodynamics the manifestation of chronic interstitial nephritis and calculus formation. Therapy is primarily not necessary, but in the presence of stone development and chronic infections surgical procedures may become indispensable. Pathogenesis, diagnosis, differential diagnosis and therapy of megacalycosis in two children are discussed.

Surgical management of benign renal cysts causing obstruction of renal pelvis.

During the last fifteen years, 6 adult patients were treated in whom obstruction of the ureteropelvic junction or the renal pelvis was caused by a benign renal cyst. The cyst extended from the renal hilus in each case and had turgor sufficient to overcome the pressure of intrapelvic urine. Operative unroofing of the cyst relieved the obstruction in each. In 5 of 6 cases, the postoperative radiographic appearance of the kidney showed marked improvement. Each patient was followed for at least five years, and no recurrence of the obstruction was noted. Operative treatment is recommended when a re

Percutaneous nephrolithotomy for management of upper urinary tract calculi in patients with autosomal dominant polycystic kidney disease.

OBJECTIVES: To evaluate the role of percutaneous nephrolithotomy in management of the patients with autosomal dominant polycystic kidney disease and associated nephrolithiasis. METHODS: A total of 19 patients with autosomal dominant polycystic kidney disease and upper urinary tract stones were included in the study. One patient had bilateral renal stones. Most of the stones were located in the renal pelvis and/or calices with a stone size >2 cm in the largest diameter. All patients were treated by percutaneous nephrolithotomy at 2 centers. Therefore, 20 percutaneous nephrolithotomy procedures

[Spontaneous rupture of a simple renal cyst to the pyelocalyceal system. Evolution from Bosniak I to IIF].

OBJECTIVE: To report the spontaneous rupture of a renal cyst into the adjacent pyelocalyceal collecting system. METHODS: We present the case of a 47 year old woman with a 17 centimeter simple renal cyst (Bosniak I) as well as ipsilateral nephrolithiasis. The patient had a febrile urinary tract infection with flank pain. A subsequent CT scan revealed that this cyst spontaneously ruptured into the renal pelvis. RESULTS: Follow up evaluations showed the former cyst has decreased in size and contain thick and nodular calcifications. At present it is a Bosniak IIF cyst. CONCLUSION: Simple renal

Limitations of extracorporeal shockwave lithotripsy for lower caliceal stones: anatomic insight.

In addition to gravity-dependent position, we suppose that other particular anatomic features may be important in the retention of stone debris in lower calices after extracorporeal shockwave lithotripsy (SWL). We analyzed the inferior-pole collecting system anatomy in 146 three-dimensional polyester resin corrosion endocasts of the pelviocaliceal system. In 74% of the cases, there was an angle of greater than 90 degrees formed between the lower infundibulum and the renal pelvis, and in 26%, the angle was 90 degrees or less. In 60%, there was a lower infundibulum 4 mm or larger in diameter. Th

Comparison of the reliability of two hydronephrosis grading systems: the Society for Foetal Urology grading system vs. the Onen grading system.

AIM: To compare the reliability of the conventional ultrasonography grading system for hydronephrosis as suggested by the Society for Fetal Urology (SFU) in 1993 and that developed by Onen in 2007. MATERIALS AND METHODS: One hundred and eighty kidneys in 90 paediatric patients were assessed by four radiologists using each of the two grading systems twice. The SFU system was graded 0-4 (0 = no hydronephrosis; 1 = visualized only renal pelvis; 2 = plus a few caliceal dilatation; 3 = all calyceal dilatation; 4 = plus parenchymal thinning). The Onen system was graded 0-4 (0 = no hydronephrosis; 1

Presence of the eggshell sign in obstructive uropathy.

OBJECTIVES: To analyze the relationship between hydronephrosis due to urinary tract obstruction and the presence of the sonographic eggshell sign, which is a recently described crescent of increased echogenicity at the caliceal/parenchymal interface and possibly an indicator of raised intrarenal pressure. METHODS: All patients presenting between 1996 and 1999 for surgical management of pelviureteral junction obstruction or congenital posterior urethral obstruction had films reviewed for the presence of the eggshell sign. RESULTS: Of 94 patients, 40 presented postnatally and 54 had hydronephr

Clinical analgesic nephropathy.

Analgesic nephropathy is recognized worldwide, but the differences in incidence in various countries, or regions, remain unexplained. Analgesic compounds may cause both functional and structural renal damage. This damage may be related to depletion of glutathione and renal vasoconstriction (probably mediated through prostaglandin depletion) and to the fact that the concentrations of glutathione and prostaglandins and their metabolites in the kidneys are manyfold their concentrations in plasma. Most patients with analgesic nephropathy are middle-aged women with histories of peptic ulcer, anemia

Robotic-assisted laparoscopic ureterocalicostomy (RALUC): a systematic review of its applications.

PURPOSE: Ureterocalicostomy refers to the anastomosis between the lower pole calyces and the ureter. Robotic-assisted laparoscopic ureterocalicostomy (RALUC) is gaining ground, ultimately. The current systematic review summarizes all applications of RALUC in both adults and children. METHODS: In line with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) Guidelines, three databases (PubMed, Scopus and Cochrane) were screened, from their inception to 16 February 2025. The following search string was used: robotic AND (ureterocalicostomy OR ureterocalicostomies OR

Laparoscopic ureterocalicostomy in children: The technique and feasibility.

Ureterocalicostomy is a salvage technique commonly used for failed pyeloplasties; it has also been reported as a primary procedure in ureteropelvic junction obstruction (UPJO). This video describes the technique of laparoscopic ureterocalicostomy for primary UPJO in a child with a malrotated kidney and parenchymal thinning. A 13-year-old girl with symptomatic UPJO was found to have a malrotated kidney with a high posterior insertion of the ureter. A laparoscopic dependent ureterocalicostomy over a double-J stent was performed. The postoperative course was uneventful, with excellent clinical an

[Surgical treatment of purulent gestational pyelonephritis long-term results].

For 20-year period surgery on the kidneys in purulent pyelonephritis was conducted in more than 130 pregnant women. Pyelonephritis was represented by apostematous nephritis and carbuncules with purulent parenchymal fusion. Long-term results (2 to 13 years) were studied in 75 women. Depending on the disease severity, nephrostomy or nephrectomy was performed (61 and 14 cases, respectively). Treatment results were assessed by excretory urography, ultrasound and radionuclide investigations of the kidneys, results of urine bacterial test. The treatment efficacy criterion was the absence of bacteriu

[Sinus-parenchyma border of the human kidney as the site of origin of intracanalicular and extravascular diffusion of fluids].

Injecting fluids into the kidney against the physiological direction of flow leads to a passage of liquid across the border-line between the renal sinus and the parenchyma. This event, caused by a sudden raising of pressure during the injection is called pyelorenal backflow. It can be divided into a number of subtypes, as there are: pyelocanalicular backflow, pyelosinous backflow, pyelovenous backflow, pyelolymphatic backflow. Among all these phenomena only the pyelocanalicular backflow can be interpreted as a genuine backflow in the fullest sense of the word, while the others never can happen

Predominant role of cytosolic phospholipase A2α in dioxin-induced neonatal hydronephrosis in mice.

Hydronephrosis is a common disease characterized by dilation of the renal pelvis and calices, resulting in loss of kidney function in the most severe cases. 2,3,7,8-Tetrachlorodibenzo-p-dioxin (TCDD) induces nonobstructive hydronephrosis in mouse neonates through upregulation of prostaglandin E2 (PGE2) synthesis pathway consisting of cyclooxygenase-2 (COX-2) and microsomal prostaglandin E synthase-1 (mPGES-1) by a yet unknown mechanism. We here studied possible involvement of cytosolic phospholipase A2α (cPLA2α) in this mechanism. To this end, we used a cPLA2α-null mouse model and found that c

Vascular polyps of ureter associated with megacalycosis and nephrolithiasis.

Multiple benign vascular polyps of the ureter are rare, and the associated congenital megacalycosis and nephrolithiasis are very uncommon. Herein we report a case with vascular polyps of the right ureteropelvic junction dissociated with extensive hydronephrosis which was considered to be megacalycosis. Clinical features and treatment modalities are reviewed.

[The 1-cm lower calyx calculus: SWT, URS or Mini-PCNL? Guidelines vs. reality.].

Urinary stones of the upper urinary tract can be considered a widespread public health concern due to their high incidence and prevalence and their health policy-related and financial implications. A significant proportion of newly diagnosed kidney stones are lower-pole stones, i.e., stones affecting the lower calyx group of the renal pelvicalyceal system. These are often diagnosed by chance, i.e., as incidental findings during ultrasound or CT scans performed for other reasons, or as "secondary stones" detected during the diagnostic work-up of symptomatic urinary stones in other locations. Characterization of intracalyceal pressure during ureteroscopy.

PURPOSE: To provide the first report of measuring intracalyceal pressures during ureteroscopy (URS). METHODS: A prospective single-center clinical study using a cardiac pressure guidewire to measure intracalyceal pressure during flexible URS was performed. Eight patients (45 calyces) undergoing URS for nephrolithiasis were included. A Verrata® pressure guide wire was passed through the working channel of a dual lumen flexible ureteroscope and into the calyces while irrigation was maintained at 150 mmHg. Pressure was measured in the renal pelvis, upper pole, interpolar, and lower pole calyces

Calyceal diverticula in children: imaging findings and presentations.

A calyceal diverticulum consists of a cystic eventration in the renal parenchyma that is lined with transitional cell epithelium with a narrow infundibular connection with the calyces or pelvis of the renal collector system; thus, the term pyelocalyceal diverticulum would be more accurate. Very rare in pediatric patients, calyceal diverticula can be symptomatic and require treatment. Calyceal diverticula are underdiagnosed because they can be mistaken for simple renal cysts on ultrasonography. To determine the approach to their follow-up and management, the diagnosis must be confirmed by excre

Retroperitoneal laparoscopic partial nephrectomy with segmental renal artery clamping for cancer of the left upper calyx: a case report.

BACKGROUND: Currently, the standard treatment for renal pelvis carcinoma is radical nephroureterectomy with bladder cuff excision. To describe the feasibility of retroperitoneal laparoscopic partial nephrectomy with segmental renal artery clamping for cancer of renal pelvis, we report this special case for the first time. CASE PRESENTATION: A 67-year-old woman received this operation. Preoperative ureteroscopy revealed a papillary neoplasm with a pedicle in the upper calyx of the left kidney. After entering the retroperitoneal space and dissociating the renal artery and renal vein, the target
