[Ipsilateral traumatic dislocation of hip and knee joints. Case report].

Traumatic obturator dislocation of the hip without fracture associated with antero-lateral knee dislocation is considered to be an extreme injury. Such a case, which has never been treated in our department, is reported here. The patient was a 40-year-old man who suffered his injury in a motorcycle accident. He had ipsilateral traumatic obturator hip dislocation and anterior knee dislocation.Within two hours of injury dislocation reduction was carried out under general anaesthesia, the anterior cruciate ligament was reconstructed using BTB allograft and a CT scan was obtained. In this type of …

Vascular lesions associated with bicruciate and knee dislocation ligamentous injury.

INTRODUCTION: The incidence of associated vascular lesions in biligamentous cruciate injuries of the knee ranges from 16 to 64%, with a mean rate of 30%. Treatment of ischemic vascular lesions associated with ligaments injury is well established, comprising emergency arterial vascular repair, most of the times combined to external fixation. In the absence of clinical symptoms of vascular lesion, some authors recommend systematically performing arteriography, while others advocate selectively prescribing this examination in doubtful clinical situations. The present study analyzed data extracted

Irreducible posterolateral knee dislocation resulting from a low-energy trauma.

A small amount of knee dislocations is included in the irreducible knee dislocations group. In such instance, medial femoral condyle is buttonholed through the gap formed by medial retinacular and capsular structures and this prevents reduction. In this study, we present two cases in which there were irreducible posterolateral knee dislocations resulting from a low-energy trauma. In both cases, dimple sign produced by the invagination of the medial retinacular structures and capsule and ecchymosis medially were noted. Soft tissue invaginated between the trochlea and intercondylar notch was ext…

[Knee dislocation treatment with temporary tibio-patellar fixation (patellar olecranization)].

Four patients with traumatic dislocation of the knee has been treated with olecranization of the patella. Mean age was 44.4 years (range 27-75 years). Three of them were politrauma patients, but none had popliteal artery injury or peroneal nerve palsy. Olecranization of the patella was performed on the day of injury. Results were assessed according to the Marshall scale. Three results were rated excellent and good with near normal knee function. In one case result was rated fair since despite stable knee and full range of motion pain persisted and marked vascular changes, not related to the in

Traumatic bilateral knee dislocations, unilateral hip dislocation, and contralateral humeral amputation: a case report.

Bilateral traumatic knee dislocations are a rarity. We report a case of bilateral traumatic knee dislocations with concomitant right hip dislocation and complete traumatic amputation of the left, nondominant upper extremity at the level of the proximal one-third of the humerus. Angiograms revealed no evidence of popliteal artery injury. Orthopedic treatment consisted of immediate reduction of the dislocations and urgent revision amputation of the upper extremity. Staged, bilateral knee ligamentous reconstructions were performed on hospital days 24 and 29, respectively. Despite this constellati…

High prevalence of knee osteoarthritis at a minimum 10-year follow-up after knee dislocation surgery.

PURPOSE: Long-term outcomes and the prevalence of osteoarthritis after surgical treatment of knee dislocations are lacking in the literature. The purpose of this study was to investigate the prevalence of knee osteoarthritis and knee function at a minimum of 10 years after knee dislocation surgery. METHODS: Sixty-five patients surgically treated for knee dislocations at a single level I trauma center between May 1996 and December 2004 were evaluated at a minimum of 10 years. Patients were evaluated with radiographs for knee osteoarthritis using the Kellgren-Lawrence (KL) grading system, Tegne…

Posterior knee dislocation after total knee arthroplasty in a patient with multiple sclerosis. A case report.

A patient with multiple sclerosis (MS) and severe osteoarthritis of the right knee with valgus deformity developed early in the postoperative period a posterior dislocation of the knee after total knee arthroplasty (TKA), due to severe spasm of her hamstring muscles. At revision surgery the posterior cruciate retaining prosthesis was exchanged for a non-linked constrained knee prosthesis. Technical considerations are described for this rare complication, which remains a serious clinical challenge for the orthopaedic surgeon.

A Comparative Study of Clinical Outcomes and Functional Status after Knee Fracture and Knee Fracture Dislocation.

The aim of this study was to compare outcomes of tibial plateau fracture dislocations (FD) with tibial plateau fractures alone. This study was an analysis of a series of tibial plateau fractures, in which FD was defined as a fracture of the tibial plateau with an associated loss of congruent joint reduction and stability of the knee, and classified by the Moore system. Patient data collected included demographics, injury information, and functional outcomes (short musculoskeletal function assessment [SMFA] score and Pain by the visual analog scale). Clinical outcomes at follow-up were recorded

Abnormally high dislocation rate following constrained condylar knee arthroplasty for valgus knee: a case-control study.

BACKGROUND: With the use of constrained condylar knee (CCK) prostheses, dislocation of the knee following total knee arthroplasty (TKA) with valgus deformity is rare. In our practice with such patients, however, an abnormally high dislocation rate was noted. It appeared to be associated with the extent of soft-tissue release which varied among surgeons following different sequences of release. We asked in CCK TKA with valgus deformity is releasing both the lateral collateral ligament (LCL) and popliteus tendon (PT) associated with the occurrence of dislocation. METHODS: This is a case-control

Recurrent Knee Dislocation With Vascular Injury in a Patient With Prior Total Knee Arthroplasty: A Case Report.

Vascular injury after total knee arthroplasty (TKA) is exceedingly rare, with the potential to have catastrophic consequences. This injury h … level of suspicion for and a low threshold to obtain CT angiography in any patient with concern for knee dislocation. TKA incidence will continue to rise as the population ages, and dislocation can lead to significant injury, morbidity, and even mortality. It must be considered, even in low-energy trauma, quickly recognized and treated with specialty assistance.

Long-term Outcomes of Surgically Treated Congenital Dislocation of the Knee.

BACKGROUND: Congenital dislocation of the knee (CDK) may be idiopathic or associated with another condition, such as Larsen syndrome or arthrogryposis. Surgical reduction of type-3 dislocation may require quadricepsplasty (QP) or femoral diaphyseal shortening (FS). Because it is unknown which treatment is more effective, we evaluated long-term outcomes using patient-reported questionnaires and gait analysis, comparing results by surgery type and underlying diagnosis. METHODS: Twelve patients (mean age, 19 mo) were treated surgically for CDK from 1985 to 2015 and studied 9 to 30 years postoper

Successful Management of Impending Skin Necrosis After a Posterolateral Knee Dislocation.

Incarcerated medial soft tissue after posterolateral knee dislocations has been described, but limited information pertaining to the etiology and management of cutaneous injuries from incarceration exists. We present the case of a 64-year-old man, where reduction of a posterolateral knee dislocation resulted in incarceration of medial ligamentous structures and impending skin necrosis. The patient avoided full-thickness skin necrosis, which could have complicated treatment options. Careful consideration of the soft-tissue envelope of the knee for preventing additional skin injury in the periop…

Traumatic Bifocal whereas and Ipsilateral Posterior Knee Dislocation: A Case Report.

CASE: A 19-year-old man sustained combined, ipsilateral inferior hip and posterior knee fracture-dislocations secondary to a motor vehicle collision. He underwent immediate closed reduction of the knee and delayed open reduction internal fixation but required emergent open hip reduction for an irreducible femoral head incarcerated on a pubic root fracture. At the 1-year follow-up, he demonstrated excellent functional outcome with painless and full hip and knee range of motion. CONCLUSION: Irreducible inferior femoral head dislocation in combination with a knee dislocation requires thoughtful …

Vascular and Neurological Injury in Children With Posterior Traumatic Knee Dislocation.

Posterior knee dislocations (PKD) in children are uncommon but may be associated with vascular injury. The purpose of this study is to characterize the frequency of vascular injury in PKD as well as define patient characteristic, procedures, types of treating hospitals, and clinical outcomes. This study utilized the National Trauma Data Set (NTDS) from the American College of Surgeons on years 2015 and 2016. All demographic and clinical data on pediatric patients (≤18 years) with the ICD codes for PKD were obtained. 44 PKD were identified. The median age was 17 years [IQR 15,18], 70% male; 49%…

Total knee arthroplasty for treatment of osteoarthritis with prolonged patellar dislocation.

Prolonged dislocation of the patella is a rare condition and is often related to severe osteoarthritis (OA) of the femorotibial (FT) joint. For this condition's treatment, numerous surgical techniques using total knee arthroplasty (TKA) have been published. To the best of our knowledge, this case report is the first description of the use of lateral release alone to treat recurrent patellar subluxation with TKA. An interesting point in this case is that the patient had a good recovery after TKA in spite of quite a long-term (a duration of almost 55 years) dislocation of her patella and develop

Is peroneal nerve injury associated with worse function after knee dislocation?

BACKGROUND: Peroneal nerve palsy is a frequent and potentially disabling complication of multiligament knee dislocation, but little information exists on the degree to which patients recover motor or sensory function after this injury, and whether having this nerve injury--with or without complete recovery--is a predictor of inferior patient-reported outcome scores. QUESTIONS/PURPOSES: The purposes of this study were to (1) report on motor and sensory recovery as well as patient-reported outcomes scores of patients with peroneal nerve injury from multiligament knee dislocation; (2) compare th…

Risk factors for peroneal nerve injury and recovery in knee dislocation.

BACKGROUND: Acute knee dislocation is rare but has a high rate of associated neurovascular injuries and potentially limb-threatening complications. These include the substantial morbidity associated with peroneal nerve injury: neuropathic pain, decreased mobility, and considerably reduced function, which not only impairs patient function but complicates treatment. QUESTIONS/PURPOSES: We therefore identified and quantified the risks associated with specific factors for peroneal nerve injury and recovery in patients with knee dislocations. PATIENTS AND METHODS: We retrospectively reviewed the …
