The floating knee. 40 cases of ipsilateral fractures of the femur and the tibia.

"Floating knee" is the term used to describe simultaneous fractures of the femur and tibia in the same limb. Thirty nine patients, with 40 such injuries, are presented with a follow up interval of six months to five years. The final functional result was poor if the femoral fracture was situated in the condylar flare and the results were comparatively better in those cases treated by cast bracing or when the fracture of the femur was stabilised internally. In all cases the fracture of the tibia was treated conservatively.

Isolated fracture-avulsions of the tibial insertions of the cruciate ligaments of the knee.

This paper discusses the treatment of isolated fracture-avulsions of the tibial insertions of the cruciate ligaments of the knee. The indications for conservative treatment or open reduction and osteosynthesis, are based on a classification by Meyer and McKeever which takes into account the displacement of the avulsed fragment. This is the crucial factor in the choice of treatment.

The Role of Arthroplasty in Acute and Failed Fracture Surgery Around the Knee.

Total knee arthroplasty (TKA) has become an increasingly valuable solution for managing acute periarticular fractures and failed fixation, particularly in elderly patients with severe comminution or underlying arthritis. Its key advantage lies in enabling immediate weight bearing and potentially reducing the need for reoperation compared with traditional fixation methods such as open reduction and internal fixation or intramedullary nailing. Although open reduction and internal fixation and intramedullary nailing remain standard fixation methods for many fracture types, primary TKA is gaining

The Incidence of Posterolateral Tibial Plateau Fractures in the Setting of Knee Dislocations.

BACKGROUND: Acute knee dislocations are devastating injuries that can be challenging for surgeons to treat. The bony integrity of the tibial plateau and the lateral femoral condyle has a fundamental meaning for translational and rotational knee joint stability. Posterolateral tibial plateau depression fractures (PLTPFs) and lateral femoral condyle impaction fractures (LFCIFs) can influence clinical outcomes, but their frequency and characteristics in the setting of acute knee dislocations are unknown. PURPOSE: To report the incidence of PLTPFs and LFCIFs in acute knee dislocations as well as

Total Knee Arthroplasty in Pseudarthrosis of Hoffa Fracture: A Case Report.

CASE: A 55-year-old woman presented with chronic left knee pain and functional disability 1 year after nonoperative treatment of Hoffa fracture of lateral femoral condyle, due to pseudarthrosis and consequent knee osteoarthritis. She underwent single-stage fracture fixation and primary total knee arthroplasty with successful outcome at her 4-year follow-up. CONCLUSION: The combination of Hoffa fracture fixation and primary total knee arthroplasty can achieve satisfactory results in terms of pain relief, functional improvements, and avoid complications related to a 2-stage procedure. This case

Heterotopic ossification in the knee following retrograde nailing of a femur fracture.

BACKGROUND: Heterotopic ossification (HO) is a pathologic bone formation in extra skeletal tissue and articular space. This was an objection to nailing of femur fractures across the knee; however, this has not been the case in many thousands of cases. Nonetheless, we present a patient in who placement of a retrograde nail resulted in calcification in the knee requiring excision. CASE PRESENTATION: A 42-year-old male presented to the clinic complaining of pain, popping, clicking, and mocking in the right knee, especially in the patellar region. Nine months prior he suffered an ipsilateral femu

Open Periprosthetic Knee Fracture: A Case Report and Review of the Literature.

INTRODUCTION: With the increase in knee and hip implants, these periprosthetic fractures will become more common especially as the population ages. Open periprosthetic fractures are rare and severe injuries and are more likely to be seen in high-energy injuries. They present challenges to the treating physician due to soft tissue damage, contamination of the existing implants, and the effects of polytrauma in the geriatric patient. METHODS CASE REVIEW REPORT AND REVIEW OF LITERATURE RESULTS: A 72-year-old woman was involved in a motor vehicle collision with multiple injuries including an open

Influence of Hip Fracture on Knee Pain During Postoperative Rehabilitation.

OBJECTIVE: To investigate whether fracture type, surgical procedure, or fracture grade affect knee pain during postoperative rehabilitation after a hip fracture. METHODS: We conducted a retrospective case-controlled study of 139 patients during postoperative rehabilitation after surgery for hip fractures. Patients were divided into two groups: patients experiencing knee pain during the first week of postoperative rehabilitation, and patients without knee pain. We compared the types of fracture, surgical procedure, and fracture grade between the two groups. RESULTS: We enrolled 52 patients (3

Factors related to stress fracture after unicompartmental knee arthroplasty.

BACKGROUND/OBJECTIVE: Unicompartmental knee arthroplasty (UKA) is a low-invasive knee surgery that enables early recovery. Stress fracture of the medial tibial plateau (MTP) is a complication of UKA that prolongs treatment once it has occurred. We investigated factors affecting its occurrence. METHODS: The study subjects were 167 patients who underwent fixed-bearing UKA between 2009 and 2016 (45 men and 122 women of mean age 77 years, including 134 with osteoarthritis of the knee and 33 with spontaneous osteonecrosis). We measured bone mineral density, installation angle of the tibial compone

The Use of Tomosynthesis in the Global Study of Knee Subchondral Insufficiency Fractures.

RATIONALE AND OBJECTIVES: Subchondral insufficiency fractures (SIF), previously termed spontaneous osteonecrosis of the knee, are marked by a sudden onset of severe pain. Other than the size of the lesion, prediction for progression to joint replacement is difficult. The objective was to determine if quantitative analysis of bone texture using digital tomosynthesis imaging would be useful in predicting more rapid progression to joint replacement. MATERIALS AND METHODS: Tomosynthesis studies of 30 knees with documented SIF were quantified by fractal, mean intercept length (MIL), and line fract

Treatment of seawater immersion-complicated open-knee joint fracture.

The current study aimed to select suitable remedies for seawater immersion-complicated open-knee joint fracture by exploring the effects of different treatment methods. Forty adult rabbits weighing 2.20 ± 0.25 kg were divided equally into internal fracture fixation group (A), seawater-immersed group with primary internal fixation (B), seawater-immersed group with secondary internal fixation (C), and seawater-immersed group with external fixation (D), using the random-digit table method. Open-femoral internal condylar fracture models were established. Group A was left untreated for 2 h, whereas

Concomitant ligamentous and meniscal knee injuries in femoral shaft fracture.

BACKGROUND: Concomitant knee injury is a common finding in femoral fractures but can be easily missed during early management of the initial trauma. Degrees of damage to the articular structures vary considerably; from only a mild effusion to complete ligamentous and meniscal tears. Since previous reports were mostly from developed societies, this study was designed to look into characteristics of associated knee injury in a sample from Iran, to represent a developing country perspective. MATERIALS AND METHODS: Consecutive patients admitted to an orthopedic ward of Baqiyatallah hospital (Tehr

Medial tibial plateau fracture complicating unicompartmental knee arthroplasty.

We describe a new cause of a medial tibial plateau fracture complicating the early postoperative rehabilitation following unicompartmental knee arthroplasty. The patient was successfully treated by open reduction and internal fixation by buttress plating the fracture and retaining the prosthesis. The treatment option used proved to be successful, although careful patient selection for unicompartmental knee arthroplasty should be recommended to decrease the risk of this complication.

Knee laxity associated with a proximal diaphyseal tibial and fibular fracture.

If operative treatment of a proximal diaphyseal tibial and fibular fracture is required, in most cases, it involves reduction and stabilization of only the tibial fracture. In this case report, after stabilization of the tibial fracture with an intramedullary nail, the patient continued to demonstrate significant varus knee laxity, despite an intact proximal tibiofibular joint and undisplaced fibular head. The stability of the knee was achieved only with internal fixation of the segmental fibular fracture. This case shows the importance of assessing knee laxity in all cases of proximal tibial

Total knee arthroplasty in patients with a prior fracture of the tibial plateau.

BACKGROUND: A fracture of the tibial plateau may predispose the knee to the development of posttraumatic arthritis. Malunion, intra-articular chondro-osseous defects, limb malalignment, retained internal fixation devices, and poor surrounding soft tissues may in turn compromise the outcome of total knee arthroplasty. The aim of our study was to evaluate the results of total knee arthroplasty in patients with a previous fracture of the tibial plateau. METHODS: The results of sixty-two condylar total knee arthroplasties performed with cement, from 1988 to 1999, in sixty-two patients with a prev

Arthroplasty in treating knee osteoarthritis and proximal tibia stress fracture.

The authors describe two cases of severely angulated tibial non-unions after proximal tibia stress fractures associated with ipsilateral osteoarthritis treated with modular knee endoprosthesis with a long tibial stem to stabilize non-union fragments. During the procedure, no additional osteosynthesis or bone grafting was added. Both non-unions healed after 6 months with no post-operative complications. The authors suggest implantation of modular knee endoprosthesis, as a single procedure, in treatment of proximal tibia non-union after a stress fracture as a result of severe varus/valgus deform

Total knee arthroplasty following prior distal femoral fracture.

BACKGROUND: Femoral fracture may predispose the knee to the development of post-traumatic arthritis by either a direct intra-articular injury or residual limb malalignment. Malunion, intra-articular osseous defects, limb malalignment, retained internal fixation devices, and compromised surrounding soft tissues may in turn affect the outcome of total knee arthroplasty (TKA) in these patients. Subtrochanteric stress fracture of the femur following total knee arthroplasty.

This study reports the first case of a subtrochanteric stress fracture of the femur after total knee arthroplasty. A 61-year-old obese woman was treated by right total knee arthroplasty for osteoarthrosis. Four months after the surgery and 1 month after full weight bearing, she complained of tenderness in the right groin and proximal thigh. There was no history of trauma. Radiographs of the right hip and of the right proximal femur were interpreted as unremarkable, and nonsteroid anti-inflammatory drugs were administered. Three months later, she had a sudden onset of increased pain with instab
