RATIONALE: Todd paralysis (a stroke-like presentation in some patients with epilepsy) caused by limbic encephalitis (LE) is not easily distinguished from acute ischemic stroke by clinicians in the emergency room.
PATIENT CONCERNS: We report a contactin-associated protein-like 2-antibody (CASPR2-Ab)-positive patient who presented with atypical LE.
DIAGNOSES: CASPR2-Ab-positive LE was the presumed diagnosis. Re-evaluation of cerebrospinal fluid (CSF) samples revealed autoantibodies targeting CASPR2 at an immunoglobulin G titer of 1:1. The clinical presentation of subacute onset seizures, abnor…

Autoimmune limbic encephalitis is part of CASPR 2 antibody-associated disease. A man with this rare disorder and a very high antibody titre had a unique history of laboratory exposure to the antigen. Together with earlier observations this case calls for caution in laboratory handling of nerve tissue.

We report a case of anti-voltage-gated potassium channel (VGKC) limbic encephalitis in a 47-year-old man presenting with a 2-year history of psychiatric features. The patient had cognitive impairment, slurred speech, and a mildly unsteady gait but no other neurological deficits or seizures. Results of blood, urine, and cerebrospinal fluid tests and magnetic resonance imaging of the brain were normal. However, electroencephalography showed an epileptogenic focus in the bilateral temporal regions with mild to moderate diffuse encephalopathy. Autoimmune panel results confirmed the diagnosis of an…

We report a case of moyamoya disease (MMD), which developed after non-herpetic acute limbic encephalitis (NHALE) associated with anti-leucine-rich glioma-inactivated 1 (LGI1) antibody. The patient's mother had a history of MMD. No vascular lesions were identified at the time of the NHALE. Nine years later, the patient visited our hospital due to memory disturbances and repeated transient ischemic attacks affecting the right limb. Diffusion-weighted magnetic resonance imaging revealed scattered areas of signal hyperintensity, and the patient was ultimately diagnosed with MMD based on angiograph…

We present a 66-year-old woman with 2 months of visual hallucinations, unintentional weight loss, and short-term memory decline, whose clinical presentation and EEG supported a diagnosis of limbic encephalitis. Subsequent evaluation for a paraneoplastic etiology revealed a renal mass, which was resected and identified as clear cell renal carcinoma. The patient's clinical condition improved after resection of the mass. When patients present with incongruous subacute neuropsychiatric symptoms, clinicians should be mindful of paraneoplastic neurological disorders, as early diagnosis and treatment…

A form of limbic encephalitis associated with antibodies against the N-methyl-D-aspartate receptor (NMDAR) was discovered in 2007. It is often a multistage illness that progresses from psychosis, memory deficits, seizures into a state of unresponsiveness with catatonic features, abnormal movements, autonomic, and respiratory instability. We present two cases of anti-NMDAR encephalitis to highlight the cardiac complications and their management.

BACKGROUND: γ-Aminobutyric acid-B receptor antibodies (GABA(B)R-ab) were recently described in 15 patients with limbic encephalitis (LE), associated with small-cell lung cancer (SCLC) or with concurrent glutamic acid decarboxylase (GAD) antibodies. We analyzed the frequency of GABA(B)R-ab in 147 patients with LE or neurologic syndromes associated with GAD-ab.
METHODS: We examined the presence of GABA(B)R-ab in 70 patients with LE (33 paraneoplastic with onconeural antibodies, 18 paraneoplastic without onconeural antibodies [5 with Gad-ab], and 19 idiopathic with either GAD-ab [5 patients] or …

BACKGROUND: Thyroid antibodies (Thy-Abs) are frequently detected in various autoimmune disorders in coexistence with other systemic autoantibodies. In association with an encephalopathy, they are often taken as evidence of Hashimoto's encephalitis (HE). However, the presence of Thy-Abs in a cohort of limbic encephalitis (LE) patients and their association with anti-neuronal autoimmunity has not been explored.
PATIENTS AND METHODS: We investigated thyroid and anti-neuronal antibodies in the sera of 24 LE patients without identified tumors by cell-based assay and radioimmunoassay and evaluated …

Paraneoplastic limbic encephalitis (PLE) is a rare disorder characterized by personality changes, irritability, depression, seizures, memory loss and sometimes dementia. The diagnosis is difficult because clinical markers are often lacking, and symptoms usually precede the diagnosis of cancer or mimic other complications. The frequency of antineuronal antibodies in patients with PLE has not been investigated. We examined the neurological symptoms and the causal tumours in 50 patients with PLE to determine the utility of paraneoplastic antibodies and other tests. The diagnosis of PLE required n…

BACKGROUND/AIM: Immune checkpoint inhibitors (ICIs), including nivolumab, have markedly improved gastric cancer (GC) outcomes. However, immune-related adverse events involving the central nervous system are rare and potentially life-threatening. Limbic encephalitis is an exceptionally uncommon manifestation, and its clinical course remains poorly understood. We conducted a literature review of ICI-induced encephalitis specifically in patients with gastrointestinal cancers to clarify its clinical characteristics and management.
CASE REPORT: We describe the case of an 83-year-old man with unres…

A small proportion of small cell lung cancer (SCLC) presents as paraneoplastic limbic encephalitis (PLE). The most implicated antibody is anti-Hu, but others such as anti-ampiphysin, anti-CRMP5 and anti-SOX1 have been frequently detected. Autoimmune encephalitis mediated by antibodies against α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor (AMPAR) is a rare but important neurological complication of SCLC. The incidence of SCLC presenting with anti-AMPAR antibody PLE is extremely low. Our case illustrates a rare presentation of SCLC with an intention to highlight the importance of…

Paraneoplastic neurological syndromes are rare immune-mediated disorders that may develop independently of tumor stage or metastatic spread. Limbic encephalitis is an uncommon manifestation, and its occurrence in association with head and neck cancers is exceptionally rare. We describe a 42-year-old man who presented with subacute ataxia, tremor, dysarthria, and altered consciousness. Brain magnetic resonance imaging revealed hyperintensities in the left hippocampus, parahippocampal gyrus, and amygdala, while cerebrospinal fluid analysis demonstrated lymphocytic pleocytosis, elevated protein l…

INTRODUCTION: Anti-LGI1 encephalitis is associated with disruptions in large-scale brain network functionality. Although hippocampal atrophy has been structurally characterized, the morphometric patterns of subcortical structures and their surface deformations remain poorly understood. We therefore investigated the shape abnormalities of subcortical structures and their morphological correlations in patients with anti-LGI1 encephalitis.
METHODS: This study included 31 patients diagnosed with anti-LGI1 encephalitis and 31 group-matched healthy controls. The mesh-based shape method was performe

Varicella zoster virus (VZV) causes chickenpox as the primary infection and then becomes latent in the cranial and spinal ganglia. VZV can reactivate with aging, immunosuppression, stress, and other factors. In our series of 15 patients with herpes zoster (HZ) central nervous system complications (8 males and 7 females, ages 41-86 years), we identified several types of complications: acute encephalitis, vasculitis, meningitis, and cranial nerve palsies, with acute limbic encephalitis (ALE) (n = 5) being particularly noteworthy. The elderly patient treated initially showed skin rash around the …

BACKGROUND: A broad range of inflammatory and neurodegenerative conditions manifest with progressive cognitive and behavioural changes. A diagnostic challenge is the differentiation of limbic encephalitis (LE) from Creutzfeldt-Jakob disease (CJD). LE and CJD are distinct neurological conditions with distinct variations in their clinical course, with overlapping clinical presentations. LE can be subdivided into autoimmune paraneoplastic and non-paraneoplastic subtypes, under the umbrella of autoimmune LE. CJD is the most prevalent form of human prion disease and the subtype sporadic CJD (sCJD) …
