BACKGROUND: Platelet to lymphocyte ratio (PLR) has been regarded as a parameter in assessing systemic inflammation. Polymyositis (PM) is a rare connective tissue diseases characterized by chronic muscle inflammation, which led to a hypothesis with respect to PLR in PM patients. Therefore, it is reasonable to investigate the relationship between PLR and PM.
METHODS: The present study included 46 newly diagnosed PM patients and 128 healthy subjects as controls. The muscle weakness of PM patients was measured using the manual muscle test (MMT) to evaluate disease activity in patients with PM.

A 38-year-old man who died from systemic Behçet's disease had previously suffered from severe, recurrent bilateral retinal vasculitis, and anterior uveitis for 10 years. Immunopathological examination of the eyes postmortem revealed marked hyaline thickening of the retinal and optic nerve vessels. The vessels had an intramural and perivascular infiltrate of T lymphocytes which stained positively for CD4 and IL2 receptor surface markers. Small numbers of cells in the optic nerve head, retinal vascular endothelium, and retinal pigment epithelium were HLA DR positive.

Conversion of androgens (aromatization) to estrogens in lymphocytes of cancer patients and healthy subjects in vitro was discovered. Aromatase activity in lymphocytes was comparable to that of fat and breast tumor tissues. It was completely suppressed in one out of two breast cancer patients receiving orimeten (aminoglutethimide). The activity in large bowel cancer was lower than in adenoma patients; however, the two groups were not balanced for age.

Proportions of T lymphocyte subsets and activated cells bearing receptors for transferrin, Il-2 and HLA-DR antigens were studied in 15 asymptomatic HBsAg carriers and 10 subjects with negative markers of hepatitis B virus infection. The effect of dipyridamol on T-lymphocyte activation was investigated. The drug did not have an irreversible toxic effect on T lymphocytes. Dipyridamol inhibited both PHA- and mumps-stimulated proliferation and the proliferative response in mixed lymphocyte cultures. Studies showed a significant decrease in the macrophage neutral protease and lymphocyte acid protease activities in patients with multiple sclerosis in remission, a significantly decreased neutral protease activity in macrophages in patients with myasthenia gravis and a significantly decreased acid protease activity in macrophages and lymphocytes in patients with polymyositis. No remarkable abnormalities were found in patients with myotonic dystrophy. These results suggest that multiple sclerosis, myasthenia gravis and polymyositis have an abnormality in immunological function.

It was found that lymphocytes secured from lymphoid organs or syphilitic rabbits, when cultured with ultrasonicate of T. pallidum (Tp-S) or phytohemagglutinin-P (PHA-P), released to the supernatant fluid a substance (ATL-anti-treponemal lymphotoxin) which exerted a direct killing effect on T. pallidum. Splenic lymphocytes were found much more active than lymphocytes isolated from popliteal and mesenteric lymph nodes, and the peripheral blood. The effect of some factors on release of the ATL by sensitized lymphocytes was established.
