The literature documenting the phototoxic effect of relatively low intensity light on the retina and the suggestions by several authors that this might influence the development of cystoid macular edema in the aphakic and pseudophakic patient is reviewed. In particular, the possibility that the operating microscope may be a factor has been emphasized. A study is presented, designed to investigate the possibility that the operating microscope is a factor important in the development of cystoid macular edema. No correlation was found. The need for further investigation into other phototoxic effe…

In patients treated with oral indomethacin for four weeks after intracapsular cataract extraction, five of 57 eyes (9%) had cystoid macular edema four to six weeks postoperatively as shown by fluorescein angiography. In a control group not treated with indomethacin, 15 of 34 eyes (44%) had cystoid mascular edema. Oral indomethacin, therefore, caused a statistically significant decrease in the incidence of aphakic cystoid macular edema.

We report a rare case of a patient who developed age-related macular degeneration (AMD) in one eye during treatment of diabetic macular edema (DME) in both eyes. A 70-year-old man was followed for diabetic retinopathy for 23 years. At his initial visit in 200X, his decimal visual acuity was 1.0 in both eyes. He underwent panretinal photocoagulation for pre-proliferative diabetic retinopathy (PPDR) in both eyes. Optical coherence tomography (OCT), which was done nine years after the initial visit, revealed a small retinal pigment epithelial detachment (PED) in the left eye. Sixteen years after …

Diabetic macular edema (DME; diabetic macular oedma in some countries), which can be associated with diabetic retinopathy, is characterized by macular thickening and accumulation of pathologic fluid in the portion of the retina responsible for central vision. DME is a leading cause of vision loss in individuals with diabetes and can severely impact their daily activities and quality of life. Standard DME treatment involves intravitreal injections of anti-vascular endothelial growth factor (VEGF) agents, and some patients receive laser photocoagulation and/or intravitreal corticosteroid injecti…

Glucocorticoids have been used for decades in the treatment of ocular disorders via topical, periocular, and more recently intravitreal routes. However, their exact mechanisms of action on ocular tissues remain imperfectly understood. Fortunately, two recently approved intravitreal sustained-release drug delivery systems have opened new perspectives for these very potent drugs. To date, among other retinal conditions, their label includes diabetic macular edema, for which a long-lasting therapeutic effect has been demonstrated both morphologically and functionally in several randomized clinica…

BACKGROUND: Potent antiretroviral therapy can lead to improved immunity in patients with acquired immune deficiency syndrome. However, ocular inflammation can occur in predisposed individuals. This inflammation is referred to as immune-recovery uveitis. Cystoid macular edema (CME) is a complication that can result from this inflammation and is emerging as a major cause of visual loss in human immunodeficiency virus (HIV)-infected patients.
CASE REPORT: A 52-year-old man presented with complaints of intermittent decreased vision that had coincided with the initiation of antiviral treatment. He…

Cystoid macular edema (CME) is a classical complication of ocular inflammation. This syndrome was already described by Irvine in 1953 but the pathogenesis of this condition remains unclear. Cystoid macular edema can result either from a rupture of the inner or from the outer blood ocular barrier. Clinical CME that is responsible for a low visual acuity must be differentiated from angiographic CME that can be present even without any decrease in visual acuity. Fluid progressively accumulates into the outer plexiform layer of the retina and pools into cystic spaces. Fluid accumulation can now be…

Medical treatment of cystoid macular edema (CME) with carbonic anhydrase inhibitors has been known for over a decade. Initial observations were based on experimental data which suggested that acetazolamide can increase fluid absorption across the retinal pigment epithelium. Carbonic anhydrase inhibitors (CAI) have also been shown to have other direct effects both on retinal and retinal pigment epithelial cell function by inducing an acidification of the subretinal space, a decrease of the standing potential as well as an increase in retinal adhesiveness. It is thought that acidification of the…

Eighty percent of 30 eyes with chronic cystoid macular edema (CME) after cataract extraction achieved improved visual acuity of three or more lines when treated with 1% indomethacin eye drops. Of these patients 53% demonstrated an "on/off" phenomenon induced by the initiation and cessation of treatment documented by visual acuity measurements and fluorescein angiography. This "on/off" phenomenon suggests that there is a direct relationship between the use of 1% indomethacin eye drops and the resolution of chronic CME after cataract extraction. Previous studies on the treatment of CME with indo…

After reviewing the definition, incidence and main etiologic factors of cystoid macular edema (CME) the authors analyze the structural and physiologic characteristics of the macular area, including the blood-retinal barriers, and the basic concepts of retinal edema. Personal observations on three different groups of diseases associated with CME, retinitis pigmentosa, pars planitis and aphakia, are presented. CME evaluation was made by slit-lamp microscopy, fluorescein angiography and vitreous fluorophotometry. In the situations examined, the main factors involved in CME formation appear to be …
