The study was performed to ascertain the value of potassium magnesium citrate, magnesium citrate, and potassium citrate in overcoming thiazide-induced hypokalemia and magnesium loss. Sixty-two healthy subjects were first administered hydrochlorothiazide, 50 mg/d. After 3 weeks of thiazide treatment (or earlier for potassium level </=3.5 mEq/L), they were randomized to receive one of three drugs while continuing to receive thiazide: potassium magnesium citrate (49 mEq of potassium, 24.5 mEq of magnesium), magnesium citrate (24.5 mEq/d of magnesium), or potassium citrate (49 mEq/d of potassium).…

By a variety of mechanisms, magnesium functions both intracellularly and extracellularly to minimize the cytoplasmic free calcium level, [Ca2+]i. This may be the chief reason why correction of magnesium deficiency, or induction of hypermagnesemia by parenteral infusion, exerts antihypertensive, anti-atherosclerotic, anti-arrhythmic and antithrombotic effects. Although the amino acid taurine can increase systolic calcium transients in cardiac cells (and thus has positive inotropic activity), it has other actions which tend to reduce [Ca2+]i. Indeed, in animal or clinical studies, taurine lowers…

Magnesium deficit in man is very difficult to detect and even more difficult to study. However, a chance observation led to the investigation of cardiac transplant recipients as an in vivo model of drug-induced magnesium depletion. Clinical circumstances were such that the question of the correlation of serum and tissue magnesium levels might be addressed. Using the same model, investigation of myocardial calcium levels was undertaken. Significant myocardial magnesium depletion accompanied persistent hypomagnesaemia in over 50 per cent of cardiac transplant recipients. Repletion of myocardial …

Thiazide can cause magnesium depletion, which may exaggerate renal potassium wasting and hypokalemia. The purpose of this double-blind, randomized trial was to compare the metabolic effects of potassium-magnesium-citrate (K-Mg-citrate) and potassium chloride (KCl) during long-term treatment with thiazide. Twenty-two normal volunteers received hydrochlorothiazide 50 mg/d. Ten subjects concurrently took K-Mg-citrate (42 mEq K/d and 21 mEq Mg/d), and 12 subjects were given KCl 42 mEq/d. Serum potassium concentration remained unchanged during K-Mg-citrate supplementation, with a change from baseli…

OBJECTIVE: The purpose of this study is to estimate the relations between ionized and total Mg levels during MgSO4 administration in patients with preterm labor and preeclampsia.
METHODS: Forty-three pregnant patients who were candidates for MgSO4 were studied (preterm labor, 27; preeclampsia, 16). The administration method was intravenous injection of MgSO4 4 g over 30 min followed by 1-2 g/h. Ionized Mg was measured by the selective ion electrode method at bedside, and compared it with total Mg levels.
RESULTS: Significant correlation was existed between levels of ionized and total Mg thro

The objective of this study was to determine the serum levels of magnesium associated with various intravenous infusion rates of magnesium sulfate (MgSO4) in high-order multiple gestations. A retrospective review of 12 triplet and 4 quadruplet pregnancies cared for in Phoenix from August 1988 to January 1992 was performed. The mothers received MgSO4 for tocolysis of preterm labor. Serum magnesium levels were drawn to help manage patients with symptoms of an excessive magnesium level or an inadequate tocolytic effect. The results are presented for triplets and quadruplets with and without pregn…

The results of an open randomized study on magnesium status and parenteral magnesium sulphate therapy in acute aluminium phosphide intoxication are presented. The study was conducted on 105 patients divided into two group (I & II). Patients of Group I did not receive parenteral magnesium and acted as blank. Magnesium levels were monitored every 6 h for 24 h. Patients of group II received magnesium sulphate therapy. It was administered as 1.0 g (8.1 mEq or 4.05 mmol) magnesium sulphate dissolved in 100 ml of 5 per cent dextrose intravenously as a bolus dose followed by 1.0 g every hour for thre…

OBJECTIVES: To compare the effects of magnesium repletion by a foods-alone approach or by magnesium supplementation on urinary magnesium and citrate excretion in patients with urine magnesium <70 mg/day.
METHODS: We reviewed medical records of patients in our stone prevention practice who were advised to start a magnesium supplement (Sup), 250-500 mg/day, or increase dietary magnesium consumption. We included adults with 24-hour urine magnesium (UMg) <70 mg, those who received magnesium recommendations (corroborated by the dietitian's clinical notes), and those with a follow-up 24-hour urine …

Magnesium (Mg) is an essential micronutrient that participates in various enzymatic reactions that regulate vital biological functions. The main aim was to assess the Mg status and its association with nutritional indicators in seventeen cystic fibrosis (CF) patients. The serum Mg and calcium (Ca) levels were determined using standardized methods and the dietary Mg intake by prospective 72 h dietary surveys. The mean serum Ca (2.45 mmol/L) and Mg (0.82 mmol/L) had normal levels, and the mean dietary intake of the Ca (127% DRI: Dietary Reference Intake) and Mg (125% DRI) were high. No patients …

AIM: To determine whether hypermagnesemia recently reported in adult patients possibly develops in children with functional constipation taking daily magnesium oxide.
METHODS: We enrolled 120 patients (57 male and 63 female) aged 1-14 years old (median: 4.7 years) with functional constipation from 13 hospitals and two private clinics. All patients fulfilled the Rome III criteria for functional constipation and were treated with daily oral magnesium oxide for at least 1 mo. The median treatment dose was 600 (500-800) mg/d. Patients were assessed by an interview and laboratory examination to de…

A genetic control of blood magnesium (Mg) levels has been suggested. To investigate the mechanisms and the biologic significance of this genetic regulation, a mouse model, ie, mice selected for low magnesium level (MGL) and high magnesium level (MGH), was developed. The purpose of this study was to explore the Mg status and Mg metabolism in female MGL and MGH mice. We observed that MGL mice had reduced total and ionized plasma Mg, lower erythrocyte Mg, lower tibia, and kidney Mg levels. In contrast, total urinary Mg and (25)Mg levels were significantly higher in MGL mice. MGL mice had smaller …

BACKGROUND: Almost 99% of the body magnesium is inside cells. The concentration of intracellular ionized magnesium (iMg) is physiologically relevant. iMg in erythrocytes is a new parameter that can help to establish reliable information on the functional magnesium status.
METHODS: iMg concentration in erythrocytes and serum was measured by ion-selective electrode, in clinical analyzer Microlyte (KONE). Total magnesium (tMg) concentration was measured by atomic absorption spectrometry (AAS). Albumin and total protein concentration were measured colorimetrically.
RESULTS: In critically ill pos…

Magnesium is the second most abundant cation in intracellular fluid and is an essential electrolyte. It has several critically important roles in the body, namely as a cofactor in numerous enzyme systems, and is involved in phosphate transfer, muscle contractility and neuronal transmission. The physiologic role, homeostasis, causes and clinical manifestations of hypo and hypermagnesemia and their therapy are briefly reviewed. Magnesium treatment is emerging as an important adjunct in the management of a few conditions: prevention and control of seizures in eclampsia, cardiovascular diseases, d…
