Complications of mandibular fractures in an urban teaching center.

PURPOSE: In this study, we compared the complications associated with open and closed treatment of mandibular fractures in an urban teaching center over a 4-year period. PATIENTS AND METHODS: We conducted a retrospective review of mandibular fracture morbidity associated with treatment by the oral and maxillofacial surgery service between 1996 and 2000. A total of 721 fractures were recorded, with 594 fractures available for review. Perioperative and postoperative complications were assessed by reviewing patient charts, operative reports, and radiographs. Complications were classified by loca

The effect on facial growth of pediatric mandibular fractures.

The incidence of facial fractures in the pediatric population is between 1.4% and 15% of all maxillofacial traumas. Forty-one percent of pediatric facial fractures involve the mandible. No study has commented on the incidence of mandibular fractures that go on to develop growth disturbances leading to asymmetry and malocclusion. A retrospective chart review was carried out that identified and followed 88 children who sustained mandibular fractures and presented to The Hospital for Sick Children in Toronto during the 13-year period from 1980 to 1993. Patient follow-up ranged from 2 to 15 years,

Treatment of mandibular angle fractures using one noncompression miniplate.

PURPOSE: This study evaluated the results in patients treated for fractures of the mandibular angle with a single miniplate. PATIENTS AND METHODS: Eighty-one patients with fractures of the mandibular angle were treated by open reduction and internal fixation using one noncompression miniplate with 2.0-mm self-threading screws placed through a transoral incision. No patient was placed into postsurgical maxillomandibular fixation. They were prospectively studied for complications. RESULTS: Thirteen patients with angle fractures (16%) experienced complications requiring secondary surgical inter

Open reduction-fixation of mandibular subcondylar fractures. A review.

From 1973 to 1990, 392 mandibular subcondylar fractures were treated at the University of California, Davis, by the Otolaryngology Department. Of these, 17% were handled by open reduction and internal fixation. Twenty-one patients from this group were located for follow-up at an average interval of 64 months. Retrospective review shows the operation to be safe, with few complications and no permanent sequelae. Patient examination often revealed abnormalities of occlusion and mandibular function; however, these objective findings did not correlate well with patients' relative lack of subjective

[Midfacial and mandibular body fractures in children and adolescents].

1423 patients with maxillo-facial trauma were admitted for hospital treatment from 1980-1986. Midfacial and/or mandibular corpus fractures were present in 87 juvenile cases. The follow-up study of 64 patients of this group revealed some interesting points: 1. Fractures of the juvenile skull must be reduced exactly. There will be no "smoothing of edges" during the later growth period. 2. Teeth which had been left in the fracture line showed no disturbance during their eruption. 3. An enlargement of the zygomatic-alveolar crest was a frequent observation in zygoma fractures. 4. There is no need

[Anatomicoclinical types of mandibular fractures in children (author's transl)].

The author describes the particular type of fragility zones found in children's mandibles, which vary during growth, and discusses the anatomicoclinical types of fractures observed from the earliest age (symphysial disjunctions) until the end of mixed dentition. Emphasis is placed on one particular type, described in a previous communication: the paramedian form, which can be explained by the atomical position of certain dental germs, as well as the importance of reconstitution of the periosteum in displaced sub-condylar fractures.

Prevalence of Temporal Bone Fractures in Patients with Mandibular Fractures Using Multidetector-Row CT.

PURPOSE: Temporal bone fracture after mandibular trauma is thought to be rare, and its prevalence has not been reported in the literature. The purpose of this study was to investigate the prevalence of temporal bone fractures in patients with mandibular fractures and the relationship between temporal bone fractures and the mandibular fracture location using multidetector-row computed tomography (MDCT). METHODS: A prospective study was performed in 201 patients with mandibular fractures who underwent 64-MDCT scans. The mandibular fracture locations were classified as median, paramedian, angle,

[Current diagnosis and therapy of anterior mandibular fracture associated with condyle fractures].

Anterior mandibular fracture, which includes symphyseal and parasymphyseal mandibular fractures, is one of the most common types of fracture in clinical work, and is usually associated with condyle fracture. This type of fracture predisposes the patients to facial widening, malocclusion, and ankylosis when not properly and timely treated because of the influence of its anatomic structure. This article reviews the characteristics, complication, diagnosis, and therapy of this type of fracture.

Impacted mandibular third molars and their influence on mandibular angle and condyle fractures--a retrospective study.

BACKGROUND: Previous retrospective analyses prove that impacted mandibular third molars (M3s) increase the risk of angle fractures and decrease the risk of concomitant fractures to the condyle. STUDY DESIGN: A retrospective cohort was designed for patients reported to the Department of Oral and Maxillofacial Surgery from January 2011 till June 2013. The study variables are presence or absence of third molar, if it is present, their position, classified using the Pell and Gregory system; angulation, classified using Shiller's method. The outcome variables were angle and condyle fractures. MAT

Treatment of mandibular symphyseal fracture combined with dislocated intracapsular condylar fractures.

OBJECTIVES: To evaluate the treatment methods of mandibular symphyseal fracture combined with dislocated intracapsular condylar fractures (MSF&DICF) and to compare the effect of different treatment methods of condylar fractures. METHODS: Twenty-eight patients with MSF&DICF were included in this study. Twenty-two sites were treated by open reduction, and all the medial condylar fragments were fixed with titanium screws; whereas the other 22 sites underwent close treatment. The surgical effect between these 2 groups was compared based on clinical examination and radiographic examination results

Mandibular fracture complications associated with the third molar lying in the fracture line.

A retrospective study of 622 patients (551 men and 71 women, age range 14-63 years) with 650 mandibular fractures crossing the third molar was conducted. In 183 patients (193 fracture lines) the wisdom tooth was retained or partially erupted and in 439 patients (457 fractures) the wisdom tooth in the fracture line was normally situated in the dentition. The analysis showed no statistically significant difference in the complication frequency associated with the extraction of retained or erupted wisdom teeth (12.50% and 8.37%, respectively). In treatment of fractures with retention of the wisdo

Assessment of pediatric mandibular fracture management in France.

Mandibular fractures in children present management challenges due to pediatric-specific factors such as mandibular growth and the presence of dental germs. There are few surgeons specifically trained to treat these fractures, and there are no national or international guidelines. Management depends on several factors, including the child's age and the location of the fracture. Treatment options include functional treatment, closed reduction with or without immobilization, and surgery. The aim of this study was to assess the current management strategies for pediatric mandibular fractures in F

Secondary mandibular osteomyelitis complicated with a pathological fracture.

INTRODUCTION AND IMPORTANCE: Pathological mandibular fractures, defined as fractures that occur in regions where bone has been weakened by an underlying pathological process, are rare, accounting for fewer than 2 % of all fractures of the mandible. Mandibular pathological fractures can have several aetiologies including osteomyelitis, osteoradionecrosis (ORN), surgical interventions, bisphosphonate-related osteochemonecrosis of the jaw and tumoral lesions. CASE PRESENTATION: We present a case of a 13-year-old male patient following a tooth infection with a right facial swelling and limited mo

An approach for simultaneous reduction and fixation of mandibular fractures.

This article presents a new approach for the design of a flexible V-shaped miniplate for mandibular fractures, which combines simultaneous fracture reduction and fixation. A Computerized Tomography (CT) based finite element model was developed to assess the reliability of this design. Muscle and mastication forces were included to replicate post-surgery loading. The V-plate is compared with a standard, linear miniplate, typically used for mandibular fixation. The results indicate that the proposed design can support the fracture while inducing limited fracture displacement, in addition to redu

Arterial Vascularization of the Mandibular Condyle and Fractures of the Condyle.

BACKGROUND: Despite substantial displacements, fractures of the mandibular condyle rarely lead to necrosis. This illustrates the negligible role of the inferior alveolar artery in intraosseous supply to the condyle, and led to this systematization of its arterial vascularization. METHODS: Forty-two temporomandibular joints from nonembalmed cadaveric specimens were studied following injection of latex (n = 32) or India ink (n = 10). RESULTS: The intraosseous branches of the inferior alveolar artery that lead to the condyle were inconstant and often rudimentary. In this study, the arteries tha

[Insufficient diagnostics in a patient with a fracture of the mandibular collum].

A 13-year-old boy was presented to oral and maxillofacial surgeon with chin and preauricular pain after afallfrom his skateboard. A panoramic radiograph did not show any indication of a fracture of the mandibular collum. Two weeks later, the boy was referred by his orthodontist to the department of Oral and Maxillofacial Surgery of a medical centre in connection with a relapse ofa class III malocclusion despite a recently completed treatment of that problem. A computertomogram displayed a fracture of the mandibular collum. In case afracture of the mandibular collum is suspected, imaging from 2
