Maternal age and the likelihood of a maternal request for cesarean delivery: a 5-year population-based study.

OBJECTIVE: The purpose of this study was to examine associations between maternal age and maternal request cesarean deliveries. STUDY DESIGN: Five-year population-based data from Taiwan (1997-2001) that covered 904,657 singleton deliveries without a clinical indication for cesarean delivery that were judged by the attending physician were subjected to multiple logistic regression, year-wise, to examine the association of maternal age with request cesarean delivery, adjusted for health care institutional characteristics. RESULTS: Request cesarean delivery rates steadily increased over the stu…

Maternal childhood adversity accelerates epigenetic aging of children.

OBJECTIVE: Although early adversity is strongly related to lifelong health disparities, it is unclear how adversity might confer risk across generations. To investigate, we tested the hypothesis that mothers' childhood adversity was associated with their epigenetic aging and that of their children and examined whether associations differed for Black and White mothers. METHOD: Dyads (N = 215) of mothers (52% White, 48% Black, Mage = 39.2, SD = 1.1) and children (N = 215, 55% female, Mage = 8.3, SD = 4.0, range 2-17) provided saliva samples to assay the Horvath clock and pace of aging calculate

The impact of advanced maternal age on the outcome of twin pregnancies.

PURPOSE: To assess the effect of advanced maternal age on the obstetrics and neonatal outcome of twin pregnancies. METHODS: A retrospective study of 716 dichorionic-diamniotic twin pregnancies delivered at our institute. The study population was divided into two groups: women aged 35-39 years (group A, n = 142) and women aged ≥ 40 years (Group B, n = 48). The control group consisted of women younger than 35 years (group C, n = 516). RESULTS: The rate of cesarean section (CS) was significantly higher among women older than 35 years compared to the control group (A 76.8% and B 87.5% vs C 65.7%…

Maternal perceptions of assisting preschool-aged children with toothbrushing.

BACKGROUND: Parent-led toothbrushing with fluoride toothpaste is part of an evidence-based strategy to prevent caries in children. There is a gap in the literature regarding perceptions of how and when to assist a child with toothbrushing from the maternal perspective. METHODS: A qualitative cross-sectional study was conducted with participants in North and North Central Appalachia to examine maternal perceptions of when and how to assist with toothbrushing. From 2018 through 2022, 301 mothers of children aged 3 through 5 years volunteered to participate in semistructured interviews from a mo

Impact of maternal age on the incidence of obstetrical complications in Japan.

AIM: To clarify the effect of maternal age on obstetrical complications through a cohort and case-cohort study. METHODS: We studied 242 715 births at 125 centers of a perinatal network in Japan from 2001 through 2005 as a base cohort. Women with single pregnancies who delivered after 22 weeks of gestation were included in the study. Six classes of maternal age were selected: <20; 20-24; 25-29; 30-34; 35-39; and ≥40 years. The cohort study was used to investigate whether age is related to obstetrical complications. By random selection 3749 births were determined as a subcohort. Risk ratio (RR)…

The impact of extremes of maternal age on maternal and neonatal pregnancy outcomes in women with pregestational diabetes mellitus.

Background:. Diabetes mellitus (DM) during pregnancy increases the risk for many complications such as preeclampsia and cesarean section. Teen (age <20) and advanced maternal age (AMA, age ≥35) pregnancies are both at increased risk for maternal and neonatal morbidity and mortality. Understanding the risks and complications unique to teen and AMA women with pregestational DM is imperative to providing care and improving outcomes.Objective:. This study sought to determine how maternal and neonatal outcomes are affected by pregestational DM in teen and AMA pregnancies.Study design:. This is a re…

Maternal and psychosocial antecedents of anxiety and depression in extremely low gestational age newborns at age 15 years.

OBJECTIVES: The prevalence of many psychiatric symptoms, including anxiety and depression, is higher in individuals born extremely preterm (EP) than in term-born individuals during childhood and adolescence. In this prospective study of adolescents born EP, we examined associations between early-life risk factors (prenatal maternal health conditions, socioeconomic and social factors) and anxiety and depression at 15 years of age. METHODS: We included 682 participants (53.2% White, 57.8% male) who were born <28 weeks gestation. Data on demographic factors, maternal health conditions and socioe

Combined effects of increasing maternal age and nulliparity on hypertensive disorders of pregnancy and small for gestational age.

BACKGROUND: The mean age of women delivering for the first time is increasing, and this combination could lead to an increased risk of perinatal complications. OBJECTIVES: The objective was to evaluate the potential combined effects of nulliparity and increasing maternal age on small for gestational age (SGA < 10th percentile) and hypertensive disorders of pregnancy (HDP). STUDY DESIGN: A population-based cohort study was conducted using data routinely collected on all births in 11 hospitals in the Burgundy perinatal network between 2007 and 2016. Pregnant women with singleton deliveries age…

Adverse maternal and neonatal outcomes among singleton pregnancies in women of very advanced maternal age: a retrospective cohort study.

BACKGROUND: There is an increasing prevalence of women who tend to delay childbirth until a very advanced age. However, there is sparse data regarding very advanced maternal age (vAMA) and the interplay between vAMA and assisted reproductive technology (ART) on adverse perinatal outcomes. The study aimed to assess the risk of adverse maternal and neonatal outcomes of vAMA women (≥43 years), and to investigate the effect of maternal age on adverse maternal and neonatal outcomes in ART pregnancies. METHODS: Data was obtained from a population-based retrospective cohort of women who delivered in…

The effect of advanced maternal age on maternal and neonatal outcomes of placenta previa: A register-based cohort study.

OBJECTIVES: Advanced maternal age (AMA) at the time of delivery generally worsens obstetric outcomes, but its effects on specific pregnancy problems, such as placenta previa, have not been adequately assessed. Therefore, the objective of the study was to explore the effect of AMA on adverse maternal and neonatal outcomes among pregnancies complicated by placenta previa. STUDY DESIGN: The study was a register-based cohort study using data of three Finnish health registries, including information of 283 324 women and their newborns. Separate multivariable logistic regression modeling was perfor…

[Second trimester maternal serum screening for Down's syndrome in women of advanced maternal age: a multi-center prospective study].

OBJECTIVE: To evaluate the performance characteristics of the second trimester double test for the detection of fetal Down's syndrome (DS) in women of advanced maternal age (AMA). METHODS: We undertook a prospective nation-wide multi-centered study and chose alpha-fetoprotein (AFP) and free beta-subunit of human chorionic gonadotrophin (free beta-hCG) as the serum markers. Between May 2004 and September 2006, 12 centers participated in the collection and analysis of maternal serum AFP and free beta-hCG. Patients with an increased risk of DS (> or = 1/270) were offered genetic amniocentesis. F…

Association of Prenatal Care Services, Maternal Morbidity, and Perinatal Mortality With the Advanced Maternal Age Cutoff of 35 Years.

IMPORTANCE: Maternal and perinatal mortality remain high in the US despite growing rates of prenatal services and spending, and little rigorous evidence exists regarding the impact of prenatal care intensity on pregnancy outcomes. Patients with an expected date of delivery just after their 35th birthday may receive more intensive care owing to the advanced maternal age (AMA) designation; whether this increase in prenatal care is associated with improvements in outcomes has not been explored. OBJECTIVE: To determine the association between the AMA designation and prenatal care services, severe…

Genomic risk profiling in advanced maternal age: a Tamil Nadu prenatal study.

BACKGROUND: Advanced maternal age (AMA; > = 35 years) is associated with increased fetal chromosomal risk and is an important indication for invasive prenatal diagnosis. This study evaluates karyotyping and chromosomal microarray analysis (CMA) findings among AMA pregnancies in Tamil Nadu, India. METHODS: In this prospective observational study, 2,200 pregnant women aged ≥35 years who underwent amniocentesis between July 2020 and June 2021 were enrolled. Conventional karyotyping was performed in all cases. CMA was performed as a reflex or complementary test when predefined clinical or cytogen…

Life satisfaction mediates advanced maternal age at childbirth and frailty across cohorts.

Advanced maternal age at childbirth (AMAC) is increasingly common, yet its long-term association with frailty remains unclear. Using three longitudinal cohorts (CHARLS, KLoSA, and HRS; N = 21,090), we examined this association via Cox proportional hazards models and mediation analysis. After multivariable adjustment, AMAC (≥35 years) was associated with increased frailty risk in the Chinese (hazard ratio [HR]: 1.21, 95% confidence interval [CI]: 1.03-1.42) and Korean (HR: 1.24, 95% CI: 1.02-1.50) cohorts, while AMAC (≥40 years) showed a similar direction in the United States cohort. Life satis…
