The scope of unmet maternal health needs in pediatric settings.

OBJECTIVE: Previous work has focused attention on the prevalence of specific maternal health problems known to affect children, such as smoking or depression. However, the cumulative health burden experienced by mothers and the potential for a practical pediatric health services response have not been examined. The aims of this study were to characterize: 1) the prevalence and cumulative burden of maternal health behaviors and conditions, 2) maternal access to a source of comprehensive adult primary care, and 3) maternal perceptions of a pediatric role in screening and referral. METHODS: We s…

Provision of maternal health services among Ohio community pharmacists practicing in maternity care deserts.

BACKGROUND: Over 2.3 million reproductive-age women in the United States live in a maternity care desert, a county without obstetric services or providers; over 3 million more reside in a county with low access to maternity care. The National Alliance of State Pharmacy Associations (NASPA) issued a toolkit outlining maternal health services community pharmacists are positioned to provide within their scope of practice. OBJECTIVES: The primary objective of this study was to determine whether community pharmacists practicing in maternity care deserts/low access areas in Ohio provide NASPA-recom…

The mistreatment of women during maternity care and its association with the maternal continuum of care in health facilities.

BACKGROUND: Mistreatment of childbearing women continues despite global attention to respectful care. In Ethiopia, although there have been reports of mistreatment of women during maternity care, the influence of this mistreatment on the continuum of maternity care remains unclear. In this paper, we report the prevalence of mistreatment of women from various dimensions, factors related to mistreatment and also its association to the continuum of maternity care in health facilities. METHODS: We conducted an institution-based cross-sectional survey among women who gave birth within three months

Threatened abortion and its consequences on maternal and fetal health.

Threatened abortion, defined as vaginal bleeding in the first trimester of pregnancy with a viable fetus, is a common obstetric complication. Therefore, it is of interest to investigate the maternal and fetal health outcomes associated with threatened abortion and to compare these outcomes with a control group of healthy pregnancies. Hence, a total of 240 pregnant women were included in this study, with 120 women diagnosed with threatened abortion and 120 healthy controls. Women with threatened abortion showed significantly higher rates of preterm delivery (25% vs. 12.5%, p<0.001), low birth w

The human periconceptional maternal-embryonic space in health and disease.

Pregnancy is established during the periconceptional period as a continuum beginning with blastocyst attachment to the endometrial epithelial surface followed by embryo invasion and placenta formation. This period sets the foundation for the child and mother's health during pregnancy. Emerging evidence indicates that prevention of downstream pathologies in both the embryo/newborn and pregnant mother may be possible at this stage. In this review, we discuss current advances in the periconceptional space, including the preimplantation human embryo and maternal endometrium. We also discuss the ro

Programming of cardiometabolic health: the role of maternal and fetal hyperinsulinaemia.

Obesity and gestational diabetes during pregnancy have multiple short- and long-term consequences for both mother and child. One common feat … to summarize and review the mechanisms by which maternal and fetal hyperinsulinaemia impact on (i) maternal health during pregnancy; (ii) placental and fetal development; (iii) offspring energy homeostasis and long-term cardiometabolic health; (iv) how interventions can alleviate these effects.

Maternal microbiome in preeclampsia pathophysiology and implications on offspring health.

Preeclampsia is a devastating hypertensive pregnancy disorder that currently affects 2%-8% of pregnancies worldwide. It is associated with maternal and fetal mortality and morbidity and adverse health outcomes both in mom and offspring beyond pregnancy. The pathophysiology is not completely understood, and there are no approved therapies to specifically treat for the disease, with only few therapies approved to manage symptoms. Recent advances suggest that aberrations in the composition of the microbiome may play a role in the pathogenesis of various diseases including preeclampsia. The matern

Bear bile use at the intersection of maternal health in Cambodia.

BACKGROUND: The consumption of bear gallbladders and bear bile in Southeast Asia is a persistent threat to bear populations. As part of a larger effort to understand the characteristics of bear part consumption in Cambodia, we uncovered a consumer base of women seeking treatment for post-partum and uterine ailments. METHODS: To better understand this aspect of consumption, we interviewed 122 women in seven different provinces in Cambodia, probing into the motivations and influences for using bear bile, as well as what types of ailments Cambodian women use it for. RESULTS: We found that it is

Intention to Engage in Maternal and Child Health Home Visiting.

Home visiting (HV) programs have the potential to improve maternal and child health. As federal and state initiatives expand the reach of these programs, understanding what factors enhance uptake and retention of the services becomes increasingly important. This qualitative study used a Reasoned Action Model (RAM) and a cultural lens to explore factors influencing the engagement of women with low-income in HV programs. We conducted 21 semi-structured interviews in both English and Spanish in a prenatal clinic in an urban public health department. The constructs most salient for participants we

Maternal education and the multidimensionality of child health outcomes in India.

Maternal education plays a central role in children's health, but there has been little research comparing the role of maternal education across health outcomes. It is important to distinguish child health outcomes from medical care outcomes. Health outcomes such as short-term morbidity and stunting are multifactorial in origin and determined by a range of factors not necessarily under a mother's control. Mother's education, given the necessary structural factors such as medical centres, is likely to lead to increased access to, and uptake of, medical services. Using data from the 2004-05 Indi

The significance of the placental genome and methylome in fetal and maternal health.

The placenta is a crucial organ for supporting a healthy pregnancy, and defective development or function of the placenta is implicated in a number of complications of pregnancy that affect both maternal and fetal health, including maternal preeclampsia, fetal growth restriction, and spontaneous preterm birth. In this review, we highlight the role of the placental genome in mediating fetal and maternal health by discussing the impact of a variety of genetic alterations, from large whole-chromosome aneuploidies to single-nucleotide variants, on placental development and function. We also discus…

Perinatal Maternal Depressive Symptoms as an Issue for Population Health.

The importance of maternal depression for child outcomes is well established, and impairments in psychosocial function and parenting are as severe in women with high subsyndromal levels of depressive symptoms as they are in women with clinical depression. The author conducted a systematic review that explored the association between maternal depressive symptoms and child neurodevelopmental outcomes, including in neuroimaging studies. The results strongly suggest that the influences of maternal depressive symptoms operate across a continuum to influence child outcomes, implying that maternal de

Maternal immunisation to improve the health of HIV-exposed infants.

HIV-exposed but uninfected (HEU) infants are at an increased risk of many infectious diseases that can contribute to the high mortality seen among HEU children. Maternal immunisation could be a promising strategy to reduce infections in HEU infants. However, very little research has explored the effect of HIV on the immunogenicity and effectiveness of vaccines given during pregnancy. We review the available evidence on maternal immunisation among women living with HIV (WLWH) for all vaccines recommended, considered, or being investigated for routine or risk-based use during pregnancy. Maternal mental health in families of children with spina bifida.

BACKGROUND: this study aimed to compare mental health of mothers of children with spina bifida with mothers of able-bodied controls. METHODS: eighty-one mothers of children with spina bifida aged 1-18 years completed the General Health Questionnaire-12 (GHQ-12) and Parenting Stress Index Short Form (PSI/SF). The controls were 69 mothers of children with acute, non-disabling illnesses. Each child's adaptive skills were assessed using the Vineland Adaptive Behaviour Scales (VABS). Logistic regression analysis was used to determine factors related to a high GHQ score (≥ 3) in all patients. RESU

Priorities for maternal and child health services: the consumer's perspective.

A convenience sample of 206 inner city department of public health patients were asked to rank the importance of health care services relative to other problems they face. Respondents spontaneously ranked money, unemployment, and housing as their most pressing concerns. When presented with a list of potential problems, housing, crime, and rent were most frequently cited. Health of children and own health were priorities for 15 and 21 respondents respectively. Pregnancy prevention and having a good pregnancy were important for seven individuals. The priorities attached by these medically and so
