Maxillary Advancement Osteotomy for Cleft Lip and Palate Sequelae: Impact of Orthodontic Space Closure of Missing Lateral Incisor on Morphologic and Occlusal Results.

BackgroundMaxillary hypoplasia is a common sequela of cleft lip and palate (CLP), often requiring surgical correction through maxillary advancement osteotomy. However, the resulting aesthetic outcome can be unpredictable. Frequently, these cases involve maxillary lateral incisor agenesis, managed either by preserving the space for dental rehabilitation or through orthodontic space closure.PurposeTo investigate the correlation between aesthetic and occlusal outcomes and the management of the missing maxillary lateral incisor space. Secondary objectives included assessing the relationship betwee

A patient with severe maxillary gingival exposure treated with combined compression osteogenesis of the anterior alveolar bone and conventional Le Fort I osteotomy.

Excessive gingival exposure at the maxillary anterior region during not only smiling (a gummy face) but also at rest creates both functional and aesthetic problems for patients. We herein introduce a unique treatment procedure for mandibular retrognathia with a gummy face. This procedure combines conventional Le Fort I osteotomy and following corticotomy at the anterior region of the maxilla. Subsequently, the anterior segment is continuously compressed (compression osteogenesis) in a posterior-superior direction until it reaches an ideal position. This procedure appears to safely and adequate

A 6-month histological analysis on maxillary sinus augmentation with and without use of collagen membranes over the osteotomy window: randomized clinical trial.

INTRODUCTION: Over the years, several modifications have been made to the sinus augmentation technique and to the materials used. However, there is still controversy about the need for using a barrier concurrently with a graft in sinus augmentation procedures. On this basis, the aim of this randomized clinical study was to investigate the effect of resorbable collagen membrane over the osteotomy window on maxillary sinus augmentation healing. MATERIALS AND METHODS: Patients who required maxillary sinus augmentation were evaluated and selected to enter the study. After maxillary sinus grafting

A patient-specific PEEK fixation system for simultaneous maxillary advancement and midfacial contour restoration in Le Fort I osteotomy: A finite element analysis.

BACKGROUND: Severe maxillary hypoplasia often requires combined skeletal repositioning and midfacial contour restoration. While Le Fort I osteotomy effectively addresses dental and skeletal discrepancies, residual midfacial deficiencies - such as infraorbital depression and paranasal flatness - frequently persist, necessitating additional soft tissue or alloplastic procedures. This study introduces a novel patient-specific polyetheretherketone (PEEK) fixation system designed to simultaneously stabilize Le Fort I-advanced maxillary segments and restore midfacial morphology in a single surgical

Maxillary surgical ciliated cysts following advancement Le Fort I osteotomy with concomitant autogenous bone grafting: a simple coincidence or a cause-effect relationship?

Surgical ciliated cysts (SCCs) are rare benign lesions that have been described after radical maxillary sinus surgery such as a Caldwell-Luc procedure for chronic sinusitis. They usually occur several years following surgery, and they typically present as an evolving swelling in the maxillary sulcus and cheek area. The pathogenetic mechanism is related to the entrapment of respiratory epithelium from the paranasal sinuses and/or the nose within the wound created by the surgical procedures. Radiologically, SCCs manifest as a well-demarcated, unilocular or multilocular radiolucency and histologi

Ankylosed maxillary incisor with severe root resorption treated with a single-tooth dento-osseous osteotomy, vertical alveolar distraction osteogenesis, and mini-implant anchorage.

Dentoalveolar ankylosis interferes with the vertical growth of the alveolar process, which can lead to an open bite, an unesthetic smile, and occlusal disharmony. This case report presents a new treatment protocol for an ankylosed tooth with severe root resorption using a combined surgical orthodontic management protocol involving a single-tooth dento-osseous osteotomy and vertical alveolar distraction osteogenesis with intraoral elastics and mini-implant anchorage. After distraction and active orthodontic treatment, the patient's anterior open bite was corrected. A Class I dental relationship

Posterior maxillary sandwich osteotomy combined with sinus grafting with bone morphogenetic protein-2 for alveolar reconstruction for dental implants: report of four cases.

Four patients underwent posterior sandwich osteotomy combined with sinus floor grafting using bone morphogenetic protein-2 and other grafting materials. The patients were treated over a period of 4 years. Two to four implants were placed in each site subsequently. Of the 12 implants placed, none failed. Alveolar crest bone levels appeared to be stable over time, with an average vertical gain of about 5 mm. Overall vertical gain, including the sinus graft, exceeded 13 mm in all patients. The procedure appears to hold promise for combined vertical alveolar defects and prominent pneumatization of

Midpalatal suture osteotomy combined with microimplant-assisted rapid palatal expansion for adult maxillary transverse deficiency treatment: a study protocol of a randomised controlled trial.

INTRODUCTION: Maxillary transverse deficiency (MTD) is a common type of malocclusion. For adult MTD patients with highly ossified midpalatal sutures, surgically assisted rapid palatal expansion (SARPE) has been the conventional method of maxillary expansion. However, SARPE has the disadvantages of significant trauma, high cost and a high incidence of adverse events. Therefore, we proposed a novel minimally invasive method for maxillary expansion, which is midpalatal suture osteotomy combined with microimplant-assisted rapid palatal expansion (MSO-MARPE). The results are expected to confirm MSO

Scleral exposure alterations following Le Fort I osteotomy (with and without maxillary impaction) in skeletal class III patients: A before-and-after clinical trial.

PURPOSE: Exposure of sclera below the iris in natural head positions is aesthetically undesirable. Studies on post-surgical changes in inferior scleral exposure following orthognathic surgery are scarce and mostly retrospective. The aim of this clinical trial is to examine the effect of Le Fort I osteotomy, a procedure for correction of malocclusion and maxillo-mandibular deformities, on the inferior scleral exposure and overall scleral surface area in skeletal class III patients. MATERIALS AND METHODS: This trial was performed on 40 eyes of 20 skeletal class III patients undergoing Le Fort I

Minimally invasive surgically assisted miniscrew anchored rapid palatal expansion (sa-MARPE) technique: avoiding conventional horizontal maxillary osteotomies - a technical note.

In skeletally mature patients, transverse maxillary deficiency poses a significant challenge due to the rigidity of craniofacial sutures. This technical note describes a minimally invasive, surgically assisted MARPE (sa-MARPE) protocol designed to overcome these limitations while avoiding complete Le Fort I osteotomies. The technique involves three small mucosal incisions and limited piezosurgical osteotomies targeting the midpalatal suture, canine pillars, and pterygomaxillary junctions. By preserving mucosal integrity and vascular supply, this approach reduces postoperative morbidity and enh

Nasal profile changes after LeFort I osteotomy (maxillary advancement) with and without ANS reduction in class III skeletal patients: a study protocol for a randomized clinical trial.

BACKGROUND: Dentofacial malformation is a common condition that affects a significant portion of the population, resulting in functional and aesthetic defects. Orthognathic surgeries, such as LeFort I osteotomy, are performed to correct these abnormalities. However, the impact of these surgeries on nasal profile changes remains unclear. Additionally, the role of anterior nasal spine (ANS) reduction in maxillary advancement surgeries of 3-5 mm range is yet to be determined. This study aims to investigate the effect of ANS reduction on soft tissue profile changes following LeFort I osteotomy wit
