Audits of clinical trials in India: a step in the right direction.

Random audits of clinical trial sites in India are due to start any time now. These regulatory measures come at a very crucial time when India's relatively young clinical research industry is poised for exponential growth. Can the industry scale up fast enough? Viveka Roychowdhury spoke to a cross section of industry experts to get their views.

Audit of diagnostic criteria for Trichomonas vaginalis in a genitourinary medicine clinic.

A comparison between high vaginal swab (HVS) wet prep examination within the genitourinary medicine (GUM) clinic and that made from an HVS transported in Amies charcoal medium to the laboratory for the diagnosis of Trichomonas vaginalis was made in a prospective study. Clinic wet prep compared with Amies had a sensitivity of 68% and specificity of nearly 100%. Using the clinic wet prep alone, 9/30 (30%) cases would have been missed.

Clinical audit as an educative tool for optometrists: an intervention study in age-related macular degeneration.

PURPOSE: Age-related macular degeneration (AMD) is a major cause of vision loss. This study investigated whether performing clinical audit and receiving analytical performance feedback altered documentation of the AMD care provided by optometrists. METHODS: Australian optometrists were recruited and completed a survey about their demographics and confidence in AMD care, and a three-month audit of their practice records using an AMD audit tool (termed the pre-audit evaluation). After receiving analytical feedback, participants identified areas for improvement and re-audited their practices aft

Screening and Telephone-Based Intervention for Unhealthy Alcohol Use in a Diverse Federally Qualified Health Center System in Texas.

BACKGROUND: Screening and brief intervention (BI) can reduce risky alcohol use but has not been widely implemented in primary care settings. We sought to implement a screening and telephone-based program within a Federally Qualified Health Center (FQHC). DESIGN: Prior to this program, adult patients were routinely screened using AUDIT-C with no further systematic follow-up. We designed a team-based, tele-care-centered program to (1) follow-up positive screening results or provider referrals with full AUDIT and (2) provide a social worker-administered two-session BI for patients with AUDIT 4-1

Utilisation of antithrombotic therapy for stroke prevention in atrial fibrillation in a Sydney hospital: then and now.

OBJECTIVE: Evidence from pivotal clinical trials conducted more than a decade ago supports the use of antithrombotic therapy, particularly warfarin, for stroke prevention in atrial fibrillation (AF). Despite the wide dissemination of this evidence since that time, there is anecdotal evidence that utilisation of therapy remains suboptimal, especially in the target elderly population, which is reflected in the development of practice tools such as the TAG Clinical Indicator ('Antithrombotics in AF' Indicator 1.6, 2007). Therefore, the objective of this study was to determine the current utilisat

A Quality Improvement Project to Assess if Preoperative Trauma Patients Are Receiving Their Prescribed Medications Preoperatively.

Introduction All preoperative trauma patients should receive their preoperative medications regardless of being nil per os (NPO). Anticoagulants, angiotensin-converting enzyme inhibitors (ACE-I), and angiotensin II receptor blockers (ARBs) should be omitted. This is according to both local and national guidelines. We noticed that some preoperative patients have not been receiving their morning medications prior to having their operations. This has led to pre and perioperative complications. The aim of this study, therefore, was to conduct a quality improvement project to assess if preoperative

Pitfalls of a Drug-Dispensing Audit Support System.

Dispensing audit support systems that authenticate pharmaceuticals via barcodes have been introduced into clinical practice to prevent dispensing errors, yet they occasionally generate false warnings, complicating clinical usage. This study sought to identify drugs prone to weight-error false warnings in the C-correct II® dispensing audit support system and to clarify its operational problems. We investigated the drugs that caused false weight-error warnings in the system, confirming that such errors did occur. Furthermore, the drugs causing weight errors tended to be significantly heavier tha

Diabetes mellitus self-management education at Jimma University Medical Center: evidence-based implementation project.

BACKGROUND: Low and middle-income countries are currently enduring the heaviest global burden of diabetes and diabetes-related mortalities. Diabetes self-management education (DSME) improves patient clinical outcomes, health status, and quality of life. Lack of awareness about best practice guidelines and recommendations may be associated with increased risk of diabetes morbidity and mortality. OBJECTIVES: The objective of this study was to improve knowledge and skill of healthcare providers and patients with diabetes by enhancing evidence-based practice through self-management education to i

Looking for a needle in the haystack? A case for detecting adverse drug events (ADE) in clinical notes.

Detection and prevention of adverse drug events (ADEs) forms a large part of the patient safety efforts in any healthcare organization. Detection of laboratory and medication based ADEs is well-studied through the use of rule-based systems, however identifying descriptive ADEs, such as somnolence, is a challenging problem. Signals for the detection of these ADEs can often only be found in free-text clinical notes. We describe the automated detection of descriptive ADEs in free-text clinician notes.

Association of unhealthy alcohol use reported in routine outpatient screening with 30-day hospital readmission risk.

BACKGROUND: Unplanned hospital readmissions within 30 days of hospital discharge are an important quality metric for hospital care. Characterizing the associations of alcohol use screening scores completed in outpatient care and risk of hospital readmissions may guide healthcare systems as they work to reduce unplanned readmissions. This retrospective cohort study examines the association between severity of alcohol use reported in outpatient settings and 30-day risk for unplanned hospital readmissions for patients in a large integrated health system. METHODS: Using claims records, we identif
