PURPOSE: An innovative certificate program aimed at expanding the rural pharmacy workforce, increasing the number of pharmacists with expertise in rural practice, and improving healthcare outcomes in rural North Carolina is described.
SUMMARY: Predicted shortages of primary care physicians and closures of critical access hospitals are expected to worsen existing health disparities. Experiential education in schools and colleges of pharmacy primarily takes place in academic medical centers and, unlike experiential education in medical schools, rarely emphasizes the provision of patient care in

The aetiology of community acquired pneumonia varies according to the region in which it is acquired. This review discusses those causes of CAP that occur in the tropics and might not be readily recognizable when transplanted to other sites. Various forms of pneumonia including the viral causes such as influenza (seasonal and avian varieties), the coronaviruses and the Hantavirus as well as bacterial causes, specifically the pneumonic form of Yersinia pestis and melioidosis are discussed.

PURPOSE: The Colorectal Cancer Control Program (CRCCP) aimed to increase colorectal cancer (CRC) screening among U.S. medically underserved populations through promotion and provision of CRC screening. We used simulation modeling to estimate the lifelong health impact and program cost-effectiveness of direct screening services, typically a single cycle of routine screening/follow-up testing provided through the CRCCP ("intervention").
METHODS: Data for this study were from CDC's Colorectal Clinical Data Elements (CCDE), which captured screening and follow-up services received from CRCCP betwe

UNLABELLED: Individuals living in rural or medically underserved areas (MUA) with chronic obstructive pulmonary disease (COPD) face significant barriers to specialised pulmonary care, including pulmonologists, diagnostic spirometry, and pulmonary rehabilitation. Remote spirometry for diagnostic screening and disease monitoring may mitigate access barriers and contribute to improved COPD management in this population. This study protocol describes the proposed implementation of a Mobile Health (mHealth) intervention using Bluetooth-enabled portable spirometry combined with a mobile disease mana…

BACKGROUND: While cervical cancer incidence rates (IR) in the United States have dropped in the last 20 years, non-cervical human papillomavirus (HPV) associated cancers increased. Many people in Texas (TX) live in medically underserved areas and have higher risk of developing HPV-associated cancers. Since previous studies of these regions focused on cervical cancer, we included other HPV-associated cancers in our analysis of IR in East TX and the TX-Mexico Border compared to other TX regions.
METHODS: Cancer data from 2006 to 2019 were obtained from the TX Cancer Registry. Cases of HPV-assoc…

OBJECTIVES: Highly effective direct-acting antivirals (DAAs) for Hepatitis C treatment are largely inaccessible in sub-Saharan Africa. Data on treatment feasibility and outcomes in clinical settings are limited. We assessed the feasibility of achieving a high (≥90%) cure rate with DAAs in six gastroenterology clinics in Cameroon.
METHODS: Patients with chronic Hepatitis C virus (HCV) infection were treated for 12 or 24 weeks with ledipasvir/sofosbuvir, ledipasvir/sofosbuvir/ribavirin or sofosbuvir/ribavirin, depending on the stage of liver disease and HCV genotype. The cure rate was defined a

OBJECTIVE: To evaluate the implementation of a programme to provide primary care physicians for remote and deprived populations in Brazil.
METHODS: The Mais Médicos (More Doctors) programme was launched in July 2013 with public calls to recruit physicians for priority areas. Other strategies were to increase primary care infrastructure investments and to provide more places at medical schools. We conducted a quasi-experimental, before-and-after evaluation of the implementation of the programme in 1708 municipalities with populations living in extreme poverty and in remote border areas. We com

Burkitt lymphoma is the most common malignancy in children in Malawi, the world's poorest country, where there is a long history of treating this disease using a 28-day cyclophosphamide-based protocol. Stage III/IV disease has had poor outcomes. In an attempt to improve the outcome for higher stage disease, anthracyclines were added to the existing protocol. The disease-free (DFS) and overall survival (OS) of 58 children with cytologically confirmed Burkitt lymphoma admitted during 2012-2014 and treated using this protocol were calculated. Six (10%) children had stage I disease, ten (17%) stag

Hypertensive intracerebral hemorrhage (HICH) causes significant morbidity and mortality. The time required to transport the patients to a specialized hospital can influence the prognosis. In the isolated islands in Nagasaki prefecture, there is no medical institution which can offer emergent neurosurgical intervention. We reviewed the cases of HICH in this region from January 2006 to September 2010, who were transferred to Nagasaki Medical Center by a helicopter after consultation via teleradiology. Eighty four cases (23%) were transferred via helicopter to our institution from isolated island

Although meningococcal disease is known to be linked to characteristics of individuals associated with social deprivation, there is only limited evidence of a relation with area-based measures of deprivation. In a small area geographical study, we ascertained 295 confirmed or probable cases occurring between 1996 and 1999 in the socially diverse resident population of Gwent Health Authority, equating to an average annual rate of 13.2 per 100,000. Incidence rates of meningococcal disease increased from 8.1 per 100,000 in the least deprived fifth of enumeration districts to 19.8 per 100,000 in t

BACKGROUND: Several programs have been designed to improve mammography screening participation in high-risk populations. The purpose of this cross-sectional study was to assess factors related to participation in a low-cost mammography screening voucher program.
METHODS: The methods involved mailing a standardized survey to 101 participants in a low-cost mammography screening voucher program. The survey was designed to yield responses about the use of free mammography vouchers, women's knowledge of breast cancer, their perceptions of their risk of getting the disease, the usefulness of mammog

INTRODUCTION: Non-physician community' (NPC) is a policy term that indicates a medically underserved area in Japan. Designated NPCs are politically targeted as the foci of medical resource allocation. NPC is defined as a specified district where 50 or more persons dwell within a geographic diameter of 4 km and medical care is not easily accessible. The definition of NPC was first introduced in 1960 and has been unchanged for more than half a century despite radical social changes in rural Japan. This study examines whether designated NPCs are still more disadvantaged in terms of geographical a…

Colorectal cancer (CRC) is the second most common cause of cancer deaths within the United States. Screening is effective in reducing the incidence and mortality of CRC; however, lower screening rates among the medically underserved have contributed to disparate outcomes in morbidity and mortality. Interventions to increase screening in this group include outreach at patient, community, provider, health system, and policy levels. In this article, we discuss the barriers and strategies to increase screening adherence in the medically underserved.

Cervical cancer, caused by HPV infection, is responsible for more than 311 000 preventable deaths every year. A global call to accelerate efforts to eliminate this disease has generated a new global strategy proposing ambitious, but achievable, targets for HPV vaccination of girls, and screening and treatment of women. The present paper addresses the suboptimal access to HPV vaccination in low-income and lower-middle-income countries (LICs/LMICs), where the burden of disease weighs most heavily, in part through co-infection with HIV. A proposed framework for action was formulated by first rev

In recent years, most of the focus on improving the quality of paediatric care in low-income countries has been on improving primary care using the Integrated Management of Childhood Illness, and improving triage and emergency treatment in hospitals aimed at reducing deaths in the first 24 hours. There has been little attention paid to improving the quality of care for children with chronic or complex diseases. Children with complicated forms of tuberculosis (TB), including central nervous system and chronic pulmonary TB, provide examples of acute and chronic multisystem paediatric illnesses t
