Compounded Complexity: The Impact of Social Determinants of Health on a Child With Cerebral Palsy and Lennox-Gastaut Syndrome.

Cerebral palsy (CP) is a common motor disorder in children; however, when combined with Lennox-Gastaut syndrome (LGS), it presents as a rare and highly complex condition requiring coordinated, multidisciplinary care. This case report highlights the challenges faced by a recently immigrated, uninsured child with limited financial resources, demonstrating how these barriers contributed to the progressive deterioration of her condition. It shows the significant impact of social determinants of health (SDoH) on the well-being and survival of children with complex medical needs. Addressing these ch

Beyond the Bedside: Addressing Structural Gaps in the Care of Undocumented, Seriously Ill Patients.

Undocumented patients face structural constraints to accessing life-sustaining treatments in the subacute setting due to gaps in health care coverage. We present the case of a 38-year-old Spanish-speaking woman with short bowel syndrome, dependent on prolonged parenteral nutrition after critical illness, who faced additional challenges as an undocumented and uninsured patient. Despite medical stabilization and a consistent desire to pursue life-prolonging care, discharge planning was complicated by a health care system not designed to support individuals without legal status or insurance. Factors Associated With Cost Barriers to Asthma Management by Insurance Coverage Status Among US Adults.

BACKGROUND: Asthma is a common chronic condition in the United States, yet despite insurance coverage, patients with asthma can potentially face cost barriers and limit access to care. OBJECTIVE: To investigate the predictors of cost barriers in asthma care and explore variations based on insurance coverage status. METHODS: We used 2019 to 2021 Behavioral Risk Factor Surveillance System Survey data and multivariable logistic regression to evaluate the relationship between insurance coverage (full year vs partial or no coverage) as study exposure and self-reported cost barriers (to primary ca

Impact of Transition from Compassionate to Scheduled Dialysis on Quality of Life for Patients at a Federally Qualified Health Center.

BACKGROUND: In the United States, uninsured patients with kidney failure often rely on emergency (compassionate) hemodialysis in acute care settings to manage life-threatening complications, as scheduled hemodialysis is often unattainable due to cost. This reactive approach has higher mortality rates, health care utilization, and cost than scheduled hemodialysis. Quality of life (QoL) outcomes are underexplored. This study evaluates changes in QoL after transitioning uninsured patients from emergency to scheduled hemodialysis. METHODS: Kidney Dialysis Quality of Life (KDQOL) surveys were admi

The impact of competing subsistence needs and barriers on access to medical care for persons with human immunodeficiency virus receiving care in the United States.

OBJECTIVES: To examine whether competing subsistence needs and other barriers are associated with poorer access to medical care among persons infected with human immunodeficiency virus (HIV), using self-reported data. DESIGN: Survey of a nationally representative sample of 2,864 adults receiving HIV care. MAIN INDEPENDENT VARIABLES: Going without care because of needing the money for food, clothing, or housing; postponing care because of not having transportation; not being able to get out of work; and being too sick. MAIN OUTCOME MEASURES: Having fewer than three physician visits in the pr

Implementing a Cancer Screening Patient Registry in an Urban Student-Run Free Clinic: A Model for Preventive Care Tracking.

Despite national guidelines promoting early cancer detection, adherence to screening recommendations remains below the USPSTF Healthy People 2030 targets. Nationally, only 71.8% of eligible adults receive colorectal cancer (CRC) screenings and 75.9% receive breast cancer (BC) screenings. Rates are significantly lower in uninsured or low-income populations, with just 7.5% and 35% receiving CRC and BC screenings, respectively. To improve presentation care delivery, a patient registry was developed at an urban, student-run free clinic serving the uninsured to track CRC and BC eligibility and scre

The Financial Toxicity of Hepatopancreatobiliary Cancer: An Analysis of the 1997-2023 National Health Interview Survey.

Patients with hepatopancreatobiliary (HPB) cancers may experience financial toxicity as a result of diagnosis and treatment. We characterized the prevalence and predictors of financial toxicity among United States (U.S.) HPB cancer patients using the National Health Interview Survey. Outcomes were delaying medical care due to cost or being unable to afford necessary medical care within the past 12 months. Prevalence was estimated in univariable analyses and sociodemographic predictors were identified in multivariable analyses. Among 5,630,270 U.S. adults with self-reported HPB cancer, 567,531

The Fragmentation of Specialty Care. The Role of Referral Location, Type of Referral, and Demographic Factors in Determining Completion Rates for Uninsured Patients in a Student-Run Free Clinic.

Little is known about how location of specialty care affects accessibility for uninsured patients at free clinics. To address this gap, the specialty referral completion rate by location and role of demographic and referral-specific factors were analyzed at an urban-based free clinic. A retrospective review was performed at a single site, student-run free clinic exclusively serving uninsured patients. Referral completion rate and predictors of referral completion were examined including age, race, ethnicity, sex, referral type, and location of referral. Significant predictors (p < 0.05) were i

Structural barriers to health care as risk factors for preterm and small-for-gestational-age birth among US-born Black and White mothers.

We develop county-level measures of structural and institutional barriers to care, and test associations between these barriers and birth outcomes for US-born Black and White mothers using national birth records for 2014-2017. Results indicate elevated odds of greater preterm birth severity for Black mothers in counties with higher uninsurance rates among Black adults, fewer Black physicians per Black residents, and fewer publicly-funded contraceptive services. Most structural barriers were not associated with small-for-gestational-age birth, and barriers defined for Black residents were not a

Time from breast cancer diagnosis to treatment among Idaho's National Breast and Cervical Cancer Early Detection Program population, 2011-2017.

PURPOSE: Idaho's Women's Health Check (WHC) Program provides breast and cervical cancer screening to under- and uninsured women via funding from the National Breast and Cervical Cancer Early Detection Program (NBCCEDP). Because WHC serves populations with less access to health care, this study evaluated time from breast cancer diagnosis to treatment for women enrolled in the WHC program and linked to Cancer Data Registry of Idaho (CDRI) case data (WHC-linked) and the remainder of female Idaho resident breast cases. METHODS: Among Idaho residents aged 50-64 years diagnosed during 2011-2017 wit

Presence of Any Medical Debt Associated With Two Additional Years of Homelessness in a Seattle Sample.

Although medical debt has been associated with housing instability, almost no research has connected homelessness to medical debt. We interviewed 60 individuals experiencing homelessness in Seattle, selected from those participating in self-governed encampments organized by a homeless advocacy organization. Most respondents reported having at least one kind of debt, with two-thirds reporting current medical debt. Almost half reported trouble paying medical bills for themselves or family members. Almost one-third believed medical debt was in part responsible for their current housing situation.

Women In The United States Experience High Rates Of Coverage 'Churn' In Months Before And After Childbirth.

Insurance transitions-sometimes referred to as "churn"-before and after childbirth can adversely affect the continuity and quality of care. Yet little is known about coverage patterns and changes for women giving birth in the United States. Using nationally representative survey data for the period 2005-13, we found high rates of insurance transitions before and after delivery. Half of women who were uninsured nine months before delivery had acquired Medicaid or CHIP coverage by the month of delivery, but 55 percent of women with that coverage at delivery experienced a coverage gap in the ensu

Trends in insurance coverage and treatment among persons with opioid use disorders following the Affordable Care Act.

PURPOSE: This short communication examines the impact of the Patient Protection and Affordable Care Act (PPACA) on insurance coverage and substance use treatment access among persons with opioid use disorders. METHODS: Data came from the 2010-2015 National Surveys on Drug Use and Health. Among persons with heroin and opioid pain-reliever use disorders, measures of insurance coverage and treatment access were compared before and after the implementation of major PPACA provisions that expanded access to insurance in 2014. RESULTS: The prevalence of uninsured persons among those with heroin use

Against colorectal cancer in our neighborhoods (ACCION): A comprehensive community-wide colorectal cancer screening intervention for the uninsured in a predominantly Hispanic community.

Colorectal cancer (CRC) is the second leading cause of cancer deaths in the USA. Screening is widely recommended but underutilized, particularly among the low income, the uninsured, recent immigrants and Hispanics. The study objective was to determine the effectiveness of a comprehensive community-wide, bilingual, CRC screening intervention among uninsured predominantly Hispanic individuals. This prospective study was embedded in a CRC screening program and utilized a quasi-experimental design. Recruitment occurred from Community and clinic sites. Inclusion criteria were aged 50-75years, unins

Breast cancer screening among low-income or uninsured women: results from the National Breast and Cervical Cancer Early Detection Program, July 1995 to March 2002 (United States).

OBJECTIVE: To describe the results of breast cancer screening among low-income and uninsured women in the only national organized screening program in the US, the National Breast and Cervical Cancer Early Detection Program (NBCCEDP). METHODS: We analyzed mammography and diagnostic follow-up data for 789,647 women who received their first mammogram in the NBCCEDP and 454,754 subsequent mammograms among these women. We calculated the rate of mammograms with abnormal findings, diagnostic follow-up, biopsy, and cancers detected per 1000 mammograms by age and racial or ethnic groups. Positive Pred

Neighborhood physical conditions and health.

OBJECTIVES: We explored the relationship between boarded-up housing and rates of gonorrhea and premature mortality. METHODS: In this ecological study of 107 US cities, we developed several models predicting rates of gonorrhea and premature death before age 65 from all causes and from specific causes. We controlled for race, poverty, education, population change, and health insurance coverage. RESULTS: Boarded-up housing remained a predictor of gonorrhea rates, all-cause premature mortality, and premature mortality due to malignant neoplasms, diabetes, homicide, and suicide after control for

Predictors of MOUD referral among persons with involvement in the criminal legal system.

INTRODUCTION: Opioid use disorder (OUD) is a significant cause of opioid-related fatality, and while medications to treat OUD (MOUD) are effective, disparities remain in the access and uptake of such medications. This study investigated factors that may influence referral to and initiation of MOUD treatment. METHODS: Data from electronic medical records of 677 patients with a history of criminal legal system involvement in a recovery program were used to examine the flow of MOUD referral. RESULTS: Among patients identified as potentially eligible for MOUD treatment, about 38.0 percent were r

The impact of insurance status on psoriasis patients' healthcare-seeking behavior: a population-based study in the United States.

BACKGROUND: Psoriasis is a chronic, inflammatory skin condition requiring long-term care. However, many psoriasis patients may not regularly receive care. Several factors affect access to care in the United States, including health insurance status. Additionally, it is unknown how health insurance status impacts the healthcare-seeking behavior of psoriasis patients. Healthcare-seeking behavior is broadly defined as an individual's actions to prevent or treat a perceived health problem, such as visiting a physician's office. Because early diagnosis and timely treatment improve patient outcomes,
