Appearance of effusion material in the attic space correlated to an impaired Eustachian tube function.

In adult Sprague-Dawley rats a dysfunction or a total blockade of the Eustachian tube was established by various experimental procedures. The appearance of effusion material in the attic was subsequently considered to be evidence of incomplete ventilation of the middle ear cavity. Though the salpingopharyngeus muscle (SPM) seemed to be the muscle which could open the Eustachian tube maximally, splitting of the soft palate and consequent interference with the tensor veli palatini muscle (TVPM) and the levator veli palatini muscle (LVPM)--but obviously not with the salpingopharyngeus muscle (SPM

Comparison of Three Methods Used in the Diagnosis of Extraesophageal Reflux in Children with Chronic Otitis Media with Effusion.

Objectives. Detection of extraesophageal reflux (EER) in children with chronic otitis media with effusion (OME) using three different diagnostic methods. Methods. Children between 1 and 7 years with OME who underwent adenoidectomy and myringotomy with insertion of a ventilation tube were included in this prospective study. EER was detected using three methods: oropharyngeal pH was monitored for 24 hours using the Restech system; detection of pepsin in middle ear fluid obtained during myringotomy was done using Peptest, and detection of pepsin in an adenoid specimen was done immunohistochemical

IgG4-Related Disease Involving the Ear: A Case Report.

IgG4-related disease is a chronic inflammatory disease with widespread clinical presentation. It mimics various malignant, infectious, and inflammatory conditions, leading to confusion in diagnosis and management. Otological manifestations, though relatively rare, can lead to significant complications. A 45-year-old male with a recent history of ventilation tube placement in the right ear presented with a sensation of imbalance associated with profound hearing loss. He was managed in line of acute otitis media with labyrinthitis with steroids and antibiotics and removal of the ventilation tube

The Efficacy of Adjuvant Intratympanic Steroid Treatment for Otitis Media with Effusion in Children.

INTRODUCTION: Otitis media with effusion (OME) is a leading cause of hearing impairment in children. Therefore, early and proper management is essential. OBJECTIVES: The objective of this research is to assess the efficacy of intratympanic (IT) steroids injection for management of otitis media with effusion (OME). METHODS: This study involved 42 children (84 ears) with bilateral OME. We used tympanometry to confirm the childreńs middle ear effusion and pure tone audiometry to determine hearing threshold. We performed myringotomy and inserted ventilation tubes (VTs) bilaterally, followed by a

Hearing impairment in ventilator-dependent infants and children.

Bronchopulmonary dysplasia is a complication of prematurity that can require months or years of ventilatory support. Among a cohort of 12 such ventilator-dependent patients, half had auditory brainstem response thresholds of 40 dB nHL or worse in the better ear. In 10 of 23 ears the threshold was 50 dB or worse. This incidence of hearing impairment is much greater than for other high-risk premature infants. Although conductive impairment was implicated in many cases, the degree of impairment is greater than that ordinarily seen with middle ear effusion.

[Eustachian tube and middle ear mechanics].

Even the most sophisticated prostheses for reconstruction of the ossicular chain do not work in the presence of Eustachian tube dysfunction. This review gives an update on the mechanisms of middle ear pressure regulation and middle ear ventilation, as well as methods for measuring the opening and closing function of the Eustachian tube. So far, in most tube function tests pressures are applied far beyond the physiological range in order to open the tube or force it to open. New methods like sonotubometry with perfect sequences (PSEQ) or the application of pressure in the nasopharynx with the E…

[Acute otitis media in childhood].

Because of its distinct anatomy, pathogens can quickly reach the middle ear of a child and cause acute otitis media. Depending on the age of the child, the clinical symptoms can vary from intense earaches, fever, pressure sensation and hearing loss to vomiting, diarrhea and refusal of food by infants. The progression of otitis media through four phases can be observed in an otoscopic examination. To improve Eustachian tube ventilation in uncomplicated cases, nose drops to reduce swelling along with pain therapy are employed first. Antibiotics can be administered to reduce the rate of complicat

Allergy skin testing under general anesthesia with treatment response in ninety-two patients with chronic serous otitis media.

This is a study of Ninety-two patients with chronic serous otitis media, all of whom have been previously treated with myringotomy and middle-ear ventilation tubes. Intradermal skin testing was done by the Rinkel technique under general anesthesia in conjunction with myringotomy and associated surgical procedures. Twenty of these patients were tested in the office one week later with little or no change in their intradermal test results. Eighty-four patients were managed with a hyposensitization program. Of these, fifty-seven have their tubes out with no fluid recurrence, twelve continue to ha

[Eustachian tube drug delivery to the tympanum in patients with otitis media with effusion].

There are no reliable data on the volume of the drug reaching the affected tympanic cavity during the catheterization of Eustachian tube (ET). Objective: to conduct an experiment and to study the effectiveness of administering a drug into the tympanic cavity of the middle ear using an original ear catheter, which allows catheterization of ET in the direction of its lumen. The study included patients with unilateral OME (n=21, the duration of the disease from 2 to 3 months), who were recommended for the ventilation tube insertion. The main inclusion criterion was 3 degree of patency of the ET.

[FOCUSING AND UPDATING INDICATIONS FOR VENTILATING TUBES INSERTION IN PEDIATRIC COCHLEAR IMPLANT CANDIDATES WHO SUFFER FROM OTITIS MEDIA].

BACKGROUND: After cochlear implantation (CI) there is concern regarding the potential risks of spread of middle ear infection along the electrode array into the cochlea and central nervous system and regarding late sequela of otitis media (OM): eardrum perforation, atelectasis and cholesteatoma. The age for implantation in children overlaps the peak age incidence of acute OM (AOM) and secretory OM (SOM) and delay of implantation reduces the potential benefit from the intervention. Therefore, control of OM by inserting ventilating tubes (VT) is widely performed in pediatric CI candidates who al

Retraction cholesteatoma of the sinus tympani.

Posteromedial retraction of the tympanic membrane, between the oval window superiorly and the round window niche inferiorly, results in the … posteromedial to the tympanic annulus and anterior to the vertical portion of the facial nerve, and middle ear ventilation after marsupialization of the epithelial retraction. Although early tympanic membrane retraction can be treated with a ventilation tube, deep epithelial pockets may require additional surgical treatment. A method for the management of sinus tympanic …

Transtympanic Hearing Aid: exploratory study on a new device.

In this paper, we present the preliminary results achieved with a transtympanic hearing aid (THA). This is a modified digital, open-fit external hearing aid (HA) designed for acute study only, which allows coupling with a pre-implanted ventilation tube. The THA conveys amplified sound directly onto the round window, bypassing the ossicular chain, in contrast with traditional HAs that convey sound onto the second or third portion of the external auditory canal. The THA has been developed as an alternative to standard HAs and active middle ear implants for patients who are unsatisfied with tradi

Experience with tympanoplasty for atelectatic ears.

Atelectasis is, unfortunately, still a common sequela of otitis media with effusion (OME)--including serous otitis media (SOM) and mucoid otitis media (MOM). When atelectatic ears provide a functional deficit, i.e., a conductive loss of sufficient magnitude or a residual conductive loss after aerating the middle ear with ventilation tube or early or strongly suspected attic cholesteatoma, a simple corrective procedure has been used which has stood the test of time. The procedure as well as experience with 51 patients (60 ears) will be described and discussed. The procedure consists of re-estab

Inflammation in the middle ear of children with recurrent or chronic otitis media is associated with bacterial load.

BACKGROUND: Viral upper respiratory tract infections have been described as an important factor in the development of otitis media (OM), although it is unclear whether they facilitate bacterial OM or can directly cause OM. To clarify the role of viral infections in OM, we compared the relative contribution of viruses and bacteria with the induction of inflammatory cytokine responses in the middle ear of children suffering from OM. METHODS: Children up to 5 years of age, with recurrent or chronic episodes of OM and scheduled for ventilation tube insertion were enrolled in a prospective study.
