Disseminated Herpes Zoster Virus: A Severe Case Complicated by Radicular Mononeuropathy in a Clinically Immunocompetent Individual.

Herpes zoster incidence and severity increase with age due to immunosenescence. While typically presenting as a unilateral dermatomal rash, severe forms such as disseminated zoster and segmental zoster paresis can occur, especially in high-risk individuals. Despite the availability of a highly effective recombinant zoster vaccine, vaccination rates remain low in the United States. This case report is significant due to the presentation of rare complications, disseminated herpes zoster and segmental zoster paresis, in an elderly patient with chronic kidney disease (CKD), underscoring the clinic

Distal Tibial Mononeuropathy From Compression by the Posterior Tibial Artery: Clinical-Electrophysiological-Ultrasonographic Correlations.

The tibial nerve is bound tightly to the posterior tibial artery in the tarsal tunnel where expansion capacity is limited. Therefore, the nerve may be vulnerable to, and damaged by chronic pulsatile trauma from an atypically positioned overriding artery, labeled "punched-nerve syndrome". In this article, we present a 49-year-old woman who presented with two months of severe burning pain in the left medial ankle and sole of the foot without antecedent trauma. Neurological examination identified dysesthetic sensation to light touch in the left medial sole of the foot, and both active and passive

The effect of pregabalin - codeine combination on partial sciatic nerve ligation - induced peripheral mononeuropathy in rats.

The present study investigates the effects of pregabalin (PGB) and codeine (COD) combination on neuropathic hyperalgesia in an animal model of peripheral nerve injury represented by partial sciatic nerve ligation. Hot plate and analgesimeter tests were performed to evaluate the influence of PGB, COD and their combination on thermal and mechanical hyperalgesia in the hind paw with partial sciatic nerve ligation. Reactivity was evaluated by measuring the latency to withdrawal of the operated hind paw from the noxious heat and pressure stimulation. Nociceptive thresholds were evaluated before (ba

Complex regional pain syndrome type I (RSD) or peripheral mononeuropathy? A discussion of three cases.

OBJECTIVE: Peripheral nerve pathology commonly results in symptoms that suggest a diagnosis of complex regional pain syndrome (CRPS) type I (RSD). We briefly review common symptoms of peripheral nerve pathology (referred pain, hyperpathia, and autonomic changes) and present three illustrative cases of peripheral nerve injury misdiagnosed and treated as RSD. The nonspecificity of current taxonomy regarding CRPS as it relates to the three cases is emphasized. DESIGN: The study is case series. SETTING: All three of the cases were diagnosed and treated for their painful symptoms at a university

Pre-emptive administration of clonidine prevents development of hyperalgesia to mechanical stimuli in a model of mononeuropathy in the rat.

Traumatic nerve injury in man can often result in the development of neuropathy. An animal model of neuropathic hyperalgesia is produced by loose ligation of the sciatic nerve in the rat. We have examined the effect of pre-emptive treatment with a number of drugs on the development of hyperalgesia in this model. Animals received clonidine (1 mg.kg-1, s.c.), morphine (5 mg.kg-1, s.c.), (+/-)-baclofen (40 mg.kg-1, s.c.), carbamazepine (50 mg.kg-1, s.c.) or vehicle (4 ml.kg-1, s.c.) 30 min prior to loose ligation or sham-operation. Morphine- and clonidine-treated animals were administered a secon

The role of the saphenous nerve in experimental sciatic nerve mononeuropathy produced by loose ligatures: a behavioural study.

Four loose ligatures were tied round the sciatic nerve of rats to produce the model of altered pain sensation first described by Bennett and Xie (1988). Hyperalgesia and hyperaesthesia were detected from 4 days after tying ligatures, becoming maximal after 14 days; normal behaviour returned by 8 weeks post-operation. Using thermal tests involving immersion of the whole foot, it was found that section of the saphenous nerve at the time of, or within a week of, placing ligatures had the effect of abolishing the hyperaesthetic behaviour and instead caused hypoaesthesia from the 4th to 10-12th day

5-HT1A receptor-mediated attenuation of heat hyperalgesia and mechanical allodynia by chrysin in mice with experimental mononeuropathy.

BACKGROUND: Persistent neuropathic pain poses a health problem, for which effective therapy or antidote is in dire need. This work aimed to investigate the pain-relieving effect of chrysin, a natural flavonoid with monoamine oxidase inhibitory activity, in an experimental model of neuropathic pain and elucidate mechanism(s). METHODS: Chronic constriction injury (CCI) was produced by loose ligation of sciatic nerve in mice. The pain-related behaviors were examined using von Frey test and Hargreaves test. The serotonin-related mechanisms were investigated by serotonin depletion with p-chlorophe

Bilateral subcutaneous pyelovesical bypass in a Hautmann neobladder followed by a mononeuropathy multiplex and an underlying polyarteritis nodosa diagnosis.

Subcutaneous pyelovesical bypasses are the best choice for the long-term palliative treatment of ureteral obstructions. In rare cases this obstruction is due to polyarteritis nodosa. We present the only reported patient with a bilateral Detour bypass in a Hautmann's neobladder. The patient also suffers from polyarteritis nodosa.

Use of contrast-enhanced sonography to investigate intraneural vascularity in a cohort of Macaca fascicularis with suspected median mononeuropathy.

OBJECTIVES: The purpose of this study was to provide clinical evidence of the use of contrast-enhanced sonography in detecting and quantifying changes in intraneural vascularity due to median mononeuropathy. METHODS: Five Macaca fascicularis monkeys were exposed to 20 weeks of repetitive work to increase their risk of developing median mononeuropathy. Contrast-enhanced sonograms were obtained in 30-second increments for 7 minutes while a contrast agent was being delivered. Data were collected immediately at the conclusion of the 20-week work exposure and then again during a recovery phase app

Pain relief by spinal cord stimulation involves serotonergic mechanisms: an experimental study in a rat model of mononeuropathy.

The aim of the present study was to examine the role of the spinal serotonergic system in the pain relieving effect of spinal cord stimulation (SCS) using a rat model of mononeuropathy. Tactile withdrawal thresholds, cold responses and heat withdrawal latencies were assessed before and after SCS. In some rats, SCS produced an attenuation of the hypersensitivity following nerve injury (SCS responding rats). When SCS was applied immediately prior to sacrifice, the 5-HT content in the dorsal quadrant of the spinal cord ipsilateral to the nerve injury was increased in SCS responding rats. But ther

Peri-administration of clonidine or MK801 delays but does not prevent the development of mechanical hyperalgesia in a model of mononeuropathy in the rat.

Following loose ligation of the sciatic nerve, rats develop a persistent hyperalgesia which mimics some of the features of traumatic neuropathy seen in man. Previously, we have shown that administration of MK801 or clonidine prior to and 30 min following loose ligation of the sciatic nerve prevented the development of hyperalgesia up to 30 days following surgery. In the current study, we have extended our observation and examined the effect of administration of clonidine (1 mg/kg, s.c.) or MK801 (0.3 mg/kg, s.c.) 30 min prior to and 6 h following loose ligation of the sciatic nerve on the deve

Extra-territorial pain in rats with a peripheral mononeuropathy: mechano-hyperalgesia and mechano-allodynia in the territory of an uninjured nerve.

The abnormal pain sensations that accompany peripheral neuropathies are sometimes found in a distribution that does not coincide with the territories of nerves or posterior roots. This 'extra-territorial' pain is one of the lines of evidence that has been advanced to support the proposal that these patients suffer from a psychogenic disorder. In the present experiments, rats were prepared with a unilateral chronic constriction injury (CCI) to the sciatic nerve. Beginning on the first postoperative day and continuing for at least 18 days, exaggerated withdrawal reflexes to pinprick stimulation,

Intrathecal treatment with dextrorphan or ketamine potently reduces pain-related behaviors in a rat model of peripheral mononeuropathy.

The therapeutic effects of dextrorphan and ketamine, two non-competitive N-methyl-D-aspartate (NMDA) receptor antagonists, on neuropathic pain-related behaviors were examined in rats with peripheral mononeuropathy induced by loose ligation of the common sciatic nerve (chronic constrictive injury, CCI). Four daily intrathecal treatments (beginning 1 h after nerve ligation) with dextrorphan or ketamine (12.5-100 nmol) reliably attenuated hyperalgesia to radiant heat and spontaneous pain-related behaviors in CCI rats. Thermal hyperalgesia also was reduced in CCI rats receiving a single intratheca

[Anti-NF155 antibody positive autoimmune nodopathy presenting as multiple mononeuropathy with predominantly superficial sensory disturbances].

Anti-NF155 antibody positive autoimmune nodopathy typically presents with symmetric deep sensory disturbance, muscle weakness and tremor. Here, we report a unique case of anti-NF155 antibody positive autoimmune nodopathy presenting as multiple mononeuropathy with predominantly superficial sensory disturbances. A 35-year-old woman presented with progressive superficial sensory disturbances on the right side of her tongue and lip, the left lower limb, the right toes, and the right forehead over a period of 4 years. There was no deep sensory disturbance, muscle weakness, or tremor. Cerebrospinal

Tibial Nerve Mononeuropathy Post-COVID-19 and Successful Management with Peripheral Nerve Stimulation: A Case Report.

BACKGROUND: Infection with COVID-19 has evolved from a novel health crisis into an entity with a myriad of increasingly recognized sequelae. Among these, peripheral neuropathy is often an underrecognized and underdiagnosed complication. Neuropathic pain can be challenging to treat with many cases being refractory to conservative management, often requiring advanced techniques. Neuromodulation has become an important tool for the treatment of the refractory cases. CASE REPORT: We present a case of a 43-year-old woman with refractory right tibial mononeuropathy secondary to COVID-19 infection w
