[New antifungal agents and bronchopulmonary mycoses].

The frequency of respiratory mycosal infections has increased over recent Years. Diagnosis has been improved by recent epidemiological data and advances in radiological and mycological diagnostic methods. Two new antifungal agents have recently received marketing approval: voriconazole and caspofungine. Voriconazole belongs to the echinocandin family of antifungals. Sites of action of antifungals have become more diversified: amphotericins act on ergosterol directly, azolated agents act on the synthesis of ergosterol, flucytosine affects synthesis of nucleic acids, and echinocandins alter the

Antifungal drugs used for systemic mycoses.

Table 1 summarizes the antifungal agents commonly used for the treatment of systemic mycoses. The treatment of these conditions remains a challenge. This is particularly the case with infections in neutropenic patients where mortality rates remain high despite the development of new drugs and the evaluation of drug combinations. Much attention has been focused on the development of new antifungal drugs that can provide a solution to this problem without concurrent toxicity and without the risk of drug resistance.

[Epidemiology of deep-seated, domestic mycoses].

The opportunistic character of deep-seated mycoses depends on granulocyte-based defense in candidosis and aspergillosis. Therefore, haematological, patients represent the group of highest risk. Mucocutaneous candidosis is controlled by macrophages. Cryptococcus neoformans forces its way into the human host via causing an imbalance in the CD8-T-cell suppressor system. An aggravating synergism exists between Cryptococcus invasion and HIV-infection which explains the severe course of cryptococcosis in AIDS patients. The following pathways of transmission are observed in opportunistic mycoses: In …

[Systemic mycoses. Epidemiology, clinical aspects, diagnosis].

Systemic mycoses are serious, often life-threatening, infections that are encountered with increasing frequency in many countries. These infections are most often seen as a result of advanced medical treatment leading to profoundly immuno-suppressed patients. In Norway, most systemic mycoses are caused by Candida, followed by Aspergillus, Cryptococci and Mucorales. This paper deals with the pathogenetic and immunologic mechanisms underlying these infections. The different agents are described, and the various clinical manifestations. A discussion of the diagnosis of systemic mycoses is followe…

Endemic mycoses: epidemiology and diagnostic strategies.

INTRODUCTION: The global frequency of endemic mycoses has considerably increased, mainly due to environmental changes, population growth in endemic areas, and the increase in HIV-related immunosuppressed status. Among the most frequent endemic mycoses are coccidioidomycosis in semi-desert climates, and paracoccidioidomycosis, and histoplasmosis in tropical climates. The inoculum can enter the host through the airway or directly through the skin. Lymphatic and hematogenous spread may involve the skin. AREAS COVERED: In this article, we provide up-to-date epidemiological and diagnostic data on …

Subcutaneous mycoses: chromoblastomycosis, sporotrichosis and mycetoma.

Subcutaneous mycoses are common in subtropical and tropical regions of the world. They are rarely observed in Europe. These mycoses are heterogeneous, but all are caused by penetrating trauma of the skin. Most cases in Europe are observed in returning travelers, aid workers, archaeologists and immigrants. Therefore, a careful, thorough history is essential in order to reach a proper diagnosis. We provide up-to-date epidemiological, clinical, diagnostic, and therapeutic data on the three most important imported subcutaneous mycoses in Europe: chromoblastomycosis, sporotrichosis and mycetoma.

New aspects of some endemic mycoses.

The treatment of mycetomas varies according to their etiological agents and the clinical state of the patient. For the treatment of eumycetomas, the azole derivatives are the drugs of choice, with itraconazole rendering better results than ketoconazole and presenting better tolerance. Actinomycetomas are treated according to different therapeutic schemes: dapsone plus sulfamethoxazol-trimethoprim (SMT), and streptomycin or amikacin or amoxicillin plus clavulanic acid. The first therapeutic scheme is very useful in the treatment of Nocardia mycetoma, while the association of amikacin plus SMT i

Host-parasite relationship in opportunistic mycoses.

Aspergillosis and mucormycosis are opportunistic fungal infections that share several unique features. The etiologic agents of aspergillosis and mucormycosis are ubiquitous in the environment, but are opportunistic organisms and usually infect only patients predisposed by some underlying disease or treatment. These infections are typically characterized by hyphal tissue invasion and a predilection of the organism for blood vessel invasion with hemorrhage, necrosis, and infarction. Also, these organisms are not dimorphic, like the true pathogenic dimorphic fungi, as they grow both in the enviro
